Tonsil volume, tonsil grade and obstructive sleep apnea: is there any meaningful correlation?

OBJECTIVES: The aims of this study were to evaluate the correlation between oropharyngeal examination and objective palatine tonsil volume in snoring adults and verify the influence of the oropharyngeal anatomy, body mass index, age, and severity of obstructive sleep apnea on actual tonsil volume. In addition, we aimed to assess the influence of tonsil size on obstructive sleep apnea in adults. INTRODUCTION: Pharyngeal wall geometry is often altered in adults who have obstructive sleep apnea, and this might influence the findings of the oropharyngeal examination that, in turn, are the key fac

Phylogenetic constraints on digesta separation: Variation in fluid throughput in the digestive tract in mammalian herbivores.

The relevance of the mean retention time (MRT) of particles through the gastrointestinal tract (GIT) is well understood and MRT(particle)GIT is an important parameter in digestion models. Solute markers have been used to estimate MRT(solute)GIT (or 'fluid passage') in animals, but the relevance of this measure is less evident and is usually sought in its relation to MRT(particle)GIT. The ratio between the two measures indicates the degree of 'digesta washing', with little washing occurring at ratios of 1, aborad washing at ratios >1 (where the solute marker travels faster than the particle mar

[Blood-nerve barrier: structure and function].

The blood-nerve barrier (BNB) is a dynamic interface between the endoneurial microenvironment and surrounding extracellular space or blood contents, and is localized the innermost layer of multilayered ensheathing perineurium and endoneurial microvessels. Since the BNB is a key structure controlling the internal milieu of the peripheral nerve parenchyma, adequate understanding of the BNB is crucial for developing treatment strategies for human peripheral nervous system disorders, including Guillain-Barré syndrome, chronic inflammatory demyelinating polyneuropathy, and diabetic and various meta

Coronary CT angiography in step-and-shoot technique with 256-slice CT: impact of the field of view on image quality, craniocaudal coverage, and radiation exposure.

PURPOSE: To evaluate the effect of a small field of view (FOV) for step-and-shoot coronary computed tomography angiography (CCTA) on craniocaudal z-coverage per scan step, image quality, and radiation exposure. METHODS: 53 patients underwent prospectively ECG-gated CCTA on a 256-slice MDCT scanner using either a FOV > 250 mm (group 1, n = 29) or a FOV ≤ 250 mm (group 2, n = 24). Craniocaudal z-coverage was determined on coronal multiplanar reformations. Image noise, signal-to-noise ratio, contrast-to-noise ratio, and qualitative image parameters were assessed. Radiation dose was estimated fro

Long-term results of the amplatzer cribriform occluder for patent foramen ovale with associated atrial septal aneurysm: impact on occlusion rate and left atrial functional remodelling.

BACKGROUND: Treatment of patients with concomitant patent foramen ovale (PFO) and atrial septal aneurysm (ASA) poses a number of challenges; while some authors have suggested the off-label use of the Amplatzer Cribriform Occluder in such anatomy, the long-term outcomes of this strategy is unknown. Our study aimed to assess the long-term impact on closure rate, left atrial functional remodelling, and clinical outcomes of off-label implantation of Amplatzer ASD Cribriform Occluder in patients with PFO and ASA. METHODS: We prospectively enrolled 160 consecutive patients with previous stroke (mea

Rhinosurgery in children: developmental and surgical aspects of the growing nose.

The anatomy of the nasal skeleton in newborns and adults are not alike. The complete cartilaginous framework of the neonatal nose becomes partly and gradually ossified during the years of growth and is more vulnerable to trauma in that period. Injury in early youth may have large consequences for development and may result in a nasal deformity which will increase during growth and reach its peak during and after the adolescent growth spurt. To understand more of the underlying problems of nasal malformations and their surgical treatment (septorhinoplasty) these items became the focus of multip

Muscle hypoxia in rheumatoid hands: does it play a role in ulnar drift?

PURPOSE: The cause of ulnar drift in patients with rheumatoid arthritis (RA) is unknown. It may occur because of external forces applied to the fingers during normal use. Alternatively, it may arise after changes in the internal forces on the anatomy of the digits owing to alterations in the supporting structures of the joints or their control mechanisms, or both. Intrinsic muscle tightness, which is commonly seen in RA hands, may be the result of adaptive shortening or a direct consequence of RA. Previous studies carried out by our group have shown that joints, tendons, and associated synoviu

Efficacy of intraoperative surgical irrigation with polihexanide and nitrofurazone in reducing bacterial load after nail removal surgery.

