Profiling of cybersickness and balance disturbance induced by virtual ship motion immersion combined with galvanic vestibular stimulation.

Profile of cybersickness and balance disturbance induced by virtual ship motion alone and in combination with galvanic vestibular stimulation (GVS) remained unclear. Subjects were exposed to a ship deck vision scene under simulated Degree 5 or 3 sea condition using a head-mounted virtual reality display with or without GVS. Virtual ship motion at Degree 5 induced significant cybersickness with symptom profile: nausea syndrome > central (headache and dizziness) > peripheral (cold sweating) > increased salivation. During a single session of virtual ship motion exposure, GVS aggravated balance di

Medical Devices; Neurological Devices; Classification of the External Upper Limb Tremor Stimulator. Final order.

The Food and Drug Administration (FDA or we) is classifying the external upper limb tremor stimulator into class II (special controls). The special controls that apply to the device type are identified in this order and will be part of the codified language for the external upper limb tremor stimulator's classification. We are taking this action because we have determined that classifying the device into class II (special controls) will provide a reasonable assurance of safety and effectiveness of the device. We believe this action will also enhance patients' access to beneficial innovative de

The Efficacy and Safety of Dorsal Root Ganglion Stimulation as a Treatment for Neuropathic Pain: A Literature Review.

OBJECTIVE: Dorsal root ganglion stimulation (DRGS) received its first regulatory approval (CE marking in Europe) in late 2011, and so its use is now almost six years old. Several thousand patients have already been treated, and a landmark trial in lower limb complex regional pain syndrome (CRPS) and causalgia has recently been published. METHODS: In this review we have summarized the literature to date on the use of DRGS in the treatment of neuropathic pain. RESULTS: The results so far are encouraging, with reports of successful use in treating a wide range of indications including postsurgi

Noninvasive neuromodulation in cluster headache.

PURPOSE OF REVIEW: Neuromodulation is an alternative in the management of medically intractable cluster headache patients. Most of the techniques are invasive, but in the last 2 years, some studies using a noninvasive device have been presented. The objective of this article is to review the data using this approach. RECENT FINDINGS: Techniques as occipital nerve stimulation or sphenopalatine ganglion stimulation are recommended as first-line therapy in refractory cluster patients, but they are invasive and maybe associated with complications. Noninvasive vagal nerve stimulation with an exter

Anatomical stages of penile erection in the agouti (Dasyprocta leporina) induced by electro-ejaculation.

The reproductive system of the male agouti is not well documented. This study describes the specific anatomical features of the free part of the penis occurring during penile erection in the agouti. Electro-ejaculation was used to induce erection in three male agoutis that had previously produced offspring. Results proved that there were four stages in the erection process. Stage 1 involved protrusion of the penis from the preputial orifice. The lateral penile cartilages were then spread (stage 2). During stage 3, there was the blooming of the head of the glans penis (penile flower) and eversi

Neuromonitoring in thyroid surgery.

PURPOSE OF REVIEW: Laryngeal nerve injuries are among the most important complications of thyroidectomy. Recently, the use of neuromonitoring has been increasingly employed in order to predict and document nerve function at the end of thyroidectomy. RECENT FINDINGS: There is much controversy in recent studies concerning neuromonitoring in thyroid surgery. Some authors believe that the method does not bring any additional reduction in the paralysis rate of the laryngeal nerves. Other researchers defend its use only in selected cases and in very specific situations. Finally, some much respected

Improving valid and deficient body segment coordination to improve FES-assisted sit-to-stand in paraplegic subjects.

We investigated dynamic optimization as a tool to improve functional electrical stimulation (FES) assisted sit to stand transfers of paraplegic subjects. The objective would be to find optimal strategy for voluntary trunk movement, which would minimize hip, knee and ankle torques and demand minimal upper limb participation during the motion. Motion of the knee and the ankle were constrained by electrical stimulation. Motion capture (MOCAP) data, and signals from handle force sensors were acquired during FES-assisted rising motion of one paraplegic subject. Based on a 3 DOF dynamic model, we us

Clinical applications of neurostimulation: forty years later.

