The cone reconstruction of the tricuspid valve in Ebstein's anomaly. The operation: early and midterm results.

OBJECTIVES: We sought to describe a new technique for tricuspid valve repair in Ebstein's anomaly and to report early echocardiographic results, as well as early and midterm clinical outcomes. METHODS: From November 1993 through August 2005, 40 consecutive patients with Ebstein's anomaly (mean age, 16.8 +/- 12.3 years) underwent a new surgical repair modified from Carpentier's procedure, the principal details of which are as follows. The anterior and posterior tricuspid valve leaflets are mobilized from their anomalous attachments in the right ventricle, and the free edge of this complex is r

Vagal control of left ventricular contractility is selectively mediated by a cranioventricular intracardiac ganglion in the cat.

Activation of the vagus nerve leads to decreases in sinoatrial (SA) rate, atrioventricular (AV) conduction, and myocardial contractility. Previous data are consistent with the hypothesis that vagal control of cardiac rate and AV conduction are mediated by two anatomically separated and physiologically independent parasympathetic intracardiac ganglia located in fat pads on the surface of the right and left atria, respectively. These data suggested that vagal control of ventricular contractility might be mediated through another intracardiac ganglion. We examined the ventricles of cat hearts his

Association of Hemispheric Laterality With Neurosurgical Intervention in Intracerebral Hemorrhage: A Posthoc Bayesian Analysis of ATACH-2 and ERICH.

BACKGROUND: Hemispheric laterality influences clinical management in stroke. Given the increasing enthusiasm for surgical management of intracerebral hemorrhage (ICH), we evaluated the association of hemispheric laterality with the likelihood of receiving neurosurgical intervention and subsequent 90-day functional outcomes. METHODS: This post hoc analysis included 3477 patients with supratentorial ICH from 2 prospective studies: the ATACH-2 trial (Antihypertensive Treatment of Acute Cerebral Hemorrhage II), conducted at 110 sites in the United States, Japan, China, Taiwan, South Korea, and Ge

Delayed depolarization and histologic abnormalities underlie the Brugada syndrome.

Brugada syndrome (BrS) is a controversial disease whose pathophysiology is still far from being fully understood. Unlike other cardiological disorders, a definite etiology has not yet been established so that it could be summarized under two main chapters: "functional" or "organic", "repolarization" or "depolarization" disorder. Despite initial descriptions leaned towards the organic substrate and delayed depolarization features, functional and repolarization theories have attracted most of the Cardiological attention for many years. Data from electrocardiography, endocavitary tracings, electr

Systemic inflammation, myocardial fibrosis, and subclinical cardiovascular disease in people living with HIV.

BACKGROUND: People living with HIV experience accelerated aging, increasing their risk of age-related diseases. Chronic inflammation may play an important role in premature vascular aging and myocardial fibrosis, leading to cardiac dysfunction. METHODS: We conducted a cross-sectional pilot study of matched veterans with and without HIV receiving care at the Providence VA Medical Center to explore the relationship between systemic inflammation, myocardial fibrosis, and subclinical cardiac disease in HIV. Participants were extensively phenotyped with questionnaires, physical exam, stress test,

The P4HA1/HIF1α feedback loop modulates endothelial dysfunction in pulmonary hypertension.

BACKGROUND: Pulmonary hypertension is a severe vascular disease characterised by pulmonary arterial remodeling and right heart failure. However, the role of the glycolytic protein P4HA1 in its pathogenesis remains elusive. METHODS: RNA-seq analysis was performed on hypoxic human pulmonary artery endothelial cells, and P4HA1 expression was validated in PH animal models. In vitro, P4HA1 knockdown was used to assess its effects on glycolysis, proliferation, and migration. Mechanistic studies included HIF1α transcriptional regulation and P4HA1-mediated HIF1α stabilization via reduced ubiquitinati

Case report: Transcatheter valve repair with Cardioband: a new treatment option for secondary tricuspid regurgitation in cardiac transplant patients.

BACKGROUND: Tricuspid regurgitation (TR) is the most frequent valvular complication after cardiac transplantation. As in native hearts, the role of surgical therapy particularly in secondary TR is unclear due to high procedural risk and unsatisfying results. Currently, percutaneous techniques are under development for TR repair with less procedural risk and promising preliminary results. CASE SUMMARY: We present a 67-year-old man who underwent heart transplantation (biatrial anastomosis) because of ischaemic heart disease 15 years ago and aortic valve replacement in 2010. Because of progressi

Tricuspid regurgitation after successful mitral valve surgery.

The tricuspid valve (TV) is inseparably connected with the mitral valve (MV) in terms of function. Any pathophysiological condition concerning the MV is potentially a threat for the normal function of the TV as well. One of the most challenging cases is functional tricuspid regurgitation (TR) after surgical MV correction. In the past, TR was considered to progressively revert with time after left-sided valve restoration. Nevertheless, more recent studies showed that TR could develop and evolve postoperatively over time, as well as being closely associated with a poorer prognosis in terms of mo

Left ventricular endocardial pacing improves resynchronization therapy in canine left bundle-branch hearts.

BACKGROUND: We investigated the benefits of the more physiological activation achieved by left ventricular (LV) endocardial pacing (ENDO) as compared with conventional epicardial (EPI) LV pacing in cardiac resynchronization therapy. METHODS AND RESULTS: In 8 anesthetized dogs with experimental left bundle-branch block, pacing leads were positioned in the right atrium, right ventricle, and at 8 paired (EPI and ENDO) LV sites. Systolic LV pump function was assessed as LVdP/dtmax and stroke work and diastolic function as LVdP/dtmin. Electrical activation and dispersion of repolarization were det

Longitudinal in vivo SPECT/CT imaging reveals morphological changes and cardiopulmonary apoptosis in a rodent model of pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) has a complex pathogenesis involving both heart and lungs. Animal models can reflect aspects of the human pathology and provide insights into the development and underlying mechanisms of disease. Because of the variability of most animal models of PAH, serial in vivo measurements of cardiopulmonary function, morphology, and markers of pathology can enhance the value of such studies. Therefore, quantitative in vivo SPECT/CT imaging was performed to assess cardiac function, morphology and cardiac perfusion utilizing (201)Thallium ((201)Tl) in control and mon
