Living with coeliac disease: a grounded theory study.

BACKGROUND: Coeliac disease can be controlled only through adherence to a gluten-free diet. This diet is highly restrictive and can be challenging to maintain. It has been linked with elevated levels of psychological distress, including depression, anxiety and social phobia. METHODS: Narratives on living with coeliac disease were written by 130 adult members of Coeliac UK (mean age 52.7 years; mean time since diagnosis 10.2 years; 67% sample female; 28% male). Qualitative analysis using grounded theory methods identified five key categories: living with widespread ignorance; social invisibili

Cannabis-related impairment and social anxiety: the roles of gender and cannabis use motives.

Social anxiety appears to be especially related to cannabis-related problems, yet the nature of this association remains unclear. Some data suggest that socially anxious men may be especially vulnerable to problematic cannabis use. The current study examined the relations between social anxiety, cannabis use and use-related problems, and motives for cannabis use by gender among 174 (42.5% female) current (past-month) cannabis users. Among men, social anxiety was significantly, positively related to the number of cannabis-related problems and coping and conformity motives. Coping and conformity

Corticolimbic brain reactivity to social signals of threat before and after sertraline treatment in generalized social phobia.

BACKGROUND: Generalized social phobia (gSP), also known as generalized social anxiety disorder, is characterized by excessive fear of scrutiny by others and pervasive avoidance of social interactions. Pathophysiologic models of gSP implicate exaggerated reactivity of the amygdala and insula in response to social evaluative threat, making them plausible targets for treatment. Although selective serotonin reuptake inhibitor (SSRI) treatment is known to be an effective treatment, little is known about the mechanism through which these agents exert their anxiolytic effects at a brain level in gSP.

Assessing fear-avoidance beliefs in patients with whiplash-associated disorders: a comparison of 2 measures.

OBJECTIVES: To examine the development of fear avoidance behaviours following whiplash injury using two different measures of fear avoidance, the Pictorial Fear of Activities Scale-Cervical (PFActS-C), and the Tampa Scale of Kinesiophobia (TSK-17). Secondarily we assessed the capacity of these measures to predict recovery status at long term follow up and initial cervical range of movement (ROM). METHODS: Ninety-eight patients with acute WAD were recruited and completed measures of pain and disability (NDI), fear avoidance beliefs and cervical ROM at baseline (<4 weeks), 3 and 6 months post i

Nonfat phobic anorexia nervosa: clinical characteristics and response to inpatient treatment.

OBJECTIVE: The aim of this study was to compare patients admitted to a specialized inpatient program for anorexia nervosa (AN) with and without a fear of weight gain in terms of specific eating disorder symptoms, general psychopathology, and treatment outcome. METHOD: Measures of specific and general psychopathology were administered at admission to and discharge from the inpatient program. Of the 138 participants, 81% (n = 112) were categorized as having clinical levels of "fear of weight gain" and 19% (n = 26) were categorized as having subclinical levels of this feature. RESULTS: The subc

Associations of infant feeding practices and picky eating behaviors of preschool children.

Picky eating behaviors are prevalent during childhood and are often linked to nutritional problems. However, information on the determinants of picky eating behaviors during infancy, when food acceptance patterns develop, is scarce. This study was conducted to evaluate the impact of infant feeding practices on the development of picky eating behaviors during preschool years. Baseline survey data from the Synergistic Theory and Research on Obesity and Nutrition Group Kids (STRONG Kids) program were used for this retrospective data analysis. Primary caregiver-child dyads were recruited from chil

Neurolinguistic programming used to reduce the need for anaesthesia in claustrophobic patients undergoing MRI.

The purpose of this study was to assess the success of neurolinguistic programming in reducing the need for general anaesthesia in claustrophobic patients who require MRI and to consider the financial implications for health providers. This was a prospective study performed in 2006 and 2007 at a teaching hospital in England and comprised 50 adults who had unsuccessful MR examinations because of claustrophobia. The main outcome measures were the ability to tolerate a successful MR examination after neurolinguistic programming, the reduction of median anxiety scores produced by neurolinguistic p

A self-report measure of subtle avoidance and safety behaviors relevant to social anxiety: development and psychometric properties.

According to cognitive theories, safety-seeking behaviors are crucial in both the maintenance and management of social anxiety. In order to facilitate assessment of these behaviors the Subtle Avoidance Frequency Examination (SAFE) was developed. Three factors emerged from the SAFE, which appeared to reflect active "safety" behaviors, subtle restriction of behavior, and behaviors aimed at avoiding or concealing physical symptoms. The SAFE demonstrated strong internal consistency, good construct validity and the ability to discriminate between clinical and non-clinical participants. In addition,

Exubera inhaled insulin in patients with type 1 and type 2 diabetes: the first 12 months.

BACKGROUND: Following National Institute for Clinical Excellence approval of inhaled insulin Exubera (Pfizer, New York, NY) in 2006, we established a dedicated clinic in January 2007 to monitor the efficacy and safety of Exubera. Between January and October 2007, eight patients started Exubera: six because of needle phobia (DSM-IV criteria) and two with injection site problems. METHODS: Data were collected at the clinic over a 12-month period from February 2007 at 3-, 6-, 9-, and 12-month intervals. The clinic is jointly led by a consultant diabetologist and a diabetes specialist nurse within

Guided and unguided self-help for social anxiety disorder: randomised controlled trial.

