fMRI evidence for dual routes to the mental lexicon in visual word recognition.

Event-related fMRI was used to investigate lexical decisions to words of high and low frequency of occurrence and to pseudowords. The results obtained strongly support dual-route models of visual word processing. By contrasting words with pseudowords, bilateral occipito-temporal brain areas and posterior left middle temporal gyrus (MTG) were identified as contributing to the successful mapping of orthographic percepts onto visual word form representations. Low-frequency words and pseudowords elicited greater activations than high-frequency words in the superior pars opercularis [Brodmann's are

Emotion comprehension in the temporal variant of frontotemporal dementia.

Frontotemporal dementia (FTD) is a neurodegenerative disease characterized by behavioural disorders that suggest abnormalities of emotional processing. Patients with the temporal variant of FTD (tvFTD) are particularly at risk for developing deficits in emotional processing secondary to atrophy in the amygdala, anterior temporal cortex (ATC) and orbital frontal cortex (OFC), structures that are components of the brain's emotional processing systems. In addition, previous studies have suggested that predominantly right, as opposed to left temporal atrophy is more likely to be associated with be

Reduced frontotemporal functional connectivity in schizophrenia associated with auditory hallucinations.

BACKGROUND: We used functional magnetic resonance imaging (fMRI) to investigate the frontotemporal disconnection hypothesis of schizophrenia. METHODS: Eight DSM-IV schizophrenia patients and 10 control subjects were studied with fMRI while they thought of the missing last word in 128 visually presented sentences. The fMRI data were analyzed comparing the effect of sentence completion (vs. rest) using a random effects analysis. RESULTS: There were no significant group differences in regional brain responses. Correlation coefficients between left temporal cortex (x = -54, y = -42, z = 3) and l

Magnetic source imaging in fixation-off sensitivity: relationship with alpha rhythm.

A patient in whom a variety of abnormal EEG findings can be elicited by elimination of central vision and fixation demonstrates fixation-off sensitivity. The underlying mechanisms of fixation-off sensitivity and its relationship with alpha rhythm remain unclear. To obtain a better understanding of this issue, we used a whole-head magnetoencephalograph to study an epileptic child with fixation-off sensitivity resulting in a 3-Hz, large-amplitude oscillation (300 microV) over the occipital regions on the EEG. Magnetic source localization revealed alpha activity around the calcarine fissure and s

Primary angiitis of the central nervous system as a first presentation in Hodgkin's disease: a case report and review of the literature.

OBJECTIVE AND IMPORTANCE: Granulomatous angiitis of the central nervous system is a rare cause of neurological deterioration. It is often diagnosed posthumously, and a high index of suspicion is necessary to make the correct diagnosis on a timely basis. CLINICAL PRESENTATION: A 27-year-old woman presented to the emergency room with complaints of worsening headache, nausea, and vomiting for 10 days, which were preceded by swelling of her tongue. At the examination, she had mild ocular tenderness, but no cranial nerve abnormalities. Radiographic examination revealed a right temporal lobe area w…

Unknown people believed to be known: the 'assoziierende Erinnerungs- fälschungen' by Kraepelin.

A male patient with epilepsy, who developed a unique form of paramnesia after an episode of zonisamide-induced psychosis is reported. The patient consistently mistook people who were quite new to him, such as staff of the hospital, for persons whom he had met long ago. However, he did not misidentify their names or other attributes, such as their occupations. This extraordinary form of misidentification does not fall into any known subcategory of misidentification syndromes. Rather, this paramnesia falls into the classical description of 'assoziierende Erinnerungsfälschungen' by Kraepelin. The

A case of mutism subsequent to cocaine abuse.

The authors report a case of mutism that was ultimately found to be associated with chronic cocaine use in an adult woman. Brain dysfunction was confirmed by SPECT scan, which revealed reduced cerebral blood perfusion and areas of infarction, presumably consequent to the cocaine addiction. This is the first reported case of such an association.

NGF content in the cerebral cortex of non-demented patients with amyloid-plaques and in symptomatic Alzheimer's disease.

There is increasing evidence that in Alzheimer's disease nerve growth factor (NGF) protein and NGF mRNA content in postmortem cortex is not decreased, but may even be elevated although the NGF-sensitive cholinergic basal forebrain neurons are preferentially affected. However, only little is known about the early pathophysiological events leading to Alzheimer's disease. We therefore measured the post-mortem NGF concentrations in temporal and frontal cortex of Alzheimer's disease patients, non-demented controls without Alzheimer's disease-related pathology, as well as non-demented patients with

Delirium in dementia.

Delirium is a common mental disorder in the elderly with old age being a major risk factor for delirium. Another major risk factor is dementia. The aim of the present study was to identify differences in occurrence of episodes of delirium between the most common dementia diagnoses and the possible importance of age, gender, severity and duration of dementia for the development of delirium. Included in the study were 175 consecutive patients with probable Alzheimer's disease, vascular dementia (VAD) or frontotemporal dementia (FTD) who were admitted to a neuropsychiatric diagnostic unit. There

Differentiating Alzheimer's disease and frontotemporal dementia: receiver operator characteristic curve analysis of four rating scales.

