Closed-loop stimulation using intracardiac impedance as a sensor principle: correlation of right ventricular dP/dtmax and intracardiac impedance during dobutamine stress test.

Changes of the unipolar right ventricular impedance during the cardiac cycle are related to the changing content of blood (low impedance) and tissue (high impedance) around the tip of the pacing electrode. During myocardial contraction, the impedance continuously increases reaching its maximum in late systole. This impedance increase is thought to correlate with right ventricular contractility, and thus, with the inotropic state of the heart. In the new Inos2 DDDR pacemaker, integrated information from the changing ventricular impedance (VIMP) is used for closed-loop regulation of the rate res

Transvenous cardioversion of atrial fibrillation using low-energy shocks.

Recent reports have suggested that transvenous cardioversion of atrial fibrillation is feasible using low-energy shocks and a right atrium coronary sinus electrode configuration. We evaluated in a prospective study the efficacy and safety of low-energy internal cardioversion of atrial fibrillation in 104 consecutive patients. Sixty-two patients presented with chronic atrial fibrillation (group I), 16 had paroxysmal atrial fibrillation (group II), and 26 had an induced atrial fibrillation episode (group III). The mean duration of the presenting episode of atrial fibrillation was 9 +/- 19 months

Analysis of Morbidity and Outcomes Associated With Use of Subdural Grids vs Stereoelectroencephalography in Patients With Intractable Epilepsy.

IMPORTANCE: A major change has occurred in the evaluation of epilepsy with the availability of robotic stereoelectroencephalography (SEEG) for seizure localization. However, the comparative morbidity and outcomes of this minimally invasive procedure relative to traditional subdural electrode (SDE) implantation are unknown. OBJECTIVE: To perform a comparative analysis of the relative efficacy, procedural morbidity, and epilepsy outcomes consequent to SEEG and SDE in similar patient populations and performed by a single surgeon at 1 center. DESIGN, SETTING AND PARTICIPANTS: Overall, 239 patien

Pulsed field ablation: from concepts to technology.

AIMS: Pulsed field ablation (PFA) has emerged as a transformative approach for treating cardiac arrhythmias by exploiting irreversible electroporation to preferentially injure myocardium while limiting collateral damage. Translating this biophysical principle into safe and effective clinical systems requires careful integration of waveform design, catheter engineering, computational and experimental modelling, safety testing, and procedural workflow. OBJECTIVE: This review summarizes the fundamental physical and engineering principles that govern cardiac PFA and examines how these principles

Direct evidence of upper motor neuron excitability changes in a patient with ALS.

A key feature of amyotrophic lateral sclerosis (ALS) pathophysiology is motor neuron hyperexcitability. However, the mechanisms of hyperexcitability are not well understood. Prior studies have used transcranial magnetic stimulation (TMS) to demonstrate increased motor cortex excitability and reduced intracortical inhibition in human ALS. Yet, interpretation of these findings is limited because measurement of muscle responses cannot disentangle the specific contribution of upper and lower motor neurons and of cortical interneurons to excitability changes. We had the rare opportunity to record d

A motor thalamic site in humans that suppresses involuntary breathing without awareness.

Breathing is generated by brainstem respiratory networks but can be controlled and modulated by forebrain activity. The recent clinical adoption of thalamic electrode implantation during intracranial electroencephalography (iEEG) provides a rare opportunity to examine the role of the human thalamus in respiratory control. Here, we tested whether thalamic stimulation alters breathing in 11 patients undergoing iEEG for epilepsy monitoring. Across 412 stimulation trials at 108 thalamic sites, thalamic stimulation induced central apnea in every participant. Apnea occurred in isolation without sens

Insula Deep Brain Stimulation for Neuropathic Pain: A Cross-Over, Randomized, Sham-Controlled Trial.

OBJECTIVE: Despite neuropathic pain affecting a significant portion of the population, many people still experience refractory conditions. Here, we evaluated the efficacy and safety of posterior-superior insula deep brain stimulation for pharmaco-resistant neuropathic pain. MATERIALS AND METHODS: This was a phase 2 randomized, double-blind, sham-controlled, cross-over trial. Subjects with chronic pharmaco-resistant neuropathic pain, whose pain had previously responded to deep repetitive transcranial magnetic stimulation of the posterior-superior insula, underwent stereotactically guided impla

Ventricular Intramyocardial Navigation for Tachycardia Ablation Guided by Electrograms (VINTAGE): Initial Human Experience.

BACKGROUND: Catheter ablation for ventricular arrhythmias is limited by a high first-attempt failure rate approaching 50% to 60%. Failure appears driven by physical inaccessibility of mapping and ablating deep intramyocardial substrate. VINTAGE (Ventricular Intramyocardial Navigation for Tachycardia Ablation Guided by Electrograms) is a novel procedure to steer guidewires within the walls of the beating left ventricle (LV) from a right ventricular entry point and deliver a deep intramural ablation electrode anywhere within the LV. OBJECTIVES: The objective of this work is to assess the provis

Sacral nerve electrical stimulation (SNES) ameliorates bladder dysfunction and detrusor fibrosis in early-stage spinal cord injury: A randomized controlled experimental study in rats.

This study evaluated the therapeutic effects of sacral nerve electrical stimulation (SNES) targeting the L6-S1 spinal nerves on neurogenic bladder dysfunction following spinal cord injury (SCI). Using a randomized controlled design, 80 female Sprague-Dawley rats were divided into four groups: sham surgery, complete T9 spinal cord transection (SCI model), SCI with electrode implantation only (control), and SCI with active SNES (3-5 Hz, 3-5 V daily for 4 weeks). Bladder function was assessed through urodynamics, while detrusor fibrosis was examined using histological and ultrastructural analyses

Subthalamic nucleus neurons encode syllable sequence and phonetic characteristics during speech.

