Oesophageal pressure swings were analysed during complete occlusion of the upper airways in 8 patients with obstructive sleep apnoea. This study was done using Wistar rats to determine if the actions of propofol (22 +/- 1, 40 +/- 2, 64 +/- 3 and 103 +/- 3 mg.kg-1 x hr-1) decreased blood pressure and heart rate through depression of brain stem vasomotor centres. To further delineate indications for noninvasive pressure support ventilation (NIPSV), we proposed this noninvasive technique as an alternative to endotracheal intubation in 17 consecutive patients with acute respiratory failure from various causes. On rare occasions after total sternectomy, patients develop persistent flail chest deformities requiring long-term mechanical respiratory assistance. We report the use of a temporary external chest shell to deliver constant negative extrathoracic pressure (CNEP) to a long-term ventilated patient with flail chest. The patient's anterior thoracic cage stabilized, and significant improvement in pulmonary function was observed. With these data in hand, an operation was done to permanently stabilize the anterior chest wall by bone grafting.

JB, a 62 year old male, was facing potential amputation of his lower left leg following traumatic injury in the left malleolar area. Previous treatments over an approximate six week period had not improved the wound condition. As an alternative treatment, we performed a cyclocryotherapy in a series of 27 eyes with chronic narrow angle and chronic open angle glaucoma. Intraocular pressure control was achieved in 91% of the eyes after a mean follow-up period of 38 months. Topical anti-glaucoma medication was needed in 65%. The complications were minor and transient. No serious complications were seen.
