High-grade dysplasia (HGD) in the cystic duct is a rare epithelial lesion that may lead to biliary tract malignancy. Choledochal cysts involving the cystic duct are extremely rare, and are usually associated with cystic dilatations of the extrahepatic biliary tract. We describe a patient who presented with jaundice and was found to have a dilatation of the common bile duct on computed tomographic imaging, consistent with a choledochal cyst. A retrospective review of 24 patients treated at the University of Chicago for pyogenic liver abscess from 1979 to 1989 was performed to determine if there is a changing etiology in pyogenic liver abscess and if percutaneous drainage is a reliable treatment alternative to open drainage. The results of diagnosis and treatment of iatrogenic injuries of the extrahepatic biliary tract in 47 patients are analysed. The injury was detected during the operation only in 6 cases, in 41 cases it was revealed on the 1st-6th postoperative day.