BACKGROUND: Internet-delivered self-help programmes with added therapist guidance have shown efficacy in social anxiety disorder, but unguided self-help has been insufficiently studied. AIMS: To evaluate the efficacy of guided and unguided self-help for social anxiety disorder. METHOD: Participants followed a cognitive-behavioural self-help programme in the form of either pure bibliotherapy or an internet-based treatment with therapist guidance and online group discussions. A subsequent trial was conducted to evaluate treatment specificity. Participants (n = 235) were randomised to one of th

Attachment to parents, social anxiety, and close relationships of female students over the transition to college.

The current study examined the process by which attachment to parents influences satisfaction with and ease in forming friendships at college. One hundred seventy-two female college freshmen completed a measure of parental attachment security the summer before their first semester of college (July 2006) and measures to assess satisfaction with and ease in forming close relationships at the end of their first semester (December 2006). Students ranged in age from 18 to 20 years (M = 18.09, SD = 0.33) and were diverse in their racial makeup (30% racial minority). Consistent with predictions deriv

Prevalence of dental fear and phobia relative to other fear and phobia subtypes.

The purpose of the present study was to estimate the point prevalence of dental fear and dental phobia relative to 10 other common fears and Diagnostic and Statistical Manual of Mental Disorders (DSM)-IV-TR subtypes of specific phobia. Data were also analysed to examine differences with regard to severity, presence of distressing recollections of fear-related events, gender, and prevalence across age. Data were obtained by means of a survey of 1,959 Dutch adults, 18-93 yr of age. Phobias were assessed based on DSM-IV-TR criteria, whereas severity of present fears was assessed using visual anal

Social anxiety disorder: epidemiology, biology and treatment.

Social anxiety disorder (SAD) is considered to be one of the most common anxiety disorders. Despite its high prevalence, the disorder is still considerably underdiagnosed and undertreated. SAD shows a typically early onset in childhood or early adolescence and generally becomes chronic. The disease places a massive burden on patients lives, affecting not only their social interactions but also their educational and professional activities, thereby constituting a severe disability. Although substantial progress in the study of the etiology of SAD has been made, no commonly accepted model has em

Trajectories of social withdrawal from middle childhood to early adolescence.

Heterogeneity and individual differences in the developmental course of social withdrawal were examined longitudinally in a community sample (N = 392). General Growth Mixture Modeling (GGMM) was used to identify distinct pathways of social withdrawal, differentiate valid subgroup trajectories, and examine factors that predicted change in trajectories within subgroups. Assessments of individual (social withdrawal), interactive (prosocial behavior), relationship (friendship involvement, stability and quality, best friend's withdrawal and exclusion/victimization) and group- (exclusion/victimizati

Parent food purchases as a measure of exposure and preschool-aged children's willingness to identify and taste fruit and vegetables.

This study explored whether parents who purchase more fruit/vegetables have preschool-aged children who are able to identify fruit/vegetables and in turn are more likely to consume them. Sixty-two parent-child pairs were recruited during a 4-month period. The data collection included a child interview, a parent/guardian interview, a fruit/vegetable taste test for children, and a month-long food-receipt collection by the parent/guardian. As the percentage of fruit/vegetables purchased by parent increased, the child was more likely to accept all of the fruit/vegetables offered to him/her. A weak

Shyness 3: randomized controlled trial of guided versus unguided Internet-based CBT for social phobia.

OBJECTIVE: In two previous randomized controlled trials Titov et al. demonstrated significant benefit from an Internet- and email-based treatment programme for social phobia. The present study (Shyness 3) explores whether participants are able to complete this programme independently. METHOD: A total of 98 individuals with social phobia were randomly assigned to a clinician-assisted computerized cognitive behavioural treatment (CaCCBT) group, a self-guided computerized CBT (CCBT) group, or to a waitlist control group. CaCCBT group participants completed the usual Shyness programme consisting

Support for the predictive validity of the SASC-R: linkages with reactions to an in vivo peer evaluation manipulation.

The present study examined the predictive validity of the Social Anxiety Scale for Children-Revised (SASC-R) by investigating associations between children's social anxiety symptoms and their cognitive and behavioral reactions in response to an in vivo peer evaluation manipulation. Participants (N=115) ages 10-13, played a computer game based on the television show Survivor and were randomized to either a peer rejection (i.e., receiving the lowest total 'likeability' score from a group of peer-judges), a peer success (i.e., receiving the highest score), or a neutral peer feedback condition. [Treatment of selective mutism].

Selective mutism is a communication disorder of childhood in which the child does not speak in specific social situations despite the ability to speak in other situations. A literature review was completed in order to provide practical guidelines for the assessment and treatment of children with selective mutism. There are many different behavioral approaches in the treatment of this disorder, e.g. contingency management, shaping, stimulus fading, escape-avoidance, self-modeling, learning theory approaches. A clearer diagnostic understanding of the disorder as part of anxiety or oppositional d
