Dupilumab-induced ocular surface disease: a primer.

The management of atopic diseases has been revolutionised by precision therapies and biological drugs that target specific immune proteins. This report elucidates a unique complication from the use of the monoclonal antibody, dupilumab, that primary care providers and subspecialists need to be aware of. A patient in her 40s consulted us for severe atopic asthma, food allergy and eczema involving the face and body. She had previously failed treatments and was started on dupilumab (which binds to the interleukin-4 [IL4] receptor and inhibits both IL-4 and IL-13). She quickly achieved remission o

Assessment of childhood allergy for the primary care practitioner.

PURPOSE: To update the primary care practitioner on the assessment of common childhood allergic illnesses. DATA SOURCES: Relevant scientific literature and published clinical practice guidelines. CONCLUSIONS: Atopic children often develop symptoms that occur in a predictable progression from atopic dermatitis to gastrointestinal disturbances, chronic serous otitis media, rhinitis, and asthma. Evaluation of allergic symptoms should be based on their chronicity, family history of atopy, and knowledge of how the information will change patient management. Both skin and blood testing are accurat

PDE4 Inhibitor-Responsive Dermatoses: An Emerging Concept in Dermatology.

Many inflammatory skin conditions share mechanistic pathways involving multiple receptors and cytokines. Perhaps there is no better endorsement of this idea than the foundational concept of "steroid-responsive dermatoses." Similarly, targeting the phosphodiesterase 4 (PDE4) enzyme, which exists at the crux of several pathways that contribute to inflammatory skin diseases, offers an avenue by which the pathogenesis of multiple conditions may be interrupted. This suggests that PDE4 inhibitors may assume a similar complementary framework for treating "PDE4 inhibitor-responsive dermatoses." Certai

Novel Clinical Applications of Topical Ruxolitinib: A Case Series.

Topical ruxolitinib, a potent Janus kinase (JAK) inhibitor, has shown significant efficacy in treating inflammatory skin conditions. While its use has already been established in atopic dermatitis and vitiligo, recent reports suggest its potential efficacy in treating other dermatoses. Specifically, topical ruxolitinib may be an effective treatment option for refractory dermatological conditions that are inflammation-driven with dysregulated activity of cytokines implicated in the JAK/STAT pathway. In this case series, we present four novel clinical applications of topical ruxolitinib in treat

Allergies and skin testing: a Nairobi experience.

A prospective study of 72 consecutive patients with symptoms of allergy is presented. Patients aged 20-39 years formed the bulk of the study population (55.6%). Most patients had allergic rhinitis (48.6%); bronchial asthma and atopic dermatitis occurred in 29 patients (40.3%) each. Drug allergies were found in 22 patients (30.6%), urticaria in 15 (20.8%), food allergies in 13 (18.1%), contact dermatitis in 10 (13.9%) and allergic conjuctivitis in 6 patients (8.3%). In many cases more than one allergic condition appeared simultaneously. Skin and prick tests and skin patch tests were done on 46

Burden of immune-related skin diseases worldwide, 1991-2021: insights and prediction from the Global Burden of Disease Study.

BACKGROUND: Immune-mediated dermatological conditions, including dermatitis, urticaria, alopecia areata, and psoriasis, are common skin diseases that contribute to substantial health loss, economic burden, and pain across individuals of all ages worldwide. METHODS: Using data from the Global Burden of Disease (GBD) 2021 study, we analyzed age-standardized incidence, prevalence rate, and disability-adjusted life years (DALYs) for global main four immune-related skin diseases-including dermatitis (atopic, contact, and seborrheic), urticaria, alopecia areata, and psoriasis from 1991 to 2021, wit

Oclacitinib (APOQUEL®) is a selective Janus kinase 1 inhibitor with efficacy in a canine model of flea allergic dermatitis.

