Modeling limbic influences on habituation deficits in chronic tinnitus aurium.

About 93% of healthy subjects suffer from tinnituslike symptoms when deprived of auditory stimuli, e.g., in a sound-proof chamber. This suggests an underlying physiological mechanism causing auditory sensations during absence of an external sound source. Grossberg suggested a mechanism by which hallucinations arise from mechanisms of learning, attention and volition. According to this mechanism notch-like hearing deficits are sufficient for experiencing auditory hallucinations, while their chronification is attributed to reorganization processes. In tinnitus sufferers the auditory sensation is

A mixed model framework for teratology studies.

A mixed model framework is presented to model the characteristic multivariate binary anomaly data as provided in some teratology studies. The key features of the model are the incorporation of covariate effects, a flexible random effects distribution by means of a finite mixture, and the application of copula functions to better account for the relation structure of the anomalies. The framework is motivated by data of the Boston Anticonvulsant Teratogenesis study and offers an integrated approach to investigate substantive questions, concerning general and anomaly-specific exposure effects of

A probit latent class model with general correlation structures for evaluating accuracy of diagnostic tests.

Traditional latent class modeling has been widely applied to assess the accuracy of dichotomous diagnostic tests. These models, however, assume that the tests are independent conditional on the true disease status, which is rarely valid in practice. Alternative models using probit analysis have been proposed to incorporate dependence among tests, but these models consider restricted correlation structures. In this article, we propose a probit latent class model that allows a general correlation structure. When combined with some helpful diagnostics, this model provides a more flexible framewor

Epidemic dynamics at the human-animal interface.

Few infectious diseases are entirely human-specific: Most human pathogens also circulate in animals or else originated in nonhuman hosts. Influenza, plague, and trypanosomiasis are classic examples of zoonotic infections that transmit from animals to humans. The multihost ecology of zoonoses leads to complex dynamics, and analytical tools, such as mathematical modeling, are vital to the development of effective control policies and research agendas. Much attention has focused on modeling pathogens with simpler life cycles and immediate global urgency, such as influenza and severe acute respira

Quantitative risk assessment of Cryptosporidium in tap water in Ireland.

Cryptosporidium species are protozoan parasites associated with gastro-intestinal illness. Following a number of high profile outbreaks worldwide, it has emerged as a parasite of major public health concern. A quantitative Monte Carlo simulation model was developed to evaluate the annual risk of infection from Cryptosporidium in tap water in Ireland. The assessment considers the potential initial contamination levels in raw water, oocyst removal and decontamination events following various process stages, including coagulation/flocculation, sedimentation, filtration and disinfection. A number

Comparing treatments in the presence of crossing survival curves: an application to bone marrow transplantation.

In some clinical studies comparing treatments in terms of their survival curves, researchers may anticipate that the survival curves will cross at some point, leading to interest in a long-term survival comparison. However, simple comparison of the survival curves at a fixed point may be inefficient, and use of a weighted log-rank test may be overly sensitive to early differences in survival. We formulate the problem as one of testing for differences in survival curves after a prespecified time point, and propose a variety of techniques for testing this hypothesis. We study these methods using

Mayo Clinic Risk Score for percutaneous coronary intervention predicts in-hospital mortality in patients undergoing coronary artery bypass graft surgery.

BACKGROUND: Current risk models predict in-hospital mortality after either coronary artery bypass graft surgery or percutaneous coronary interventions separately, yet the overlap suggests that the same variables can define the risks of alternative coronary reperfusion therapies. Our goal was to seek a preprocedure risk model that can predict in-hospital mortality after either percutaneous coronary intervention or coronary artery bypass graft surgery. METHODS AND RESULTS: We tested the ability of the recently validated, integer-based Mayo Clinic Risk Score (MCRS) for percutaneous coronary inte

Prediction of survival after liver transplantation by pre-transplant parameters.

OBJECTIVE: Score-based medical urgency criteria are used for necessity-oriented liver transplantation (OLT) but lead to an increasing number of complications in patients with reduced post-OLT survival. A prediction of outcome would improve preoperative patient selection and management. MATERIAL AND METHODS: One-hundred-and-thirty-three consecutive adult patients (63.9% men, mean age 47.4+/-11.2 years) given transplants between May 2004 and November 2005 at the Hannover Medical School were analysed retrospectively using univariate and multivariate methods. RESULTS: Indications were: 27.1% vir

An open-label pilot study of alefacept for the treatment of pyoderma gangrenosum.

BACKGROUND: Pyoderma gangrenosum (PG) is a chronic inflammatory disease that causes painful cutaneous ulcers that are difficult to treat. Currently, systemic immunosuppressants, often including prednisone, are the mainstay of therapy. Long-term therapy with these agents is often required which exposes patients to possible adverse effects. An alternative treatment that is safe and effective is truly needed. OBJECTIVE: To study the efficacy and safety of alefacept, which inhibits T-cell activation and selectively reduces the T-cell population, for treatment of PG. METHOD: In this prospective o

Computational studies of liquid water and diluted water in carbon tetrachloride.

