The clinical relevance of complex regional pain syndrome type I: The Emperor's New Clothes.

The management of patients with chronic pain is a nearly daily challenge to rheumatologists, neurologists, orthopedic surgeons, pain specialists and indeed a issue in nearly every clinical practice. Among the myriad of causes of pain are often included a unique syndrome, generally referred to as complex regional pain syndrome type I (CRPS). Unfortunately CRPS I has become a catch all phase and there are serious questions on whether it exists at all; this has led to an extraordinary number of poorly defined diagnostic criteria. It has also led to an etiologic quagmire that includes features as

Comparative study of effect of single and multiple suture techniques on inflammatory complications after third molar surgery.

PURPOSE: Pain, swelling, and trismus are the most common complications associated with third molar surgery. Several methods of alleviation of these complications have been described. The effect of single and multiple suture techniques on these complications was compared in the present study. PATIENTS AND METHODS: All consecutive patients 18 years of age or older who had been referred for surgical extraction of their impacted teeth between January and December 2007 at the maxillofacial unit of the Aminu Kano Teaching Hospital were recruited and randomized into 2 groups. All selected participan

Initial experience with a cardiologist-based chest pain unit in an emergency department in Israel.

BACKGROUND: Emergency room triage of patients presenting with chest pain syndromes may be difficult. Under-diagnosis may be dangerous, while over-diagnosis may be costly. OBJECTIVES: To report our initial experience with an emergency room cardiologist-based chest pain unit in Israel. METHODS: During a 5 week pilot study, we examined resource utilization and ER diagnosis in 124 patients with chest pain of uncertain etiology or non-high risk acute coronary syndrome. First assessment was performed by the ER physicians and was followed by a second assessment by the CPU team. Assessment was based

Specificity of diagnostic nerve blocks: a prospective, randomized study of sciatica due to lumbosacral spine disease.

Temporary nerve blocks using local anesthetic are employed extensively in the evaluation of pain problems, particularly lumbosacral spine di … limited role for uncontrolled local anesthetic blocks in the diagnostic evaluation of sciatica and referred pain syndromes in general. Negative blocks or a pattern of responses may have some predictive value, but isolated, positive blocks are non-specific. This lack of specificity may, however, be advantageous in therapeutic applications.

Clinical applications of invasive spinal cord stimulation in neurological disorders: a narrative review.

INTRODUCTION: Invasive spinal cord stimulation (SCS) is used in selected patients with refractory chronic neuropathic pain and has also been explored in spinal cord injury, disorders of consciousness, and Parkinson's disease. However, the strength of evidence differs substantially across indications. This review summarizes the current clinical applications of invasive SCS and distinguishes relatively established pain indications from emerging neurological applications. METHODS: We conducted a structured search of PubMed, Embase, the Cochrane Central Register of Controlled Trials, Web of Scien

Traumatic abdominal wall hernia diagnosed 14 years after a bad fall with lumbar spine fracture.

UNLABELLED: Traumatic abdominal wall hernias have significant implications for patients and insurance companies, especially when not been discovered at the time of trauma. We present the case of a gardener who sustained a bad fall during work in 1994 with immediate admission to the hospital for treatment. A fracture of the second lumbar spine body has been diagnosed and stabilized operatively. Postoperative computer tomography and magnetic resonance examinations demonstrated correct healing of the fracture. Neither the pain in the sacral spine, the left leg and left lower abdominal wall nor th

Evidence of distorted proprioception and postural control in studies of experimentally induced pain: a critical review of the literature.

OBJECTIVES: Deficits in proprioception and postural control are common in patients with different musculoskeletal pain syndromes. It has been proposed that pain can negatively affect proprioception and postural control at a peripheral level, however research is limited to animal studies. Human studies have shown that it is more likely, that the link between pain and proprioceptive deficits, lies within changes in the central nervous system where noxious and non-noxious stimuli may overlap. In clinical studies, causality cannot be determined due to other factors which could confound the assessm

Neurofeedback for Pain Management: A Systematic Review.

Background: Chronic pain is a significant global health issue. For most individuals with chronic pain, biomedical treatments do not provide adequate relief. Given the evidence that neurophysiological abnormalities are associated with pain, it is reasonable to consider treatments that target these factors, such as neurofeedback (NF). The primary objectives of this review were to summarize the current state of knowledge regarding: (1) the different types of NF and NF protocols that have been evaluated for pain management; (2) the evidence supporting each NF type and protocol; (3) if targeted bra

SUcceSS, SUrgery for Spinal Stenosis: protocol of a randomised, placebo-controlled trial.