Oclacitinib is a novel Janus kinase (JAK) inhibitor that potently inhibits JAK1-dependent cytokines involved in allergy, inflammation, and pruritus (IL-2, IL-4, IL-6, IL-13, and IL-31). Oclacitinib (Apoquel®, Zoetis Inc, Parsippany, NJ) is approved for the treatment/control of pruritus associated with allergic dermatitis and treatment/control of clinical manifestations of atopic dermatitis in dogs at least 12 months of age. To evaluate the effectiveness of oclacitinib in dogs with flea allergy dermatitis, the JAK1 selective inhibitor was tested in a placebo-controlled, masked, single-dose (0.4

Causal associations between fluid intake patterns and dermatitis risk: a Mendelian randomization study.

BACKGROUND: Dermatitis is one of the most common skin disorders across the world. Atopic dermatitis (AD) and contact dermatitis (CD) are its two primary types. Few studies have focused on the causal relationship between fluid intake and dermatitis. With an Mendelian Randomization (MR), this study investigated the potential causal effects of alcohol, coffee, tea, and water intake on the risk of AD and CD. METHODS: Utilizing genetic variants as instrumental variables (IVs), a two-sample MR analysis was implemented based on data from the UK Biobank and FinnGen r9 consortium. Fluid intake was cat

Epidemiologic characteristics of rhinitis in Turkish children: the International Study of Asthma and Allergies in Childhood (ISAAC) phase 2.

Rhinitis is a common problem with important comorbidities. In order to search the association between rhinitis, allergic phenotypes and other risk factors in Turkish children, a parental questionnaire about allergic diseases and risk factors, and skin prick test (SPT) with 13 inhalant allergens were performed in a population-based sample of 2774 children aged 9-11 yr. Bronchoprovocation testing with hypertonic saline (HS)and total IgE analysis were limited to a subsample of 350 children. Rhinitis was defined as a problem with sneezing, rhinorrhea, or nasal congestion when the child did not hav

Does Previous Anaphylaxis Determine Differences Between Patients Undergoing Oral Food Challenges to Cow's Milk and Hen's Egg?

Background: Oral food challenges (OFCs) are still the reference standard for confirming food allergy, yet the influence of previous anaphylaxis on challenge outcomes remains uncertain. Patients with a history of anaphylaxis are often considered at higher risk, which may affect the clinical decision-making process. This study aimed to identify predictors of OFC failure stratified by a history of anaphylaxis, given that prior investigations have predominantly considered anaphylaxis as an overall risk factor, without delineating distinct risk factor profiles according to anaphylaxis history. Analysis of the Conjunctival Microbiome in Patients with Atopic Keratoconjunctivitis and Healthy Individuals.

BACKGROUND: Atopic dermatitis (AD) has been clarified that imbalance of bacterial and fungal communities in the skin and gut play key roles in immunologic dysfunction. Atopic keratoconjunctivitis (AKC), one of severe ophthalmic manifestation of AD, could be related with dysbiosis as same as AD. OBJECTIVE: In this case-control study, the roles of conjunctival microbial communities in AKC were evaluated by a comparative analysis with healthy controls (HCs). METHODS: 16S rRNA sequencing was used to construct libraries of compositional information for a total of 30 volunteers including 20 patien

[Bronchial asthma. Long-term therapy and prognosis].

Bronchial asthma is not a disease entity but a nonspecific reaction of the lung characterized by recurrent reversible airway obstruction. Successful treatment depends on identification of the type of asthma according to causative factors such as allergens, respiratory tract infections, complicating factors (sinusitis, smoking parents) and the degree of severity. For allergic or mixed asthma the identification and elimination of relevant allergens may be the only therapy required. Drugs for long-term treatment are Beta-2-mimetics, chromolynglycate and theophylline as a slow-release preparation.

Alopecia Areata Is Associated with Atopic Diathesis: Results from a Population-Based Study of 51,561 Patients.

