The Effects of Criminal Legal Stigma on Substance Use Among Justice-Involved Girls and the Mitigating Role of Resilience.

BACKGROUND: Over the past two decades, the proportion of girls entangled in the criminal legal system has grown markedly. While prior research has examined the adverse effects of criminal labeling, much of this work emphasizes the outcome of reoffending. In contrast, the health-related consequences of criminal records, particularly for female youth, remain critically underexplored. METHODS: This study examined the impact of accumulated crime and delinquency records (ACDR), in conjunction with resilience factors, on substance use among girls involved in the juvenile justice system (n = 1,648).

Leveraging a digital health intervention to improve recovery outcomes among people with substance misuse experiencing housing insecurity.

BACKGROUND: People with substance use disorders (PwSUD) who experience housing insecurity have disproportionate overdose deaths. Digital interventions may improve care among PwSUD, but evidence specifically for housing-insecure PwSUD, who face unique barriers, remains limited. METHODS: We examined the efficacy of a mHealth tool (uMAT-R) among people who have misused substances and reported current housing insecurity. Substance use, cravings, access to basic needs, social disconnection, and digital literacy were assessed at baseline and 1 month and compared by uMAT-R use. Generalized estimatin

Advancing substance use disorder biology by studying underlying gene x environment interactions.

Substance use disorders (SUDs) are heritable, complex genetic disorders associated with a growing list of genetic variants that are common in the population at large. These variants occur in noncoding genomic regions and are presumed to confer risk by altering the expression of one or more target genes; yet, by definition, SUD cannot occur without repeated exposures to drugs, cannabis, or alcohol. Moreover, there seem to be critical neurodevelopmental windows, both in utero and adolescence, during which drug exposure impacts the brain most. Deciphering the molecular mechanisms by which genetic

Public drug use, policing, and the limits of decriminalization in British Columbia: A mixed methods study.

In January 2023, British Columbia implemented a province-wide decriminalization policy for simple drug possession that aimed to reduce criminal penalties for people who use drugs. However, the policy emerged alongside intense public and political scrutiny of visible drug use which led to new restrictions on public consumption. This study uses a mixed-methods approach that draws on quantitative data from the 2024 Harm Reduction Client Survey (N = 622) and in-depth interviews with people who use drugs (N = 38) to examine how people who use drugs have experienced decriminalization in relation to

Management of Opioid Withdrawal Syndrome and Commencement of Opioid Dependence Treatment Following Chronic Use of a Vaping Product Containing Protonitazene: Case Report.

INTRODUCTION: Nitazenes, benzimidazole synthetic potent opioid μ-receptor agonists, have been implicated in increased risk for opioid toxicity. Subtypes of nitazenes, including protonitazene, have been recently detected in multiple overdose cases in Australia. We describe a case report of protonitazene dependence, inpatient withdrawal management and induction onto opioid dependence treatment. CASE PRESENTATION: A 22-year-old man with no significant medical history presented to a metropolitan emergency department in opioid withdrawal after regular vaping of a product sold as cannabinoids. He r

Management of the Critically Ill Patient with Opioid Use Disorder.

The management of critically ill patients with opioid use disorder presents a unique challenge, as overdose, withdrawal, or concomitant intoxication, due to an unregulated drug supply, leads to physiologic changes that potentially impact standard treatment. This review article outlines an emergency medicine-focused approach for early recognition and stabilization of opioid overdose or withdrawal. Topics include naloxone, use of medications for opioid use disorder, recognition of concurrent intoxication, treatment of α2-agonist withdrawal, multimodal analgesia, and sedation for opioid-tolerant

Framing drug policy reform: Third-sector advocacy and the 2024 New South Wales Drug Summit.

BACKGROUND: Drug policy remains a politically and socially contested domain, with third-sector voices often marginalised despite their central role in advocacy and service delivery. The 2024 New South Wales (NSW) Drug Summit provided a rare, government-convened deliberative forum for third-sector organisations to contribute to state drug policy debate. This study examined how third-sector organisations framed drug policy issues and proposed solutions in advocacy documents produced in the lead-up to the summit and assessed the extent to which these frames were reflected in the summit's official

"It Gave me Goosebumps": A Reflexive Thematic Analysis of the Experiences of Adults With Substance Use Disorders in a Recovery-Oriented Group Songwriting Session.

Although music therapy can have positive impacts on motivation, treatment readiness, and craving for people with substance use disorders (SUD), most of the literature is from the objectivist paradigm and lacks the voices and lived experiences of people with addictions. A nuanced and situated understanding of music therapy that centers the experiences and voices of service users may help improve treatment. Therefore, the purpose of this study was to understand the lived experience of adults with SUD on a detoxification unit who participated in a single recovery-oriented group blues songwriting

The Khánh Hoà model: A case study of voluntary community-based drug treatment in provincial Vietnam.

