Time and Money: The True Costs of Health Care Utilization for Patients Receiving "Free" HIV/Tuberculosis Care and Treatment in Rural KwaZulu-Natal.

BACKGROUND: HIV and tuberculosis (TB) services are provided free of charge in many sub-Saharan African countries, but patients still incur costs. METHODS: Patient-exit interviews were conducted in primary health care clinics in rural South Africa with representative samples of 200 HIV-infected patients enrolled in a pre-antiretroviral treatment (pre-ART) program, 300 patients receiving antiretroviral treatment (ART), and 300 patients receiving TB treatment. For each group, we calculated health expenditures across different spending categories, time spent traveling to and using services, and h

Medical student patient experiences before and after duty hour regulation and hospitalist support.

BACKGROUND/OBJECTIVE: With the growth of hospitalist services and the reduction in residency work hours, medical education has changed dramatically. The objective of this study was to examine changes in junior medical student-patient encounters after initiation of residency work hours and implementation of a large hospitalist practice at our academic medical center. DESIGN: Medicine clerkship students from 2002-2007 recorded the number of hospital patients and their principal diagnoses cared for during a 6-week block rotation. Comparisons were made between clerkship experiences among students

Accessibility to medical institutions in Aichi Prefecture: A geographic information systems-based analysis.

We investigated the geographic accessibility of medical institutions in Aichi Prefecture, Japan, using Geographic Information Systems (GIS). Three facility types-pharmacies, hospitals, and clinics-were analyzed across 11 secondary healthcare regions. Accessibility was evaluated based on spatial proximity by calculating population and elderly population coverage rates, defined as the proportion of residents located within the vicinity of medical institutions. Catchment areas were defined by travel mode: 400-m buffers represented a 5-min walk, and 2-km buffers approximated a 5-min drive. We test

Healthcare Cost Patterns in CDK4/6 Inhibitor-Treated HR+/HER2- Metastatic Breast Cancer.

BACKGROUND AND OBJECTIVES: Cyclin-dependent kinase 4/6 inhibitors (CDK4/6i) are standard first-line treatment for hormone receptor-positive, human epidermal growth factor receptor 2-negative (HR+/HER2-) metastatic breast cancer, yet the cost architecture across the full treatment trajectory remains poorly characterized. This study aimed to characterize phase-specific healthcare costs using a Japanese nationwide claims database. METHODS: We conducted a retrospective cohort study using the DeSC-IQVIA integrated Claims data (April 2014-May 2025). Patients with HR+/HER2- metastatic breast cancer

Adaptation in emergent adulthood for persons with prior experience in residential youth care in Norway: an observational study.

BACKGROUND: The adversities experienced by youth placed in Residential Youth Care (RYC) challenge their development through adolescence and into adulthood. Yet, we know little about how they adapt during the transition into emergent adulthood. Here, we examine their socioeconomic and civil status and psychosocial functioning, how they compare with peers, and whether there are systematic variations across sex and age. METHODS: Ten years after the initial assessment of 400 12-20-year-olds residing in RYC in Norway, we conducted a follow-up assessment when they were ages 22-30, enrolling 157 (52

Impact of strategies to improve flow and lower hospital admissions for low back pain in the emergency department: an interrupted time-series analysis.

BACKGROUND: Low back pain (LBP) is a common, disabling condition that often results in presentation to the emergency department (ED). There is currently a paucity of research examining how to optimise care for this patient group and concurrently improve service efficiency. We assessed the impact of a suite of co-designed strategies to improve the flow and reduce admission rate of people with LBP presenting to the ED. METHODS: A quasi-experimental implementation science study in a 30-bed ED at an Australian tertiary referral hospital. We implemented four improvement strategies: (1) co-design a

Oral Cephalexin Population Pharmacokinetics and Target Attainment Analysis in Infants 7-60 Days Old.

BACKGROUND: There are limited data to guide oral antibiotic dosing in neonates and young infants, particularly for intravenous (IV) to oral transition. Cephalexin is a promising oral treatment for neonatal pathogens, including Enterobacterales and methicillin-susceptible Staphylococcus aureus (MSSA). However, maturational changes in gastrointestinal absorption and kidney function during early infancy complicate extrapolation of dosing from older populations. We evaluated cephalexin pharmacokinetics and simulated dosing strategies to achieve pharmacodynamic targets in infants ≤60 days old. MET

Endothelial dysfunction accelerates AKI-to-CKD transition by promoting β-catenin activation in macrophages.

Acute kidney injury (AKI) frequently progresses to chronic kidney disease (CKD), resulting in long-term renal dysfunction. Although traditional risk factors such as hypertension, diabetes, and aging contribute to this transition, endothelial dysfunction has emerged as a central mediator. In a murine model of severe ischemia-reperfusion injury (IRI), we observed persistent fibrosis with sustained activation of β-catenin signaling, especially when there is an endothelial nitric oxide synthase (eNOS) deficiency. Impaired nitric oxide (NO)-cyclic guanosine monophosphate (cGMP)-protein kinase G (PK

Lifetime economic burden of hemophilia using a nationwide real-world healthcare data.