BACKGROUND: Evidence of TH1/IFN-γ overactivation as a major pathogenic driver somewhat conflicts with data supporting robust allergic background in patients with alopecia areata (AA). Previous investigations of immunological dysregulations show that both TH1- and TH2-related markers are overexpressed in AA. Clinical correlations in large populations may shed light on the immune pathways most likely to result in the clinical phenotype of AA. OBJECTIVE: To investigate the atopic comorbidities among patients with AA in a large population-based study. METHODS: This is a cross-sectional retrospec

Prenatal prediction of infant atopy by maternal but not paternal total IgE levels.

BACKGROUND: The atopic history of parents has long been used to predict infant atopy. However, bias from questionnaires of allergic history are also frequently suspected, because a large number of vasomotor rhinitis, intrinsic asthma, and seborrheic dermatitis cases are probably misinterpreted to be atopic diseases. OBJECTIVE: We attempted to identify a risk factor other than parental atopic history to predict elevated infant IgE levels and infant atopy. METHODS: A total of 655 core families were prenatally recruited, and finally 545 families completed the study for the prospective analysis

Candida parapsilosis in a case of canine otitis media.

Candida parapsilosis, commonly present on the skin and mucous membranes, has been described as an opportunistic pathogen increasingly associated with infections. We present the first documented case of severe otitis media caused by this yeast species in a French Bulldog with a history of atopic dermatitis and seborrhea. The diagnosis was confirmed by culture and molecular methods. Susceptibility testing showed resistance only to fluconazole; however, antifungal therapy could not be initiated, as the patient died in an emergency room. This case highlights the potential of C. parapsilosis to cau

Use of dupilumab for recalcitrant bullous pemphigoid: A case report.

Bullous pemphigoid is an autoimmune blistering disease affecting the dermo-epidermal junction, most commonly seen in older patients. First-line treatment includes systemic, topical corticosteroids and/or steroid-sparing immunosuppressants. Treatment with these medications may be limited by their safety profile. Dupilumab is a humanized monoclonal antibody targeting interleukin-4 and interleukin-13 cytokines currently indicated for moderate-to-severe atopic dermatitis, severe asthma, chronic rhinosinusitis with nasal polyposis, and moderate-to-severe prurigo nodularis. We report a case of a pat

A Comprehensive Review on Allergic Disorders, Their Epidemiological Trend and Barriers in Management.

Allergy is an important public health disorder. Common allergic disorders include asthma, rhinitis, conjunctivitis, atopic eczema, contact dermatitis, and food and drug allergies. In this article, allergic disorders are discussed comprehensively, along with their epidemiological trends, quality of care, and barriers in management. Allergic disorders have increased in prevalence throughout the world, except for a few foci (e.g., Spain) where asthma is decreasing. Prevention of allergen exposure, pharmacotherapy, and immunotherapy are different modes of controlling allergy. Extensive training, r

Hypothesis: Viral infections of pregnant women may be early triggers of childhood type 1 diabetes and other autoimmune disease.

Children and adolescents with early onset autoimmune diseases have a different seasonality of month of birth than the general population. This pattern is consistent with an infection during pregnancy affecting the fetus or an infection immediately after birth that act as early triggers of the autoimmune diseases. We present data supporting the use of Rotavirus vaccinations in the reduction of incidence of childhood T1D and propose further investigations into whether other anti-virus vaccinations may reduce the burden of other autoimmune diseases such as multiple sclerosis, atopic dermatitis, p

Bacterial infections are common among eczema herpeticum inpatients in a cross-sectional analysis of the 2016 Kids' Inpatient Database.

Eczema herpeticum (EH) is a cutaneous infection with herpes simplex virus, typically in the context of atopic dermatitis. Pediatric hospitalizations for EH have increased over the past several decades, yet few studies have investigated comorbidities or epidemiology of pediatric EH inpatients. We searched the 2016 Kids' Inpatient Database and recorded demographics, comorbidities, and associated costs for patients with EH. We found bacterial infections were a common comorbidity which affected nearly half (47.4%) of hospitalized patients, suggesting the importance of regular monitoring of pediatr
