NAPSAQ-1: National Patient Sleep Assessment Questionnaire in depression.

Objectives. Sleep disturbance is a common feature of depression. Symptoms often persist after treatment of the depressive episode, representing a risk factor for relapse. There is a lack of data regarding the nature of sleep disturbance in depression in the UK. Methods. We surveyed patients' views about their depressive symptoms and associated sleep difficulties. We received 513 responses via postal questionnaire. Results. A total of 97% reported sleep difficulties during depression: symptoms of insomnia were more frequently reported than hypersomnia. Ninety-nine percent of those with sleep pr…

Phonological development in children with obstructive sleep-disordered breathing.

Adeno-tonsillar hypertrophy with obstructive sleep disordered breathing (OSDB) is known to affect oral-motor function, behaviour, and academic performance. Adeno-tonsillectomy is the most frequently performed operation in children, with total tonsillectomy (TE) being more common than partial resection, 'tonsillotomy' (TT). In the present study 67 children, aged 50-65 months, with OSBD were randomized to TE or TT. The children's phonology was assessed pre-operatively and 6 months post-operatively. Two groups of children served as controls. Phonology was affected in 62.7% of OSBD children before…

Alterations in sleep EEG activity during the hypopnoea episodes.

The Obstructive Sleep Apnoea Hypopnoea Syndrome (OSAH) means 'cessation of breath' during the sleep hours and the sufferers often experience related changes in the electrical activity of the brain and heart. The aim of this paper is to investigate any possible changes in the human electroencephalographic (EEG) activity due to hypopnoea (mild case of cessation of breath) occurrences by applying the non-linear and linear time series methods. The results from this one-subject study indicated significant changes in the human EEG activity due to hypopnoea episodes by applying the non-linear, Lyapun…

Manipulation of skin temperature improves nocturnal sleep in narcolepsy.

OBJECTIVE: Besides excessive daytime sleepiness, disturbed nocturnal sleep is a major complaint of patients with narcolepsy. Previously, alterations in skin temperature regulation in narcoleptic patients have been shown to be related to increased sleepiness. This study tests the hypothesis that direct control of nocturnal skin temperature might be applied to improve the disturbed sleep of narcoleptic patients. METHODS: Participants were eight patients (five males) diagnosed as having narcolepsy with cataplexy according to the ICSD-2 criteria, mean (SD) age 28.6 (6.4) years, range 18-35 years.…

[Cardiovascular risk among adults with obstructive sleep apnea syndrome].

Cardiovascular diseases and sleep-disordered breathing have been recognized as a public health problem in Mexico and worldwide. These two groups of disorders are closely associated and the evidence accumulated over the last 25 years indicates that obstructive sleep apnea syndrome (OSAS) is an independent risk factor in systemic arterial hypertension, coronary artery disease and stroke. Other associations have also been described, linking these disorders with pulmonary hypertension, cardiac arrhythmias, sudden death during sleep and congestive heart failure. Treatment with continuous positive a…

Sleep position and laterality of benign paroxysmal positional vertigo.

OBJECTIVE: The aim of this study was to investigate the frequency of posterior semicircular canal benign paroxysmal positional vertigo in each ear, and to assess the association between the ear affected by benign paroxysmal positional vertigo and the head-lying side during sleep onset. Based on a previous study which used objective methods to prove the preference of the elderly for the right head-lying side during sleep, we hypothesised that a predominance of the same head-lying side in benign paroxysmal positional vertigo patients may affect the pathophysiology of otoconia displacement. STUD…

A case of obstructive sleep apnea syndrome: unusual comorbidity.

INTRODUCTION: Launois-Bensaude syndrome (LBS) is a rare disease, characterized by the accumulation of fatty tissue predominantly in the neck, shoulders and thorax, whose diagnostic is clinical. OBJECTIVES: We describe a new case in a 73 year-old man. As the patient was obese, complained of snoring and was treated for hypertension, we looked for a morbid association with an obstructive sleep apnea syndrome (OSAS). METHODS: A polysomnography (PSG) and a cervical magnetic resonance imaging (MRI) were performed. RESULTS: PSG demonstrated OSAS with an apnea-hypopnea index of 43/h. Cervical MRI s…

Gender differences in obstructive sleep apnea and treatment implications.