BACKGROUND: Patients with hemophilia require lifelong treatment, but the real-world lifetime economic burden of hemophilia remains unclear. This study aims to estimate the lifetime economic burden of hemophilia using real-world data, accounting for cost variation by disease phase and over time. METHODS: Male patients with hemophilia A (PwHa) or B (PwHb) recorded in South Korea's Health Insurance Review and Assessment (HIRA) database from 2007 to 2022 were included. Survival was estimated using the rolling extrapolation method. A phase-specific costing approach was applied, distinguishing thre

Differences between neonatal units with high and low rates of breast milk feeding for very preterm babies at discharge: a qualitative study of staff experiences.

BACKGROUND: Breast milk has significant benefits for preterm babies, but 'very preterm' babies are unable to feed directly from the breast at birth. Their mothers have to initiate and sustain lactation through expressing milk for tube feeding until their babies are developmentally ready to feed orally. There are wide disparities between neonatal units in England in rates of breast milk feeding at discharge. This study explored health professionals' experiences of barriers and facilitators to their role in supporting breast milk feeding and breastfeeding for very preterm babies. METHODS: 12 he

Novel Method for Assessment of His Bundle Pacing Morphology Using Near Field and Far Field Device Electrograms.

BACKGROUND: The 12-lead ECG is considered the gold standard to differentiate between selective (S), nonselective (NS) His bundle pacing (HBP), and right ventricular septal capture in routine clinical practice. We sought to assess the utility of device EGM recordings as a tool to identify the type of HBP morphology. METHODS: One hundred forty-eight consecutive patients underwent HBP with a 3830 Select Secure lead (Medtronic, Inc) at 3 centers between October 2016 and October 2017. The near field V-EGM morphology (NF EGM), near field V-EGM time to peak (NFTime to peak), and far-field EGM QRS du

From opiates to methamphetamine: building new harm reduction responses in Jakarta, Indonesia.

BACKGROUND: Despite the rise of stimulant use, most harm reduction programs still focus on people who inject opioids, leaving many people who use methamphetamine (PWUM) underserviced. In Asia, especially, where methamphetamine prevalence has overtaken opioids prevalence, harm reduction programs assisting PWUM are rare. The few existing innovative practices focusing on methamphetamine use lie underreported. Understanding how these programs moved their focus from opiates to methamphetamine could help inspire new harm reduction responses. Hence, this paper analyzes a newly implemented outreach pr

Social-structural factors influencing periods of injection cessation among marginalized youth who inject drugs in Vancouver, Canada: an ethno-epidemiological study.

BACKGROUND: Injection drug use is associated with HIV and hepatitis C transmission, overdose, and other preventable harms. These harms are heightened for structurally vulnerable injection drug-using populations, as their social conditions pose barriers to safer injecting. Previous research on injection cessation has largely focused on adult drug-using populations. Little qualitative work has examined the social, structural, and environmental factors that shape periods of injection cessation among youth and young adults. Such research is essential to understanding how we can best reduce harms a

DamX Controls Reversible Cell Morphology Switching in Uropathogenic Escherichia coli.

UNLABELLED: The ability to change cell morphology is an advantageous characteristic adopted by multiple pathogenic bacteria in order to evade host immune detection and assault during infection. Uropathogenic Escherichia coli (UPEC) exhibits such cellular dynamics and has been shown to transition through a series of distinct morphological phenotypes during a urinary tract infection. Here, we report the first systematic spatio-temporal gene expression analysis of the UPEC transition through these phenotypes by using a flow chamber-based in vitro infection model that simulates conditions in the b

Association Between Assistance With Medicaid Enrollment and Use of Health Care After Incarceration Among Adults With a History of Substance Use.

IMPORTANCE: The transition from prison to community is characterized by elevated morbidity and mortality, particularly owing to drug overdose. However, most formerly incarcerated adults with substance use disorders do not use any health care, including treatment for substance use disorders, during the initial months after incarceration. OBJECTIVE: To evaluate whether a prerelease Medicaid enrollment assistance program is associated with increased health care use within 30 days after release from prison. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study included 16 307 adults

Graft Loss in Pediatric and Young Adult Kidney Transplantation in New Zealand: Who Is at Greatest Risk and When?

BACKGROUND: Much is reported regarding the detrimental effect of transfer to adult services for adolescent and young adult (AYA) kidney transplant (KT) recipients. However, AYA recipient age independent of time post-KT, and not relating to transfer of care, is also a strong predictor of graft loss. We assessed KT graft survival if experiencing solely pediatric (PO) or adult services (AO) versus transfer from pediatric to adult services (PTA). METHODS: A retrospective cohort analysis of all first kidney transplant recipients between birth-24 years of age, from 2000 to 2019 in New Zealand. Part
