Bilateral simultaneous rupture of the quadriceps tendons is very rare and occurred in an 82-year-old man. This may be the fifth case to have been reported in the English literature. Only minor stress on extensor mechanisms preceded the ruptures. Interrupted mattress sutures restored muscle continuity and function. A "pull-out" wire system was not used for protection of healing muscles because adequate amount of tissue mass was available for approximation. Excellent results require intensive postoperative physical therapy. Old age of the patient should not be a deterrent to surgical treatment i

The posterolateral corner (PLC) is an important stabilizer of the knee. This complex of ligaments and tendons functions as the primary restraint to varus and posterolateral rotation of the knee. Injury to the PLC can result in chronic instability, a varus-thrust gait, and early arthrosis of the medial compartment of the knee if left untreated. Several techniques have been designed to address injuries of the PLC. Over the last 30 years, our understanding of the posterolateral corner as well as its operative reconstruction evolved. This evolution has attempted to refine what is an "anatomic" rec

Anterior cruciate ligament (ACL) tears are common and may occur in isolation or with other internal derangements of the joint. Tears of the patellar tendon (PT) occur less frequently and are rarely associated with intra-articular pathology. Acute combined tears of both the ACL and PT are known complications of high-energy traumatic knee dislocations. We present a case of an acute concomitant ACL and PT tears in a low-energy non-dislocated knee. To our knowledge, this injury has only been described in a limited number of case reports in the orthopedic literature. We present the imaging findings

Cerebral injury remains a significant complication of cardiac surgery. This complication is evaluated by clinical means that include a neurologic examination. In this report, the most important components of this type of evaluation are described. The neurologic complications of cardiac surgery can be determined by comparing structured preoperative and postoperative clinical evaluations. The neurologic examination must include a mental state examination, examination of cranial nerves, motor, sensory, and cerebellar systems, examination of gait and station, and deep tendon and primitive reflexes

Isolated rupture of the radial collateral ligament of the metacarpophalangeal joint of the fifth digit is a rare but disabling injury. Treatment is complicated by the influence of the hypothenar musculature, which deviates toward the ulna and flexes the digit. We have managed two cases, one chronic and one acute, by open reduction and repair. Both cases demonstrated distraction of the collateral ligament ends by the extensor sagittal band. This operative finding is not too dissimilar from the interposition of the adductor pollicis tendon between the ends of a ruptured ulnar collateral ligament

Tendinopathy is a common overuse injury with limited preventive options. This study evaluated a novel topical metformin lotion (ML) for its ability to prevent Achilles tendinopathy induced by intensive treadmill running (ITR) in mice. ML at concentrations of 3% and 6% was topically applied daily to the skin overlying the Achilles tendons for four weeks prior to the initiation of ITR. After the four-week topical ML pretreatment period prior to ITR induction, blood samples were collected for ELISA-based analysis of inflammatory markers (HMGB1, IL-1β, and PGE2), and tendons were examined using hi

INTRODUCTION AND IMPORTANCE: Simple closed lateral elbow dislocation is rare, often associated with neurovascular injury and closed reduction may be difficult. Open complex postero-lateral dislocation has not been previously reported.
CASE PRESENTATION: We present a case of open left posterior lateral elbow dislocation secondary to assault with a cutlass causing an avulsion fracture of the lateral epicondyle and common extensor origin tendon which prevented congruent reduction until the avulsed fragment was fixed with a bone screw.
CLINICAL DISCUSSION: The patient had hemorrhagic shock from

The musculoskeletal system is composed by hard and soft tissue. These tissues are characterized by a wide range of mechanical properties that cause a progressive transition from one to the other. These material gradients are mandatory to reduce stress concentrations at the junction site. Nature has answered to this topic developing optimized interfaces, which enable a physiological transmission of load in a wide area over the junction. The interfaces connecting tendons and ligaments to bones are called entheses, while the ones between tendons and muscles are named myotendinous junctions. Sever

PURPOSE: The incidence of extensor pollicis longus (EPL) tendon rupture in the setting of nondisplaced distal radius fractures is unknown. Extensor pollicis longus rupture is a known complication after distal radius fractures and is believed to occur more frequently after minimally displaced and nondisplaced distal radius fractures. Our study sought to define the incidence of EPL tendon rupture after nondisplaced distal radius fractures presenting to a level 1 trauma center.
METHODS: Using our billing database, we identified distal radius fractures presenting to our institution between 2006 a

There is a lack of knowledge regarding predictive factors for a successful clinical outcome after an anterior cruciate ligament (ACL) reconstruction. The aim was to investigate pre-operative factors that may predict good outcomes as measured with the subscales Function in sports and recreation and Quality of life in Knee injury Osteoarthrithis Outcome Score (QOL-KOOS), one-leg hop test and Tegner Activity Scale 12 months after ACL reconstruction. Sixty-four patients, operated with either patellar or hamstring tendon graft, were included. Demographics, range of motion, anterior knee laxity, piv

BACKGROUND: The purpose of this study was to evaluate preliminary results with Kalix subtalar arthroereisis in sinus tarsi for stage II posterior tibial tendon dysfunction.
METHODS: Twenty-one patients with stage II posterior tibial dysfunction, according to Johnson and Storm, underwent surgical treatment between July 1999 and December 2000. All patients were evaluated clinically using the America Orthopaedic Foot and Ankle Society (AOFAS) hindfoot-ankle score. We performed a tendon repair depending on the type and location of the injury and implanted a Kalix endorthesis in the sinus tarsi.

We studied a group of 602 patients who had anterior cruciate ligament reconstructions between 1987 and 1992. An autogenous patellar tendon graft was used, regardless of preexisting patellofemoral pain or chondromalacia. The surgeon and rehabilitation protocol were the same for all patients, with emphasis on obtaining full knee hyperextension postoperatively. All patients were evaluated by a questionnaire designed to determine the incidence and severity of anterior knee pain as it relates to sporting or daily living activities, prolonged sitting, stair climbing, and kneeling. Range of motion fo

OBJECTIVE: Treating wounds with exposed tendons following trauma is challenging, as addressing such injuries typically involves providing vascularised tissue coverage. Platelet-rich fibrin (PRF) is used to generate a granular wound bed for skin grafts. This study aimed to investigate the optimal wound length for complete vascular granulation using PRF treatment.
METHOD: This was a retrospective case series study, conducted in the Affiliated Hospital of Nantong University, China. Between January 2016 and January 2020, patients with tendon-exposed wounds <4cm below the knee joint following acut

OBJECTIVE: This study aimed to determine the safety and accuracy of ultrasound-guided acupotomy percutaneous loosening of the transverse carpal ligament.
METHODS: The 100 upper limb specimens were equally divided into the ultrasound-guided acupotomy group (U) and the nonultrasound-guided acupotomy group (N). For the U group, we simulated ultrasound-guided acupotomy loosening of the transverse carpal ligament in a human specimen, and for the N group, we performed the loosening of the transverse carpal ligament through the same approach under nonultrasound-guided conditions. The safety and accu
