Ketoconazole gel 2% in the treatment of moderate to severe seborrheic dermatitis.

Seborrheic dermatitis traditionally has been treated with topical steroids. In current practice, however, antifungal agents such as ketoconazole often are used because Malassezia yeasts are thought to play a role in the disease pathogenesis. Ketoconazole gel 2% has been developed for the once-daily treatment of seborrheic dermatitis. This gel is almost invisible after application, unlike ketoconazole cream, and may offer advantages in patient acceptance and adherence to treatment. Three randomized, double-blinded, vehicle-controlled, multicenter, parallel-group phase 3 studies evaluated the ef…

Widespread granulomatous dermatitis of infancy: an early sign of Blau syndrome.

BACKGROUND: Pediatric sarcoidosis has traditionally been divided into 2 distinct groups: (1) school-aged children and adolescents with frequ … previously been detected in Blau syndrome kindreds. CONCLUSION: We draw attention to granulomatous dermatitis as an early manifestation of Blau syndrome and highlight emerging molecular evidence that this heritable autoinflammatory disorder and early-onset sarcoidosis represent a single disease entity.

Filaggrin's fuller figure: a glimpse into the genetic architecture of atopic dermatitis.

Prevalent mutations in the FLG gene underlie the common skin disorder ichthyosis vulgaris and are significant risk factors for atopic dermatitis (eczema). The recent publication of a strategy to sequence this difficult gene identifies a spectrum of both prevalent and rare mutations that collectively have a significant impact on susceptibility to atopic disease.

Three cases of severe Rhus dermatitis in an English primary school.

We report three paediatric cases of severe allergic contact dermatitis caused by a Japanese lacquer tree (Rhus verniciflua), which is a rare specimen plant in the UK. The diagnosis of allergic contact dermatitis produced by plants that are not indigenous to a particular country is more likely to be delayed, as well as mistaken for cellulitis.

Study on the mechanism of photosensitive dermatitis caused by ketoprofen in the guinea pig.

To investigate the mechanism on photosensitive dermatitis caused by ketoprofen (KP) in humans, the following experiments were performed by topical application on guinea pigs. The phototoxicity study involving treatment with 10% solution of KP, its enantiomers (R-KP and S-KP), loxoprofen, and flurbiprofen revealed no phototoxic reactions. In the photoallergenicity study, KP and its enantiomers (0.5-2% solution) induced skin reaction at all dosages; however, loxoprofen and flurbiprofen (1-5% solution) did not induce such a photoallergenic reaction. These results suggest that the chemical structu…

Allergic contact dermatitis to sodium benzoate chloroacetamide in a sorbolene lotion.

Sodium benzoate chloroacetamide is sometimes used as a combination preservative in cosmetics to prevent the growth of bacteria, mould, fungi and yeasts. A case of occupational allergic contact dermatitis to this preservative is described in a 32-year-old female personal care assistant, who was found to have allergic contact dermatitis involving the hands, caused by the sorbolene lotion supplied in her workplace. Patch testing to chloroacetamide, sodium benzoate chloroacetamide and the sorbolene lotion was positive. Patch testing to sodium benzoate alone was negative. She had been found to reac…

[What additional measures should be recommended in atopic dermatitis in children?].

The so-called 'adjuvant' measures are an important part of atopic dermatitis (AD) consultations. The practitioner is the 'expert' in the patients' eyes in prescribing, proposing, counselling and replying to the questions concerning moisturizers, thermal spring water cures, the resort to alternative medical, and vaccinations. Moisturizers are aimed at rapidly restoring water in the epidermis, decreasing the sensitivity to irritants and improving the patients' comfort. The available products are usually composed of water, occlusive agents, humidifiers, varyingly combined with tensioactive agents…

[Use of clarithromycin in the treatment of atopic dermatitis complicated by staphylococcal infection].

Thirty patients (19 boys and 11 girls) with atopic dermatitis complicated by pyodermia were treated with clarithromycin tablets (Fromilid, KRKA, Slovenia) in a dose of 7.5 mg/kg body weight twice a day. The average treatment course was 7 days. The severity of atopic dermatitis was evaluated by the SCARAD Index. Thirty patients of the reference group were treated with intramuscular lincomycin in a dose of 10-20 mg/kg body weight twice a day for 7 days. The difference in the average SCORAD Index before and after the treatment with clarithromycin amounted to 19.95 vs 14.68 (p < 0.005) with the us…

[The presence of CD30 on lymphocytes in the inflammatory infiltrate of acute atopic dermatitis].

Atopic dermatitis (AD) has cellular immunohistochemical features similar to those of allergic contact dermatitis (ACD). There is plenty of evidence for T-cell activation in this disease such us the presence of T-lymphocytes which carry CD30, CD3, CD4, CD45RO markers. The aim of this study was to show the presence of lymphocyte-surface antigens including CD30, CD3, CD4, CD45RO in patients with acute AD and to compare the presence of the same molecules in patients with acute ACD (nickel-induced) because of the possible morphologic difference of these two entities. The presence of the stated mole…

Allergic contact dermatitis in children: strategies of prevention and risk management.

Over recent years, allergic contact dermatitis in children has repeatedly been reported as a significant clinical problem. It is generally accepted that allergic contact dermatitis is rare in the first years of life, and with increasing age (by the age of 10 years) reaches the incidence seen in adults. As in adults, metals are one of the most common sensitizers in children, along with rubber chemicals and fragrances. The influence of fashion trends and lifestyle such as piercing, decorative skin paintings, the hype of natural remedies and cosmetics (e.g. tea tree oil) or the use of cosmetical …

Epithelial barrier function and atopic diathesis in rosacea and perioral dermatitis.

BACKGROUND: Rosacea and perioral dermatitis (PD) are common dermatoses, the aetiology and pathogenesis of which remain speculative. Objectives To investigate skin barrier function and features of atopy in both diseases. METHODS: We studied 75 patients with rosacea and 75 with PD. Transepidermal water loss (TEWL) was measured in three regions of the face (lateral chin, perinasal cheek, side of the nose) and the patients were assessed for atopy by clinical criteria, prick tests and specific IgE against a mixture of aeroallergens (CAP SX1). The control group consisted of 125 individuals with no …

Long-term remission in patients with dermatitis herpetiformis on a normal diet.

BACKGROUND: A life-long gluten-free diet is the treatment of choice for dermatitis herpetiformis, which is considered to be coeliac disease of the skin. OBJECTIVES: To investigate the effects on long-term remission of dermatitis herpetiformis in patients who underwent a gluten challenge and subsequently reintroduced dietary gluten. PATIENTS AND METHODS: We studied 38 patients (14 male and 24 female) with biopsy-confirmed dermatitis herpetiformis. They had followed a gluten-free diet for a mean of 8 years, achieving clinical remission and intestinal normalization. The patients were asked to r…
