The Potential of Non-ablative Erbium (YAG) Laser Treatment for Complications After Midurethral Sling Surgery: A Narrative Review.

Midurethral sling (MUS) surgery, using tension-free vaginal tape and transobturator tape, has been widely adopted for the treatment of stress urinary incontinence (SUI). However, postoperative complications, including persistent urinary incontinence, mesh exposure, and pain, have become problematic, and surgical treatments for these complications face challenges, such as invasiveness, treatment-resistant cases, and recurrence. This review provides an overview of the current evidence regarding these complications and the potential of vaginal non-ablative erbium (YAG) laser (VEL) treatment as a …

Ventriculoperitoneal Shunt Failure 3-year after Shunt Surgery Caused by Migration of Detached Ventricular Catheter into the Cranium: A Case Study of Idiopathic Normal-pressure Hydrocephalus.

Idiopathic normal-pressure hydrocephalus (iNPH) is a neurological disorder that typically presents with gait disturbance, cognitive impairment, and urinary incontinence. Although most patients respond to cerebrospinal-fluid shunting, some do not react well because of shunt failure. A 77-year-old female with iNPH underwent ventriculoperitoneal shunt implantation, and her gait impairment, cognitive dysfunction, and urge urinary incontinence improved. However, 3 years after shunting (at the age of 80), her symptoms gradually recurred for 3 months and she did not respond to shunt valve adjustment.…

Shear Wave Elastography in the Evaluation of the Urethral Sphincter Complex after Radical Prostatectomy.

Post-prostatectomy pelvic floor muscle rehabilitation has a significant impact on urinary continence recovery. The aim of our study was the evaluation of urethral sphincter complex using shear wave elastography in patients after radical prostatectomy, with and without postsurgical pelvic floor muscle rehabilitation. Thirty-three patients were included in the study. The severity of urinary incontinence after radical prostatectomy was evaluated. Transrectal ultrasound examination was performed in all patients with shear wave elastography to visualize the urethral sphincter complex and estimate i…

Are male slings for post-prostatectomy incontinence a valid option?

PURPOSE OF REVIEW: Stress incontinence in men is still a common problem after surgical treatment of prostatic disease. This article reviews the techniques and results of recently described surgical slings available to treat male stress incontinence. RECENT FINDINGS: Medium-term follow-up (mean 3-4 years) of patients treated with bone-anchored slings has been recently published, suggesting a success rate (cured or improved) of 70-80%. Short-term follow-up (mean of 6-12 months) of transobturator retrourethral slings demonstrates a success rate of 62-83%. Novel sling designs include mechanisms t

Long term follow-up study of the marsupialization technique for urethral diverticula in women.

Fifty-seven women with urethral diverticula were evaluated between 1977 and 1986. Thirty-seven patients had a history of documented recurrent urinary tract infections. Other common leading symptoms were dysuria, frequency, stress urinary incontinence and dyspareunia. The diagnosis was established in 26 patients by voiding cystourethrography. A Spence marsupialization procedure was performed upon 40 patients. Postoperatively, all patients had normal voiding cystourethrograms, sterile urine and relief of symptoms. On long term follow-up study (mean of five years), 53 patients considered the oper…

Vaginal Bipolar Radiofrequency Treatment of Mild SUI: A Pilot Retrospective Study.

Background and Objectives: This retrospective study investigates the action of a bipolar, temperature controlled, endovaginal RF handpiece for the treatment of mild, moderate, and severe stress urinary incontinence with a minimally invasive approach. Stress urinary incontinence (SUI) is a common condition resulting in involuntary urine leakage, with an associated social and psychological impact. SUI is the most common type of urinary incontinence in women. Materials and Methods: We retrospectively studied 54 patients for this study. The bipolar radiofrequency energy used in all patients was 50…

Long-term transition of urinary status after robot-assisted radical prostatectomy.

