Immune-related adverse events (irAE) during the administration of immune-checkpoint inhibitors (ICIs) become more evident due to the increased use of these therapies. To remind the importance of early recognition of this phenomenon, we report a paradigmatic case characterized by severe systemic inflammatory myopathy and severe cardiac involvement that abruptly precipitated in an untoward ending after one single dose of Pembrolizumab.

Jim Umbricht (1930-1964) was a Major League baseball pitcher for the Pittsburgh Pirates and Houston Astros (formerly Houston Colt .45s). In 1963, he was diagnosed with stage III malignant melanoma which presented with a right leg pigmented skin lesion and right groin lymphadenopathy. Umbricht was treated at MD Anderson Cancer Center and Tumor Institute in Houston with the relatively new therapeutic modality of isolated limb perfusion. Multiple opioids are known to trigger mast cell degranulation. We report the case of a neonate with blistering skin lesions at birth who died of multi-organ failure after administration of morphine. Given the excessive histamine release and potential complications associated with morphine administration, alternative opioids and adjuvants should be considered in infants presenting with evidence of bullous or infiltrative skin lesions until mastocytosis is ruled out.
