Effectiveness of combined eccentric and concentric exercise over traditional cardiac exercise rehabilitation programme in patients with chronic heart failure: protocol for a randomised controlled study.

INTRODUCTION: Exercise-based rehabilitation is a standard feature of chronic heart failure management. The effectiveness of eccentric exercise could offer new opportunities for better tailoring rehabilitation programme to patients' limitations. The goal of the study is to contrast the impact of a mixed eccentric and concentric cycling training programme, to that of conventional concentric cycling rehabilitation in patients with chronic heart failure (peak oxygen consumption (VO2Peak) < 15 mL⋅kg-1⋅min-1, ejection fraction <40%). METHODS AND ANALYSIS: It is a prospective, open, controlled and r

A qualitative study of tobacco interventions for LGBTQ+ youth and young adults: overarching themes and key learnings.

BACKGROUND: Smoking prevalence is very high among lesbian, gay, bisexual, transgendered and queer (LGBTQ+) youth and young adults (YYA) compared to non-LGBTQ+ YYA. A knowledge gap exists on culturally appropriate and effective prevention and cessation efforts for members of this diverse community, as limited interventions have been developed with and for this population, and there are very few studies determining the impact of these interventions. This study identifies the most salient elements of LGBTQ+ cessation and prevention interventions from the perspective of LGBTQ+ YYA. METHODS: Three

What Happens After Menopause? (WHAM): protocol for a prospective, multicentre, age-matched cohort trial of risk-reducing bilateral salpingo-oophorectomy in high-risk premenopausal women.

INTRODUCTION: Women at high inherited risk of ovarian cancer are advised to undergo risk-reducing bilateral salpingo-oophorectomy (RRBSO) at age 40-45 years or when their families are complete. Most women are premenopausal at this age, so RRBSO will induce surgical menopause. Despite the clear benefits of RRBSO for cancer risk reduction, much less is known about the impact on non-cancer outcomes that contribute to health and well-being and inform surveillance and management strategies. METHODS AND ANALYSIS: This will be a multicentre, prospective cohort study of 105 premenopausal high-risk wo

Qualitative evaluation of selected social factors that impact sexual risk-taking behaviour among African students in Kwazulu-Natal, South Africa.

BACKGROUND: The incidence of HIV and AIDS continues to be a source of great concern within universities in South Africa. Furthermore, university students constitute an important community in the intervention against the HIV/AIDS epidemic. Students in the age group of 15-24 years are at a greater risk of HIV infection than any other group in the country; yet, little is known about why they continue to engage in risky sexual practices. OBJECTIVES: This study was designed to explore the sexual behaviour of students in a metropolitan Durban University of Technology in KwaZulu-Natal to understand

Rationale and design of the African group A streptococcal infection registry: the AFROStrep study.

INTRODUCTION: Group A β-haemolytic Streptococcus (GAS), a Gram-positive bacterium, also known as Streptococcus pyogenes, causes pyoderma, pharyngitis and invasive disease. Repeated GAS infections may lead to autoimmune diseases such as acute post-streptococcal glomerulonephritis, acute rheumatic fever (ARF) and rheumatic heart disease (RHD). Invasive GAS (iGAS) disease is an important cause of mortality and morbidity worldwide. The burden of GAS infections is, however, unknown in Africa because of lack of surveillance systems. METHODS AND ANALYSIS: The African group A streptococcal infection

Learning of Grammar-Like Sequences by Children With Developmental Language Disorder: An Electrophysiological Examination.

PURPOSE: Developmental language disorder (DLD) is a prevalent neurodevelopmental condition that affects approximately 7% of children in the United States. One notable characteristic of DLD is difficulty in mastering nonadjacent dependencies, such as the third-person singular -s or passive construction be -ed. This study examines behavioral and electrophysiological measures of nonadjacent dependencies learning in children with DLD in the context of a visual grammar-like learning paradigm. METHOD: Fifty-one children (22 DLD, 29 typical language development [TD]) aged 8;0-12;4 (years;months) com

Clinical Symptomatology, Cognition and Psychosocial Wellbeing in Adolescents With Myalgic Encephalomyelitis/Chronic Fatigue Syndrome: A Cross-Sectional Observational Study.

