Bilateral secondary neurolymphomatosis of the internal auditory canal nerves: a case report.

BACKGROUND: Neurolymphomatosis describes the malignant lymphomatous infiltration of nerves. METHODS: We encountered a unique case of a 47-year-old patient with non-Hodgkin's lymphoma presenting with bilateral sensorineural hearing loss, vestibular dysfunction and bilateral facial nerve palsy. RESULTS: Magnetic resonance imaging demonstrated enhancement and thickening of internal auditory canal nerves bilaterally consistent with neurolymphomatosis. Patient was treated with combined intrathecal chemotherapy and total brain irradiation. CONCLUSIONS: One must always remain vigilant for metastat

Analysis of speech perception outcomes among patients receiving cochlear implants with auditory neuropathy spectrum disorder.

OBJECTIVE: Cochlear implantation (CI) is currently the main device option for children with auditory neuropathy spectrum disorder (ANSD) who receive minimal benefit from conventional amplification. This study examines potential prognostic factors associated with post-CI speech performance in this population. STUDY DESIGN: Retrospective chart review. SETTING: Academic center. PATIENTS: ANSD patients without inner ear abnormalities implanted with unilateral or bilateral CI between 1998 and 2010. INTERVENTION: CI and speech perception testing. MAIN OUTCOME MEASURE: Post-CI speech perception

Magnetic resonance imaging fails to show evidence of reduced endolymphatic hydrops in gentamicin treatment of Ménière's disease.

OBJECTIVE: To verify the hypothesis that intratympanic (IT) gentamicin (Gent) treatment in Ménière's disease (MD) is capable of reducing endolymphatic hydrops (EH), as evaluated by 3-dimensional fluid-attenuated inversion recovery (3D-FLAIR) sequence in a 3-Tesla magnetic resonance imaging (MRI) unit, after IT gadolinium administration. PATIENTS: A total of 8 patients (5 men and 3 women; aged 40-78 yr; median, 60 yr) with definite MD participated in the investigation. The duration of the disease ranged from 1 to 10 years (median, 4 yr), with a prevalence of vertigo spells of 1 to 6 per month

Lateral semicircular canal plugging with endolymphatic sac decompression as new surgical treatment for intractable Meniere's disease.

CONCLUSION: Lateral canal plugging with endolymphatic sac decompression (LCPESD) is a new therapeutic approach to intractable Meniere's disease. Further clinical data will be required. OBJECTIVE: The objective of this paper is to present a new surgical procedure, i.e. lateral canal plugging (LCP) with endolymphatic sac decompression (ESD), which we defined as LCPESD. Two cases of intractable Meniere's disease, a 48-year-old male and a 48-year-old female, were successfully treated by this method. METHODS: LCPESD was performed under general anesthesia. First ESD was undertaken followed by LCP.

Sublocalization and volumetric growth pattern of intracanalicular vestibular schwannomas.

OBJECTIVE: None of several previous reports on the growth pattern of vestibular schwannomas (VS) have dealt with the sublocalization and volumetric growth pattern of intracanalicular tumors. This paper reports such data from 196 patients. STUDY DESIGN: All VS patients have been registered prospectively at one center in Denmark since 1975. Data on intracanalicular tumors were drawn from the database, yielding 196 patients with a diagnostic and at least one control magnetic resonance imaging scan. All images were retrieved and the tumor sublocalization, size, and growth rate determined. RESULT

Clinical examination of labyrinthine-defective patients out of the vertigo attack: sensitivity and specificity of three low-cost methods.

Many reports have appeared in the medical literature concerning the clinical examination at the bedside of patients with vertigo and, even if few controversial opinions exist, the observation of one or more kinds of nystagmus is generally regarded as suggesting an organic aetiology. So far, the presence of nystagmus has been generally considered to be crucially important for clinicians who are daily asked to differentiate between an "organic" cause of vertigo (for example, a labyrinthine dysfunction) and a "non-organic" cause of vertigo, such as a panic disorder. Albeit, it should not be forgo

Translabyrinthine removal of large acoustic neuromas in young adults.

OBJECTIVE: the authors reviewed the clinical manifestations and the surgical outcomes in a series of young patients who underwent removal of large acoustic neuromas via the translabyrinthine approach. METHODS: 40 young adults who underwent a translabyrinthine removal of acoustic neuromas 3 cm or greater in size were analyzed. The patient's age ranged from 17 to 30 years. The mean size of tumor was 4.25 cm. RESULTS: the primary symptoms are similar to those in adult but usually less intense. The average interval time between the primary symptom and the diagnosis was 17 months. A high percenta

Labyrinthine involvement and multiple perforations of the tympanic membrane in acute otitis media due to group A streptococci.