Obstructive sleep apnea (OSA) is a common cause of daytime sleepiness for millions of Americans. It is also a disease associated with an increased likelihood of hypertension, cardiovascular disease, stroke, daytime sleepiness, motor vehicle accidents, and diminished quality of life. A number of population-based studies have shown that OSA is more common in men than in women and this discrepancy is often evident in the clinical setting. There are a number of pathophysiological differences to suggest why men are more prone to the disease than women. Although the exact mechanisms are unknown, dif…

Defining common outcome metrics used in obstructive sleep apnea.

Sleep-disordered breathing a spectrum that ranges from snoring through disorder of increased airway resistance, to overt sleep apnea affects many clinical disease outcomes. Traditionally, disease outcomes have been measured by polysomnography, with the most common metric being the apnea hypopnea index (AHI). Multiple other clinical metrics are commonly used to assess the severity and impact of disease on important outcomes of obstructive sleep apnea (OSA). These allow assessment of sleepiness, quality of life, performance, and medical, especially cardiovascular outcomes. Currently the availabl…

Sleep-disordered breathing in patients with decompensated heart failure.

Sleep-disordered breathing (SDB) has a higher prevalence in patients with heart failure than in the general middle-aged population. Obstructive sleep apnea (OSA), one of the forms of SBD, promotes poorly controlled hypertension, coronary events, and atrial fibrillation events that can lead to acutely decompensated heart failure (ADHF), and evidence suggests that untreated OSA increases mortality in patients with heart failure. Cheyne-Stokes respiration and central sleep apnea (CSA) have long been associated with heart failure and, in many patients, can coexist with OSA. In this article, we pro…

Conservative treatment of obstructive sleep apnea: a case report.

Conservative treatment of obstructive sleep apnea includes weight loss, changes in sleep posture, drug therapy, nasal continuous positive airway pressure and placement of an intraoral prosthesis. This article describes the success achieved in a case treated with conservative therapy, that is, intraoral prosthesis. A mandibular advancement prosthesis was fabricated for a 35-year-old male patient suffering from obstructive sleep apnea. The prosthesis was fabricated at a raised vertical dimension, with the central area open for the air passage. This advanced mandibular position enhanced the paten…

Obstructive sleep apnea, seizures, and childhood apraxia of speech.

Associations between obstructive sleep apnea and motor speech disorders in adults have been suggested, though little has been written about possible effects of sleep apnea on speech acquisition in children with motor speech disorders. This report details the medical and speech history of a nonverbal child with seizures and severe apraxia of speech. For 6 years, he made no functional gains in speech production, despite intensive speech therapy. After tonsillectomy for obstructive sleep apnea at age 6 years, he experienced a reduction in seizures and rapid growth in speech production. The findin…

Neural drive to human genioglossus in obstructive sleep apnoea.

One postulated mechanism for obstructive sleep apnoea (OSA) is insufficient drive to the upper-airway musculature during sleep, with increased (compensatory) drive during wakefulness. This generates more electromyographic activity in upper airway muscles including genioglossus. To understand drives to upper airway muscles, we recorded single motor unit activity from genioglossus in male groups of control (n = 7, 7 +/- 2 events h(-1)) and severe OSA (n = 9, 54 +/- 4 events h(-1)) subjects. One hundred and seventy-eight genioglossus units were recorded using monopolar electrodes. Subjects were a…

Clinical perspective of obstructive sleep apnea-induced cardiovascular complications.

Obstructive sleep apnea (OSA) syndrome is a highly prevalent disorder characterized by recurrent upper airway collapse during sleep, and associated with repetitive episodes of transient oxygen desaturation during sleep. It disrupts normal ventilation and sleep architecture, and is typically associated with excessive daytime sleepiness, snoring, and witnessed apneas. Besides being associated with neurocognitive impairment, mood and behavioral effects, and increased risk for work-related and traffic accidents, OSA has also been implicated in the pathogenesis of various cardiovascular diseases, i…