AIM: Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a disabling illness that frequently begins in adolescence, yet adolescents with ME/CFS remain an under-researched population. We aimed to characterise the cognitive and psychosocial wellbeing of adolescents with ME/CFS relative to normative benchmarks and unaffected age-peers. METHODS: Adolescents with mild-to-moderate ME/CFS (n = 25) and healthy controls (n = 25) aged 10-19 years were recruited in Melbourne, Australia. Each completed a set of clinical questionnaires and cognitive assessments to assess ME/CFS symptomatology,

Complex Pitch Perception Deficits in Dyslexia Persist Regardless of Previous Musical Experiences.

PURPOSE: Pitch perception is important for speech sound learning, and reading acquisition requires integration of speech sounds and written letters. Many individuals with dyslexia exhibit auditory perception deficits that may therefore contribute to their reading impairment given that complex pitch perception is crucial for categorizing speech sounds. Given rising interest in music training as a reading intervention, understanding associations between prior music experiences and pitch perception is important. This study explored the relationship between pitch perception skills and reading abil

Articulatory Insights into the L2 Acquisition of English-/l/ Allophony.

In many English varieties, /l/ is produced differently in onsets and codas. Compared with "light" syllable-initial realizations, "dark" syllable-final variants involve reduced tongue tip-alveolar ridge contact and a raised/retracted tongue dorsum. We investigate whether native French and Spanish speakers whose L1 lacks such positionally conditioned variation can acquire English-/l/ allophony, testing the hypotheses that (1) the allophonic pattern will be acquired by both groups but (2) learners will differ from native speakers in their phonetic implementation, particularly in codas; and (3) Fr

Brain language networks and cognitive outcomes in children with frontotemporal lobe epilepsy.

INTRODUCTION: Pediatric frontal and temporal lobe epilepsies (FLE, TLE) have been associated with language impairments and structural and functional brain alterations. However, there is no clear consensus regarding the specific patterns of cerebral reorganization of language networks in these patients. The current study aims at characterizing the cerebral language networks in children with FLE or TLE, and the association between brain network characteristics and cognitive abilities. METHODS: Twenty (20) children with FLE or TLE aged between 6 and 18 years and 29 age- and sex-matched healthy c

Emotional freedom technique versus written exposure therapy versus waiting list for post-traumatic stress disorder: protocol for a randomised clinical MRI study.

INTRODUCTION: The emotional freedom technique (EFT) is an acupuncture-based psychotherapy that combines tapping on acupoints with cognitive reframing. EFT has been previously shown to have potential for treating post-traumatic stress disorder (PTSD). However, further clinical evidence and underlying mechanisms of EFT are yet to be fully explored. This proposed clinical trial aims to examine the effect of EFT on patients with PTSD compared with the waitlist (WL) and active controls. METHODS AND ANALYSIS: This study was designed as a randomised, assessor-blinded, three-arm clinical MRI study. A

Longitudinal Grammaticality Judgments of Tense Marking in Complex Questions in Children With and Without Specific Language Impairment, Ages 5-18 Years.

PURPOSE: Identification of children with specific language impairment (SLI) can be difficult even though their language can lag that of age peers throughout childhood. A clinical grammar marker featuring tense marking in simple clauses is valid and reliable for young children but is limited by ceiling effects around the age of 8 years. This study evaluated a new, more grammatically challenging complex sentence task in children affected or unaffected with SLI in longitudinal data, ages 5-18 years. METHOD: Four hundred eighty-three children (213 unaffected, 270 affected) between 5 and 18 years

Construction of a patient decision aid for the treatment of uncomplicated urinary tract infection in primary care.

BACKGROUND: Uncomplicated urinary tract infection (uUTI) is very common among women in primary care. The risk of developing pyelonephritis remains low after uUTI, nonetheless, empiric antibiotic therapy is frequently prescribed for symptomatic purposes. This may lead to adverse effects and antibiotic resistance. Furthermore, patients may express the will to limit the use of antibiotics. Some European countries recommend discussing a delayed prescription with the patient and developing a shared decision. The aim of this study is to create a patient decision aid (PtDA) used in primary care setti