We present here three cases of acute otitis media caused by a virulent group A streptococcal infection that rapidly led to deterioration in hearing. Two of the three cases presented with severe sensorineural and mixed hearing loss with multiple tympanic membrane perforations, and the third presented with severe bilateral sensorineural hearing loss following acute otitis media involving group A streptococci. All patients were treated with systemic (piperacillin) and topical antibiotics (ofloxacin ear drops): one patient also received a systemic steroid (betamethasone). Deafness persisted in one

The ototoxic effects of ethyl benzene in rats.

Exposure to organic solvents has been shown to be ototoxic in animals and there is evidence that these solvents can induce hearing loss in humans. In this study, the effects of inhalation of the possibly ototoxic solvent ethyl benzene on the cochlear function and morphology were evaluated using three complementary techniques: (1) reflex modification audiometry (RMA), (2) electrocochleography and (3) histological examination of the cochleas. Rats were exposed to either ethyl benzene (800 ppm, 8 h/day for 5 days) or to control conditions. The RMA threshold increased significantly by about 25 dB,

[Autoimmune deafness].

UNLABELLED: A REAL ENTITY: Deafness resulting from an autoimmune mechanism is suggested by a growing number of clinical and experimental arguments. The audio-vestibular system is known to be involved in a certain number of systemic diseases, particularly Cogan's disease. In other cases, the inner ear alone is involved; deafness may be the first manifestation of a systemic disease or result from a possible immunological mechanism. CLINICAL ASPECTS: Autoimmune deafness is very invalidating. Bilateral perception deafness is observed in 80% of the cases and vestibular involvement is found in 70%

Effect of compression ratio in a slow-acting compression hearing aid: paired-comparison judgments of quality.

Paired-comparison judgments of quality were obtained from 20 hearing-impaired listeners (half with a small dynamic range and half with a large dynamic range) for speech-in-noise (vent, apartment, and cafeteria) processed through a slow-acting compression hearing aid. Compression ratio was varied (1, 1.5, 2, 3, 5, and 10:1). Compression threshold, attack time, and release time were fixed. Sound quality judgments were significantly affected by compression ratio, noise, and dynamic range. Preference decreased with increasing compression ratio. The selection of compression ratio. The selection of

Contralateral suppression of transient evoked otoacoustic emissions: intra-individual variability in tinnitus and normal subjects.

Contralateral acoustic stimulation reduces the amplitude of the transient evoked otoacoustic emission (TEOAE) in humans. The mechanism is thought to be mediated, at least in part, through the medial olivocochlear efferent system innervating the outer hair cells. To assess its usefulness as a possible clinical test, TEOAE suppression was measured in each ear of 12 subjects over a 6-week period, and these data are shown in detail for four subjects representing extremes of variability in a tinnitus and a non-tinnitus group. Intra-subject test results (n = 18) exhibited a varying extent of suppres

Facial nerve function following middle fossa and translabyrinthine acoustic tumor surgery: a comparison.

Postoperative facial function was compared for middle cranial fossa and translabyrinthine removal of acoustic tumors. Included were 164 primary acoustic tumor surgeries performed between 1988 and 1991 in patients with normal preoperative facial function; postoperative House-Brackmann facial function grade available immediately postoperatively, at time of discharge and at 1 year or more postoperatively; and a tumor 1.5 cm or smaller. There were 116 translabyrinthine surgeries and 48 middle fossa surgeries. There was no significant difference in facial function results between the two surgical a

Coffin-Lowry syndrome with sensorineural deafness and labyrinthine anomaly.

A case of Coffin-Lowry syndrome is described. The child had the typical face and also soft and puffy hands with tapering fingers. His psychomotor development was retarded. Audiological tests revealed profound hearing impairment. Anomalous labyrinths were shown by CT scan of the temporal bones. It is inferred that deafness is one of the symptoms of this syndrome.

Yaw direction neurons in the cat inferior olive.

1. Single units that responded to yaw rotation were recorded extracellularly in the caudal inferior olive (IO) of barbiturate-anesthetized cats. Of 276 neurons, 55 responded reliably to yaw, and extensive quantitative data were recorded from 25. 2. No yaw-sensitive IO neuron responded to somatosensory or auditory stimuli but two responded, though unreliably, to flash. 3. Yaw-sensitive IO cells fired at low (1-4 spikes/s), irregular rates during one direction of rotation. Though cells responded reliably during yaw, firing rates varied considerably from cycle to cycle. Rotation speed and acceler

Idiopathic sudden progressive hearing loss and round window membrane rupture.

We investigated 24 cases of idiopathic sudden progressive deafness (ISPD) in which worsening of sensorineural hearing loss after initial examination was confirmed by audiometry. Exploratory tympanotomy was performed in 10 of 24 patients, and revealed perilymphatic leak in 4 patients. However, there was a higher incidence and a more severe degree of vertigo and imbalance in the group that was operated on than in the non-operated group. Our study has shown that there was no clear difference in the degree of hearing improvement between the operated group and the conservatively treated group. In b