Speech is a complex behavior that can be used to study unique contributions of the basal ganglia to motor control in the human brain. Computational models suggest that the basal ganglia encode either the phonetic content or the sequence of speech elements. To explore this question, we investigated the relationship between phoneme and sequence features of a spoken syllable triplet and the firing rate of subthalamic nucleus (STN) neurons recorded during the implantation of deep brain stimulation (DBS) electrodes in individuals with Parkinson's disease. Patients repeated aloud a random sequence o

Are tinnitus burden and tinnitus exacerbation after cochlear implantation influenced by insertion technique, array dislocation, and intracochlear trauma?

INTRODUCTION: Although numerous studies suggest that cochlear implantation (CI) generally alleviates the overall burden of tinnitus, certain patients experience tinnitus exacerbation following CI. The exact cause of this exacerbation is still uncertain. This prospective study aimed to investigate whether cochlear trauma, resulting from scalar dislocation of the electrode array, affected postoperative tinnitus intensity, tinnitus burden, and speech perception. Additionally, the influence of CI insertion technique, insertion depth, insertion angle, and cochlear morphology on postoperative tinnit

High frequency alternating current neurostimulation decreases nocifensive behavior in a disc herniation model of lumbar radiculopathy.

BACKGROUND: The purpose of this study was to evaluate if kilohertz frequency alternating current (KHFAC) stimulation of peripheral nerve could serve as a treatment for lumbar radiculopathy. Prior work shows that KHFAC stimulation can treat sciatica resulting from chronic sciatic nerve constriction. Here, we evaluate if KHFAC stimulation is also beneficial in a more physiologic model of low back pain which mimics nucleus pulposus (NP) impingement of a lumbar dorsal root ganglion (DRG). METHODS: To mimic a lumbar radiculopathy, autologous tail NP was harvested and placed upon the right L5 nerve

Normative intracranial EEG maps epileptogenic tissues in focal epilepsy.

Planning surgery for patients with medically refractory epilepsy often requires recording seizures using intracranial EEG. Quantitative measures derived from interictal intracranial EEG yield potentially appealing biomarkers to guide these surgical procedures; however, their utility is limited by the sparsity of electrode implantation as well as the normal confounds of spatiotemporally varying neural activity and connectivity. We propose that comparing intracranial EEG recordings to a normative atlas of intracranial EEG activity and connectivity can reliably map abnormal regions, identify targ

Low-frequency stimulation of a fiber tract in bilateral temporal lobe epilepsy.

OBJECTIVE: Pharmacoresistant bilateral mesial temporal lobe epilepsy often implies poor resective surgical candidacy. Low-frequency stimulation of a fiber tract connected to bilateral hippocampi, the fornicodorsocommissural tract, has been shown to be safe and efficacious in reducing seizures in a previous short-term study. Here, we report a single-blinded, within-subject control, long-term deep-brain stimulation trial of low-frequency stimulation of the fornicodorsocommissural tract in bilateral mesial temporal lobe epilepsy. Outcomes of interest included safety with respect to verbal memory

Effects of anode position on pedicle screw testing during lumbosacral spinal fusion surgery.

OF BACKGROUND DATA: Pedicle screws are commonly placed with lumbar/lumbosacral fusions. Triggered electromyography (tEMG), which employs the application of electrical current between the screw and a complementary anode to determine thresholds of conduction, may be utilized to confirm the safe placement of such implants. While previous research has established clinical thresholds associated with safe screw placement, there is variability in clinical practice of anode placement which could lead to unreliable measurements. PURPOSE: To determine the variance in pedicle screw stimulation threshold

High-resolution parahisian mapping and ablation using microelectrode embedded ablation catheters.

BACKGROUND: Accurate mapping of the compact atrioventricular (AV) node is critical during ablation of a range of arrhythmias. OBJECTIVE: The purpose of this multicenter prospective study was to test the hypothesis that microelectrode (ME)-embedded catheters more accurately define the near-field compact AV node compared to conventional catheters. METHODS: For the mapping phase, detailed AV junction maps were created in 47 patients using an ME-embedded catheter. His electrograms (EGMs) detected by conventional electrodes (Hisc) and by ME (Hisμ) were annotated. For the ablation phase, AV nodal

Thalamic Local Field Potentials Are Related to Long-Term DBS Effects in Tourette Syndrome.

Background: Local field potential (LFP) recordings helped to clarify the pathophysiology of Tourette syndrome (TS) and to define new strategies for deep brain stimulation (DBS) treatment for refractory TS, based on the delivery of stimulation in accordance with changes in the electrical activity of the DBS target area. However, there is little evidence on the relationship between LFP pattern and DBS outcomes in TS. Objective: To investigate the relationship between LFP oscillations and DBS effects on tics and on obsessive compulsive behavior (OCB) comorbidities. Methods: We retrospectively ana

Long-Term Deep Brain Stimulation in Treatment-Resistant Obsessive-Compulsive Disorder: Outcome and Quality of Life at Four to Eight Years Follow-Up.

BACKGROUND: Obsessive compulsive disorder (OCD) is a severe disabling disease, and around 10% of patients are considered to be treatment-resistant (tr) in spite of guideline-based therapy. Deep brain stimulation (DBS) has been proposed as a promising treatment for patients with trOCD. However, the optimal site for stimulation is still a matter of debate, and clinical long-term follow-up observations including data on quality of life are sparse. We here present six trOCD patients who underwent DBS with electrodes placed in the bed nucleus of the stria terminalis/anterior limb of the internal ca