With the recent technological advances, neurostimulation has provided new hope for millions of patients with debilitating chronic pain conditions that respond poorly to other therapies. Outcome research demonstrated that patients with failed back surgery syndrome and complex regional pain syndromes benefit significantly from neurostimulation in pain reduction, functional capacity, and quality of life. Increasing clinical evidence supports the use of neurostimulation in post-herpetic neuralgia, peripheral neuropathy, occipital neuralgia, and other neuropathic pain conditions. Strong clinical ev

Vagus nerve stimulation for depression: rationale, anatomical and physiological basis of efficacy and future prospects.

Treatment-resistant depression (TRD) is a major public health concern due to its high costs to society. One of the novel approaches for the treatment of depression is the vagus nerve stimulation (VNS). Therapeutic brain stimulation through delivery of pulsed electrical impulses to the left cervical vagus nerve now has established safety and efficacy as an adjunct treatment for medication-resistant epilepsy and has recently been approved as an adjunct long-term treatment for chronic or recurrent depression. There is considerable evidence from both animal and human neurochemical and neuroimaging

Vagus nerve stimulation: indications and limitations.

Vagus nerve stimulation (VNS) is an established treatment for selected patients with medically refractory seizures. Recent studies suggest that VNS could be potentially useful in the treatment of resistant depressive disorder. Although a surgical procedure is required in order to implant the VNS device, the possibility of a long-term benefit largely free of severe side effects could give VNS a privileged place in the management of resistant depression. In addition, VNS appears to affect pain perception in depressed adults; a possible role of VNS in the treatment of severe refractory headache,

Vagal nerve stimulation: overview and implications for anesthesiologists.

Vagal nerve stimulation is an important adjunctive therapy for medically refractory epilepsy and major depression. Additionally, it may prove effective in treating obesity, Alzheimer's disease, and some neuropsychiatic disorders. As the number of approved indications increases, more patients are becoming eligible for surgical placement of a commercial vagal nerve stimulator (VNS). Initial VNS placement typically requires general anesthesia, and patients with previously implanted devices may present for other surgical procedures requiring anesthetic management. In this review, we will focus on

Sphincter repair: its present place.

Faecal incontinence has become a condition of interest only in the last 30 years. In 1970 at St Mark's Hospital, there were four admissions and over the next 20 years this had risen to 55 per year. Today faecal incontinence forms a significant part of colorectal practice. The original approach in the 1970s over a 20 year period was sphincter repair. Gynaecologists and general surgeons had for many years been occupied in the reconstruction of the anal sphincter after localised traumatic injury. In the 1970s, diffuse weakness of the pelvic floor was identified and characterised by neuromuscular

The current perception thresholds in normal pregnancy.

The purpose of this study was to evaluate a quantitative analysis of the nociceptive threshold, using the current perception threshold (CPT), in women with normal pregnancies and to assess the relationship between nociceptive thresholds and ovarian sex steroids. The subjects consisted of 10 women with singleton pregnancies and 14 age-matched healthy female volunteers. The CPTs (5,250, and 2,000 Hz) of the dominant ankle section were determined with a Neurometer CPT/C (Neurotron, Baltimore, MD). Blood samples were collected after these examinations, and the total 17beta-estradiol (E(2)) and pro

Deep brain stimulation of the Vim nucleus of the thalamus for the treatment of tremor.

The surgical treatment of tremor has evolved considerably in the past few years. Of the several conditions associated with severe tremor, the most common are Parkinson's disease (PD) and essential tremor (ET). Levodopa therapy reduced drastically the number of patients with PD who require surgery because of inadequate control. However, there remains a small number of "tremor dominant" PD patients for whom surgical treatment is often the best option. The ventralis intermediate nucleus (Vim) of the thalamus has been the preferred surgical target for the treatment of parkinsonian tremor for many

Ca2+ accumulation and cell damage in skeletal muscle during low frequency stimulation.

Electrical stimulation has been shown to produce a marked increase in Ca2+ influx and Ca2+ content in rat skeletal muscle. Long-term low-frequency stimulation (1 Hz, 240 min) increased 45Ca uptake by 30% and 154% in soleus and extensor digitorum longus muscles, respectively. Studies using Ca2+-fluorescent dyes have shown that intracellular concentrations of free Ca2+ are increased up to threefold during long-term low-frequency stimulation, suggesting that muscle cells have difficulties in handling the Ca2+ taken up during stimulation. Furthermore, long-term low-frequency stimulation induces le