Alzheimer's disease and frontotemporal dementia are two of the most common types of degenerative dementia. In conjunction with a complete neuropsychiatric examination, the Executive Interview, Mini-Mental State Exam, Alzheimer's Disease Scale, Pick's Disease Scale and the Informant-Based Questionnaire have been used to distinguish between these two disorders on clinical grounds. In a review of two studies, scores from these rating scales are subjected to receiver operator characteristic curve analysis to determine the cut-off value for each scale which yields the highest sensitivity and specif

Ubiquitin-related cytoskeletal abnormality in frontotemporal dementia: immunohistochemical and immunoelectron microscope studies.

Although reports of dementia lacking the distinctive non-Alzheimer-type histopathology have been increasing, the concept is still far from clear. It has become apparent that this population shows neuropathological heterogeneity, and some recent reports have proposed a classification or criteria for these disease conditions. Of the reported cases, frontotemporal dementia (FTD) of motor neuron disease is unique in that the neurons of the hippocampus and entorhinal cortex have ubiquitin-related abnormalities. Recently, a new ubiquitin-related abnormality, characterized by ubiquitinated inclusions

Arteriovenous malformation associated with pleomorphic xanthoastrocytoma.

A case of a left temporo-occipital arteriovenous malformation associated with a pleomorphic xanthoastrocytoma is described. The patient had the vascular lesion with a stable right hemiparesis for many years prior to his recent clinical deterioration. Correlation is made with nine previously reported cases of angiogliomas. With the close proximity and temporal correlation of the two components of this lesion, as well as pathological evidence, the authors propose that angioglioma may be the product of reactive glial proliferation and transformation secondary to a pre-existing vascular malformati

Omental transplantation using a superficial temporal artery previously used for encephaloduroarteriosynangiosis.

BACKGROUND: Childhood moyamoya disease is a chronically progressive cerebrovascular occlusive disease affecting the territories of the anterior, middle, and posterior cerebral arteries. Surgery used in treatment of moyamoya disease to vascularize the brain include direct and indirect anastomoses. METHODS: Intracranial omental transplantation (OMT) was performed using a branch of the superficial temporal artery (STA) that had been used previously for encephaloduroarteriosynangiosis (EDAS) in five children with moyamoya disease. All five children continued to have paraparetic transient ischemic

Comparison of neuropsychological functioning in Alzheimer's disease and frontotemporal dementia.

Neuropsychological changes distinguishing mild Alzheimer's disease (AD) from frontotemporal dementia (FTD) have been described, but empirical verification of differential cognitive characteristics is lacking. Archival neuropsychological data on 15 FTD patients, 16 AD patients, and 16 controls were compared. Controls outperformed both patient groups on measures of verbal and nonverbal memory, executive ability, and constructional skill, with AD patients showing more widespread memory decline. No differences were found between the 3 groups in confrontation naming, recognition memory, or basic at

Diurnal variations of EEG power in healthy adults.

EEG power variations were evaluated in 9 healthy young adults from 8.45 a.m. to 9 p.m. and at 7 a.m. the next day. EEG signals were obtained from 16 electrodes in closed eyes and open eyes situations. Diurnal power variations were calculated for each frequency component, according to the recording situation (RS) and to the scalp site. Regarding values in the early morning on the first day, the power of almost all the frequency components showed an important diurnal increase. It came back close to initial values at 7 a.m. on the second day, which is in agreement with the existence of EEG circad

Galanin receptor binding sites in the temporal and occipital cortex are minimally affected in Alzheimer's disease.

Galanin receptor binding sites were examined in the inferior temporal and medial occipital gyri of patients with Alzheimer's disease (AD) and matched control subjects using quantitative autoradiography. In the inferior temporal gyrus, galanin binding was reduced selectively in layers V-VI of the AD cases compared to controls, the magnitude of the reduction (45%) being similar to that of choline acetyltransferase activity (40%) in this region. In the medial occipital gyrus, galanin binding in the AD cases was not different from controls in any cortical layer despite a reduction in choline acety

Initial 'schizophrenia-like' psychosis in Pick's disease: case study with neuroimaging and neuropathology, and implications for frontotemporal dysfunction in schizophrenia.

'Schizophrenia-like' psychosis has not been reported previously as a prodrome of Pick's disease, a dementia of frontotemporal pathology. A woman having a consistent clinical diagnosis of typical schizophrenia who developed increasing affectivity and cognitive deficits was examined by computed tomography and brain biopsy. This presentation was found to be associated with left frontotemporal atrophy, left sylvian fissure abnormalities and enlargement of the anterior and temporal horns of the left lateral ventricle. On biopsy, all the neuropathological hallmarks of Pick's disease were present. Un