To clarify and statistically analyze the long-term transition of urinary condition and quality of life (QOL) after robot-assisted radical pr … ysis (Hazard ratio = 0.50 and 0.60, P < 0.01 and < 0.01, respectively). Urinary problems other than urinary incontinence can be restored to the baseline level after RARP. QOL differed between patients who showed a "pad-free" status and "1 pad per day" status. Additionally, preoperative IPSS-S was suggested to be a predictor of both "pad-free" status and "1 pad per day …

Synchronous Bladder Neck Contracture Dilation at the Time of Artificial Urinary Sphincter Placement Is Safe and Effective.

OBJECTIVE: To treat men with bladder neck contracture (BNC) and stress urinary incontinence, neither long-term nor comparative data exist to support the superiority of simultaneous BNC intervention at the time of artificial urinary sphincter placement (synchronous) or staged BNC intervention followed by artificial urinary sphincter placement (asynchronous). This study aimed to compare the outcomes of patients treated with synchronous and asynchronous protocols. METHODS: Using a prospectively maintained quality improvement database, we identified all men between the years of 2001-2021 with a h…

Recurrent stress incontinence. Causes and treatment in the female patient.

Recurrent urinary incontinence after surgery requires complete evaluation, including urodynamic testing, to make an accurate diagnosis. Documented genuine stress incontinence, if mild to moderate, may respond to nonsurgical treatment, but more severe cases require surgery. Numerous effective surgical procedures have been developed.

Pelvic Floor Disorders.

Pelvic floor disorders commonly affect women and may cause distress and difficulty with daily functions and self-image. Urinary incontinence may present as stress incontinence, urgency incontinence, or in some combination (mixed incontinence). Symptomatic pelvic organ prolapse (POP) occurs when the patient is bothered by the sensation of a herniation of the pelvic organs through the vagina. Although POP is often distressing and embarrassing, it is not considered life-threatening unless the patient cannot urinate or defecate. There are numerous ways to treat these conditions, including conserva…

[Current diagnostics and therapy of the overactive bladder and urge incontinence].

Urinary incontinence affects millions of people worldwide and also represents a social problem. Costs of urinary incontinence and overactive bladder are very high. Urge incontinence is the involuntary loss of urine associated with a strong desire or urge to urinate. There are two types of urge incontinence: One is associated with involuntary detrusor contractions leading to a loss of urine, the other is characterized by a hypersensitive bladder in which micturition reflexes are induced due to an increased afferent activity. It is important to distinguish between an idiopathic type of urge inco…

[Medical treatment of disorders of the bladder sphincter].

A lot of drugs can increase or decrease bladder activity or urethral sphincter activity. Some of them are used for treatment of incontinence or chronic retention. In case of severe pollakiuria or urge incontinence secondary to bladder instability, numerous medications can be used: anticholinergic agents, musculotropic relaxants, calcium antagonists, prostaglandin inhibitors, beta-adrenergic agonists, tricyclic antidepressants. Anticholinergic agents are regularly used on first choice. When stress incontinence is present, alpha agonists administered with estrogen and perineal reeducation repres

Does detubularization improve continence in bladder replacement?

Camey in the seventies promoted bladder replacement. In 1987, the French Association of Urology gave us the opportunity to review 729 Tubularized Ileocystoplasty (Camey operation) [1]. The day time continence was excellent or acceptable (mild stress incontinence) on 91% of the patients, the night time continence was excellent (no pads, no leakage) or acceptable (one pad or less than 3 wakes at night) for 44% of the patients (56% had to use a device). Since 1985, the detubularization attempted to improve the continence rate. Today, the review of the literature shows that day time continence has

The management of detrusor instability.

Detrusor instability is a urodynamic diagnosis made when the detrusor is shown objectively to contract, spontaneously or on provocation, during the filling phase of a cystometrogram while the patient is attempting to inhibit micturition. It often is responsible for symptoms of urgency, frequency, nocturia, urge incontinence, and nocturnal enuresis, but is not synonymous with any of them. Furthermore, it may be responsible for urinary incontinence which appears to be simple stress incontinence, and should be excluded before an operation for genuine stress incontinence is undertaken. Patients wi…