For almost three decades, compulsory detention has shaped Vietnam's response to illicit drug use. Each year, up to 60,000 people who use drugs (PWUD) are confined in Compulsory Drug Detention and Rehabilitation Centres (CDDRCs), despite not being charged with or convicted of criminal offences. Detention is justified through a moral framework that categorises drug use as a 'social vice', subordinating health needs to judgements of individual character. In 2013, the Vietnamese government introduced the Renovation Plan for Drug Treatment, promoting a shift toward voluntary, community-based treatm

Medicaid Managed Care Plan Alignment With State Substance Use Disorder Treatment Coverage Requirements.

UNLABELLED: Policy Points States contract with Medicaid managed care plans to administer benefits for roughly 70 million Medicaid enrollees, yet little is known about how plan benefit policies for substance use disorder (SUD) treatment medications align with state requirements. In this study, we found that among the population of 167 Medicaid managed care plans responsible for SUD pharmacy benefits in 2021, many did not align with state requirements to cover SUD treatment medications. Many plans imposed prior authorization requirements on these medications, even when prohibited from doing so.

Prospects of GLP-1 Therapies for Addiction and Mental Health Comorbidities-Quo Vadis?: A Review.

IMPORTANCE: Glucagon-like peptide-1 (GLP-1) therapies have revolutionized the management of chronic conditions like obesity and diabetes. Consistent with the overlap between feeding and metabolic pathways and those mediating addictive behaviors, growing evidence suggests that GLP-1 therapies may also be beneficial for treating alcohol and other substance use disorders (ASUDs). This review discusses the current landscape of GLP-1 therapies in the context of ASUDs, mental health considerations, and gaps and opportunities in this field. OBSERVATIONS: Preclinical evidence across several experimen

Polysubstance and Single Substance Use Among Adults: Is Gambling a Differentiating Factor?

INTRODUCTION: The use of multiple substances including binge drinking and other substances - often referred to as polysubstance use - is generally associated with worse treatment outcomes. There has been limited research on the relationship between gambling behaviors and polysubstance use. OBJECTIVES: This study examined demographic and behavioral attributes including engagement in gambling activities in relation to polysubstance use. METHODS: Data came from a population-based sample of adults from the state of Iowa (n = 3,843). Multinomial regression using a logit link was used to model pol

Acute Coronary Syndrome and Recreational Drug Use: A Comprehensive Review.

Acute coronary syndrome (ACS) remains a leading cause of cardiovascular morbidity and mortality worldwide. Although traditional cardiovascular risk factors remain central to ACS development, recreational drug use is increasingly recognized as a clinically relevant trigger, particularly in younger patients with fewer conventional risk factors. This narrative review synthesized evidence identified through searches of PubMed/MEDLINE and Scopus up to June 2026, including clinical guidelines, systematic reviews, observational studies, mechanistic investigations, and clinically informative case-base

Chemsex among women: specifities, risks, and expectations in general practice.

BACKGROUND: Chemsex refers to the intentional use of psychoactive substances to initiate, intensify, or prolong sexual experiences. While mainly described among men who have sex with men (MSM), recent data suggests its emergence in other groups, including women. AIM: This study explores the characteristics of Chemsex among women and determines their needs and expectations in general practice. METHOD: We conducted semi-structured interviews with seven women engaging in Chemsex and living in Brussels. We then analysed our findings by using Interpretative Phenomenology Analysis (IPA) and runnin

Are School-Aged Children Who Self-Identify as Neurodivergent at Greater Risk of Substance Misuse? A Study of 20 000 Secondary School Students.

BACKGROUND: Neurodiversity is becoming a more widely used term amongst the general population, recognising neurodivergent people outside of traditional deficit-based models of neurodevelopmental conditions such as autism, ADHD, dyslexia and dyspraxia. Although neurodevelopmental conditions have been associated with a variety of externalising behaviours, including substance misuse, there is limited research looking at whether the broader neurodivergent population may also share these risks. This study's primary aim was to understand the relationship between self-reported neurodivergence and dif

The Prevalence of Anabolic-Androgenic Steroid Use Among Same-Sex Attracted Men: A Systematic Literature Review and Meta-Analysis.

ISSUES: This review aimed to estimate the prevalence of non-medical anabolic-androgenic steroid (AAS) use among same-sex attracted men (SSM). APPROACH: We conducted a systematic review of peer-reviewed articles in English, focusing on AAS use among SSM. We excluded grey literature and studies that measure AAS use through some form of analysis (e.g., hair or urine). Ten databases were searched. Titles and abstracts for all articles were screened, followed by full-text assessment and data extraction by multiple authors.