BACKGROUND: A common challenge of nail avulsion surgery is the associated bacterial contamination and infection that can manifest. The toe has a difficult anatomy to antiseptically prepare and properly maintain throughout the surgical procedure, lending to this widespread problem. OBJECTIVE: We conducted a controlled, prospective randomized study to examine the antiseptic efficacy of 3 intraoperative irrigation methods during nail avulsion surgery. METHODS: We compared intraoperative antiseptic irrigation using 0.9% saline solution (24 patients), 0.2% nitrofurazone (22 patients), and 0.1% po

MR imaging of optic neuropathy with extended echo-train acquisition fluid-attenuated inversion recovery.

BACKGROUND AND PURPOSE: XETA, also known as Cube, is a relatively new 3D FSE sequence that can be used to perform whole-brain FLAIR T2-weighted imaging at isotropic high spatial resolution. This high-resolution volumetric imaging, coupled with both fat and fluid suppression, permits ideal evaluation of optic nerve anatomy and signal intensity; therefore, we hypothesized that XETA FLAIR would be useful for the detection of T2 signal-intensity abnormality in the optic nerve. Our purpose was to evaluate the sensitivity of XETA FLAIR for this abnormality and to compare it with the coronal FSE T2 F

The first 50s: can we achieve acceptable results in vestibular schwannoma surgery from the beginning?

OBJECTIVE: Vestibular schwannoma surgery requires a profound knowledge of anatomy and long-standing experience of surgical skull base techniques, as patients nowadays requests high-quality results from any surgeon. This educes a dilemma for the young neurosurgeon as she/he is at the beginning of a learning curve. The presented series should prove if surgical results of young skull base surgeons are comparable respecting carefully planned educational steps. METHODS: The first 50 vestibular schwannomas of the first author were retrospectively evaluated concerning morbidity and mortality with an

The safety of same-day endoscopy and percutaneous liver biopsy.

BACKGROUND AND AIMS: The aim of this study was to review our experience with same-day endoscopy (SDE) plus percutaneous liver biopsy (PLB) and to evaluate its safety compared to PLB alone. METHODS: We retrospectively examined records of all patients who underwent PLB between January 2003 and September 2009 and identified those who underwent SDE and matched these patients to those undergoing PLB alone. Serious adverse events (SAEs) were analyzed using our endoscopic database (EndoPro, Pentax) and were divided into those occurring immediately post-procedure and those occurring after discharge.

Cartilage regeneration and repair testing in a surrogate large animal model.

The aging human population is experiencing increasing numbers of symptoms related to its degenerative articular cartilage (AC), which has stimulated the investigation of methods to regenerate or repair AC. However, the seemingly inherent limited capacity for AC to regenerate persists to confound the various repair treatment strategies proposed or studied. Animal models for testing AC implant devices and reparative materials are an important and required part of the Food and Drug Administration approval process. Although final testing is ultimately performed in humans, animal testing allows for

Construction and evaluation of an anatomically correct multi-image modality compatible phantom for prostate cancer focal ablation.

PURPOSE: Focal therapy using lasers is emerging as an alternative strategy for prostate cancer treatment. However, to our knowledge no anatomically correct models are available to test imaging and ablation techniques. Animal models present ethical, anatomical and cost challenges. We designed and validated an inexpensive but anatomically correct prostate phantom incorporating tumor, rectum and urethra that can be used for simulated and experimental magnetic resonance guided focal intervention. Our secondary aim was to asses the phantom using other imaging modalities. MATERIALS AND METHODS: The

Microanatomical and immunohistochemical study of the human anterior branch of the medial antebrachial cutaneous nerve of forearm at the antecubital fossa and its clinical implications.

PURPOSE: Poor prognosis of nerve repair in patients may be due to changes in intraneural anatomy with age. Also, chances of Complex Regional Pain Syndrome-Type I (CRPS-I) secondary to peripheral nerve injury are comparatively high. The present study is to find the fascicular pattern of the anterior branch of the medial antebrachial cutaneous nerve of forearm (MACN) (at antecubital fossa), microanatomic morphometric characteristics of its connective tissue components (adipose tissue) and changes with age and study of intraneural sympathetic fiber content. MATERIAL AND METHODS: Sixty six human

Human neural stem cell grafts in the spinal cord of SOD1 transgenic rats: differentiation and structural integration into the segmental motor circuitry.

Cell replacement strategies for degenerative and traumatic diseases of the nervous system depend on the functional integration of grafted cells into host neural circuitry, a condition necessary for the propagation of physiological signals and, perhaps, targeting of trophic support to injured neurons. We have recently shown that human neural stem cell (NSC) grafts ameliorate motor neuron disease in SOD1 transgenic rodents. Here we study structural aspects of integration of neuronally differentiated human NSCs in the motor circuitry of SOD1 G93A rats. Human NSCs were grafted into the lumbar prot