INTRODUCTION: Central lumbar spinal stenosis (LSS) is a common cause of pain, reduced function and quality of life in older adults. Current management of LSS includes surgery to decompress the spinal canal and alleviate symptoms. However, evidence supporting surgical decompression derives from unblinded randomised trials with high cross-over rates or cohort studies showing modest benefits. This protocol describes the design of the SUrgery for Spinal Stenosis (SUcceSS) trial -the first randomised placebo-controlled trial of decompressive surgery for symptomatic LSS. METHODS AND ANALYSIS: SUcce

Rare oto-tricho-tussia/tinnitus: A case report.

KEY CLINICAL MESSAGE: Oto-tricho is associated with the symptoms of gag reflex and non-neuropathic tinnitus, which can be treated by removing the hair and its follicle. ABSTRACT: Oto-tricho refers to a disorder of hair growth in the tympanic membrane (TM). In its early stages, it can manifest as symptoms such as tinnitus, chronic pain, cough, and nausea. If left untreated, it can potentially lead to tympanosclerosis, TM micro-perforation, and hearing loss. In this report, we present a case study of a 33-year-old male with hair and follicle growth observed on the left TM. Over the course of th

Bladder pain syndrome and pregnancy.

Bladder pain syndrome (BPS) is a poorly understood condition. In pregnancy, lower urinary tract symptoms and pain are common, but the possibility of BPS is rarely considered and almost never explored. The consequences of BPS on pregnancy and vice versa are poorly understood, and management options appear to be limited. This article reviews the current evidence to allow us to better counsel, investigate, diagnose and manage patients with suspected or known BPS who fall pregnant or who are considering pregnancy. MEDLINE, EMBASE and PubMed were searched for a combination of mesh terms of keywords

Heterotopic ossification beneath the upper abdominal incision after radical gastrectomy: Two case reports.

BACKGROUND: Heterotopic ossification (HO) is a rare clinical phenomenon that refers to bone formation in nonossifying tissues. CASE SUMMARY: This report presents two cases of HO beneath the upper abdominal median incision after radical gastrectomy. The first patient had postoperative pain below the incision area. There were no signs of anastomotic leakage, and the wound healed. Computed tomography (CT) findings 2 wk postoperatively were negative for HO, but the 6-wk CT showed HO beneath the incision. The patient refused reoperation, and after conservative therapy, the pain was gradually relie

Implementation of Office Hysteroscopy for the Evaluation and Treatment of Intrauterine Pathology.

Hysteroscopy provides a minimally invasive strategy to evaluate intrauterine pathology and manage conditions such as abnormal uterine bleeding, infertility, intrauterine adhesions, müllerian anomalies, and intrauterine foreign bodies. Increasing access to hysteroscopy procedures in the office has the potential to improve patient care by minimizing financial and logistical barriers, aiding in streamlined diagnosis and treatment planning, and potentially averting unnecessary operative procedures and anesthesia. Office hysteroscopy refers to procedures performed in outpatient settings where pain

Post-Mastectomy Pain: An Updated Overview on Risk Factors, Predictors, and Markers.

After breast surgery, women frequently develop chronic post-mastectomy pain (PMP). PMP refers to the occurrence of pain in and around the area of the mastectomy lasting beyond three months after surgery. The nature of factors leading to PMP is not well known. When PMP is refractory to analgesic treatment, it negatively impacts the lives of patients, increasing emotional stress and disability. For this reason, optimizing the quality of life of patients treated for this pathology has gained more importance. On the basis of the findings and opinions above, we present an overview of risk factors a

Time, history and mythology in the cultural construction of the "nature of pain": positioning Jewish settlements' evacuation within the religious timeline.

The concept of cultural trauma serves as a useful analytical framework for analyzing social processes of the creation of trauma narratives. Following the conceptualization of "cultural trauma" by Jeffrey Alexander, this article focuses on representations of the "nature of pain" produced by mental health professionals who identified with Religious Zionism working Jewish settlers involved in the 2005 evacuation of from Gaza and the West Bank. Based on ethnographic and written materials collected during fieldwork at a mental health center that offered professional interventions to settlers, I arg

First Bite Syndrome After Bilateral Temporomandibular Joint Replacement: Case Report.

First bite syndrome (FBS) refers to intense pain in the parotid region, which coincides with the first bite of every meal, gradually subsides over the next several bites, but returns with the first bite of the next meal. The definitive diagnosis can be readily established by the characteristic onset of pain after the first bite of every meal. Pain is typically most intense at the first meal of the day, although some patients experience symptoms when thinking of food or salivating. FBS is a recognized complication of surgery within the parapharyngeal space; however, other surgical procedures in