Molecular dynamics simulations were carried out to study solvent effects on the energetic and dynamical properties of water molecules in liquid water and in carbon tetrachloride (CCl4). In these studies, the free-energy profiles or potentials of mean force (PMF) for water dimers in both solvents were computed. The computed PMF results showed a stable minimum near 3 A for the O-O separation, with a minimum free energy of about -2.8 kcal/mol in CCl4, as compared to a value of -0.5 kcal/mol in liquid water. The difference in free energy in water as compared to that in CCl4 was expected and is the

Pentraxin 3 is elevated in haemodialysis patients and is associated with cardiovascular disease.

BACKGROUND: Pentraxins are mediators of inflammation as well as markers of the acute-phase reaction. While elevation of C-reactive protein (CRP) in patients with renal failure and its association with cardiovascular disease is well described, there are no data on pentraxin 3 (PTX3) in this population. METHODS: Plasma was obtained from 44 chronic haemodialysis (HD) patients, 35 peritoneal dialysis (PD) patients, 39 patients with chronic renal failure (CRF) not on dialysis therapy and 14 age-matched normal subjects. PTX3 production in whole blood was also investigated in samples taken before an

Interaction of total blood calcium levels and brain catecholamines during the formation and reinforcement of memory traces in hypoparathyroidism.

Interactions between the total blood calcium level and catecholamine contents in limbic brain structures during the formation and extinction of a conditioned passive avoidance reflex were studied in rats with hypoparathyroidism. Impairments of calcium support for the processes of conditioned reflex activity were seen after removal of the parathyroid glands. Animals with hypoparathyroidism not only showed changes in basal dopamine and noradrenaline levels in limbic structures, but also impairments of catecholamine dynamics during the acquisition and forced extinction of a conditioned passive av

Echocardiographic assessment of mitral valve morphology after Percutaneous Transvenous Mitral Commissurotomy (PTMC).

AIMS: PTMC produces significant changes in mitral valve morphology as improvement in leaflets mobility. The determinants of such improvement have not been assessed before. METHODS AND RESULTS: The study included 291 symptomatic patients with mitral stenosis undergoing PTMC. Post-PTMC subvalvular splitting area was a determinant of post-PTMC excursion in both the anterior (B 0.16, 95% CI 0.03 to 0.30, p < 0.05) and the posterior (B 0.12, 95% CI 0.01 to 0.24, p < 0.05) leaflets. Another determinant was the post-PTMC transmitral pressure gradient for anterior (B -0.02, 95% CI -0.04 to -0.005, p

[Functional recovery of Oddi's sphincter after balloon dilatation of the papilla of Vater. An experimental study in rabbits].

INTRODUCTION: Balloon dilatation of the papilla of Vater is used to treat biliary lithiasis. The results and complications rate of this technique are excellent. Published data indicate that this procedure does not significantly alter the physiology of the sphincter of Oddi and that normal function is maintained. Papillary balloon dilatation would therefore provide an advantage over other techniques in which sphincteric function is abolished. The objective of this study was to evaluate the functional status of the sphincter of Oddi after balloon dilatation of the papilla of Vater. MATERIAL AND

Ambient air pollution and cardiac arrhythmias in patients with implantable defibrillators.

BACKGROUND: Previous studies of ambient air pollution and ventricular tachyarrhythmias in patients with implantable cardioverter defibrillators have yielded mixed results. METHODS: We examined this relationship in a study of 518 patients with 6287 tachyarrhythmic event-days over a 10-year period in Atlanta, Georgia. The air quality data included daily measurements of PM10, ozone, nitrogen dioxide, carbon monoxide, and sulfur dioxide for the entire study period, as well as speciated measurements of PM2.5 mass and oxygenated hydrocarbons for the final 4 years of the study. Our primary analyses

Temporal and spatial trends in the occurrence of human and veterinary antibiotics in aqueous and river sediment matrices.

The occurrence of 15 antibiotics belonging to three different groups, tetracyclines (TCs), sulfonamides (SAs), and macrolides (MLs), mainly used to prevent or treat illness for humans and also to control disease or to promote the growth for animals was studied in aqueous and sediment matrices. The result of spatial and temporal statistical analysis revealed that measured concentrations of individual antibiotics were significantly different depending on sampling location and time periods for aqueous and sediment samples. High concentrations of human-used antibiotics were detected downstream of

Predictive models for post-operative nausea and vomiting in patients using patient-controlled analgesia.

This study identified predictive factors for post-operative nausea and vomiting (PONV) in patients using patient-controlled analgesia (PCA) and developed five predictive model pathways to calculate the probability of PONV using decision tree analysis. The sample consisted of 1181 patients using PCA. Data were collected using: a specifically designed check-off form to collect patient-, surgery-, anaesthesia- and post-operation-related data; the Beck Anxiety Inventory to measure pre-operative anxiety; and a visual analogue scale, to measure post-operative pain. The incidence of PONV was 27.7%. N

Improving point predictions of random effects for subjects at high risk.

The prediction of random effects corresponding to subject-specific characteristics (e.g. means or rates of change) can be very useful in medical and epidemiologic research. At times, one may be most interested in obtaining accurate and/or precise predictions for subjects whose characteristic places them in a tail of the distribution. While the typical posterior mean predictor dominates others in terms of overall mean squared error of prediction (MSEP), its tendency to 'overshrink' has motivated research into alternatives emphasizing other criteria. Here, we specifically target MSEP within a ce
