A clinico-epidemiological study of rescuer burns.

Rescuer burn is a relatively newer terminology introduced to define the burns sustained by a person attempting to rescue a primary burn victim. Few studies have been published thus far on this peculiar type of burns. Due to the general neglect of the rescuer burns victim and discontinuation of treatment in most cases, once the primary victim dies, the rescuer often ends up in badly infected wounds and has a delayed return to work. A prospective study was conducted at the B J Medical College and Civil Hospital, Ahmedabad from January 2009 to December 2012 on the rescuer burns patients treated i

[Prophylaxis of CRPS I and recurrent CRPS I].

In the foreground of the avoidance of a CRPS as well as a recurrent CRPS, also pathophysiological considerations and use-possibilities of different drugs stand beside the observance of generally valid principles for treatment. Its consistent transposition cannot certainly prevent the CRPS in each case admittedly, the installment of the appearance or recurrence of a CRPS clearly lowers however.

Escharotomies, fasciotomies and carpal tunnel release in burn patients--review of the literature and presentation of an algorithm for surgical decision making.

Escharotomies are usually performed in patients with circumferential third degree burns of the extremities or anterior trunk. Fasciotomies are recommended for patients who sustained high voltage (or associated crush) injuries, with entrance or exit wounds in one or more extremities. Carpal tunnel release is practiced routinely in some services for cases of electrical injury. We have reviewed the literature which provides relatively little information as to when should these procedures actually be performed and what would happen if they were not done. We present a series of patients treated at

Trauma patients with multiple extremity injuries: resource utilization and long-term outcome in relation to injury severity scores.

BACKGROUND: Trauma patients with multiple extremity injuries (MEI) make heavy demands on hospital resources and face long-term difficulties in rehabilitation, yet the literature contains little about their treatment as a distinct group. METHODS: In this study, a cohort of 54 patients with MEI, all treated at a Level I trauma center, was compared with a trauma control (TC) group that had major injuries not focused at the extremities (but excluding patients with neurologic sequelae of head or spinal cord injuries). Demographic features, primary measures reflecting utilization of hospital resour

[The floating joint injury of the lower and upper extremity--epidemiology, therapy and results in 40 extremities].

GOAL: Floating Joint Injuries (FJI) are resulting from high energy traumas and are often combined with additional neuro/vascular damage. The high incidence of severe open or closed soft tissue injuries is complicating the initial management and requires a broad surgical know-how also in minimal-invasive fixation techniques. In a retrospective analysis of our cases treated between 1980 and 1995, we try to find out some important therapeutical feedback for the future. MATERIAL AND METHODS: Of the 37 patients, 33 had a FJI of the lower (2 bilateral) and 7 patients of the upper extremity. 90% wer

Leg length preservation with pedicled fillet of foot flaps after traumatic amputations.

Six patients with insufficient soft-tissue coverage after lower limb trauma were treated with pedicled fillet of foot flaps to achieve primary stump closure and to preserve leg length. The flaps used were all based on either the posterior tibial neurovascular pedicle, the anterior tibial neurovascular pedicle, or both. Five flaps survived; one patient required conversion of a through-knee to an above-knee amputation and debridement of the flap because of venous thrombosis of the pedicle. In three of the cases, a functional knee joint was preserved. The patients ranged in age from 21 to 54 year

[Vascular injuries in everyday practice].

BACKGROUND: It was the objective of this retrospective study to analyse the causes of injury, surgical approaches, outcome, and complications in patients with vascular trauma and to report our experience with vein homografts for arterial reconstruction in the upper and lower limbs and cervicothoracic region in patients operated on over a period between 1981-2001. PATIENTS: In 128 patients with peripheral arterial injuries the mechanism was direct penetration in 90 cases and blunt injury in 20 cases. In 4 cases a chronic damage (false aneurysm, AV fistula) was observed. Isolated vascular traum

[Evaluation of anterior cruciate ligament reconstruction using anterior tibial muscle tendon allograft].

PURPOSE OF THE STUDY: Injury to the anterior cruciate ligament (ACL) frequently results in the development of knee instability. This is managed by ACL reconstruction using autogenous or allogeneic grafts. This study presents a comprehensive evaluation of the patients with ACL reconstruction using a tendon allograft harvested from the anterior tibial muscle (ATM). MATERIAL AND METHODS: The group under study comprised 53 patients, 33 men and 20 women, with unilateral ACL reconstruction while the other knee was intact. The average age was 32.4 years (17 to 51) and the average follow-up was 33,4

Meniscal Repair Achieves Comparable Mid-Term Functional Performance to Partial Meniscectomy Following Anterior Cruciate Ligament Reconstruction in Highly Active Patients.

OBJECTIVE: Concomitant meniscal injuries accompany 40%-60% of anterior cruciate ligament (ACL) ruptures. Surgeons often face a clinical choice between meniscal repair, which prioritizes long-term joint preservation, and partial meniscectomy, which is traditionally perceived to facilitate earlier functional recovery. Despite the biological advantages of repair, persistent concerns remain that its restrictive postoperative protocols might impede mid-term functional performance compared to meniscectomy. This study aims to compare quadriceps/hamstring strength, dynamic balance, functional hop perf

Standard RTS criteria effectiveness verification using FMS, Y-balance and TJA in footballers following ACL reconstruction and mild lower limb injuries.

The anterior cruciate ligament (ACL) is the most frequently injured knee ligament. In previous studies, it was demonstrated that patients following ACL reconstruction may present motor deficits which may be related to increased risk of injury. The objective of the study was to determine whether players who have passed RTS assessment still have deficits in movement patterns or in neuromuscular control after such a serious injury as ACL rupture and reconstruction,. Sixty-five male football players (age 18-25 years), recruited from regional teams, were divided into three groups: (1) ACL group-sub

First human experience with autologous Schwann cells to supplement sciatic nerve repair: report of 2 cases with long-term follow-up.

OBJECTIVE Long-segment injuries to large peripheral nerves present a challenge to surgeons because insufficient donor tissue limits repair. Multiple supplemental approaches have been investigated, including the use of Schwann cells (SCs). The authors present the first 2 cases using autologous SCs to supplement a peripheral nerve graft repair in humans with long-term follow-up data. METHODS Two patients were enrolled in an FDA-approved trial to assess the safety of using expanded populations of autologous SCs to supplement the repair of long-segment injuries to the sciatic nerve. The mechanism

Biomechanical Comparison of 3 Current Ankle Syndesmosis Repair Techniques.

BACKGROUND: Significant debate exists regarding optimal repair for unstable syndesmosis injuries. Techniques range from screw fixation, suture-button fixation, or a combination of the two. In this study, 3 common repairs were compared using a simulated weightbearing protocol with internal and external rotation of the foot. METHODS: Twenty-four lower leg specimens with mean age 54 years (range, 38-68 years) were used for testing. Following creation of a complete syndesmotic injury (AITFL, ITFL, PITFL, interosseous membrane), specimens were repaired using 1 of 3 randomly assigned techniques: (1

Elastic nailing of tibia shaft fractures in young children up to 10 years of age.

INTRODUCTION: Although tibia shaft fractures in children usually have satisfactory results after closed reduction and casting, there are several surgical indications, including associated fractures and soft tissue injuries such as open fractures. Titanium elastic nails (TENs) are often used for pediatric tibia fractures, and have the advantage of preserving the open physis. However, complications such as delayed union or nonunion are not uncommon in older children or open fractures. In the present study, we evaluated children up to 10 years of age with closed or open tibial shaft fractures tre
