The Cardiovascular Health Study is an observational cohort study of risk factors for cardiovascular disease in 5201 participants, ages > or = 65 years. We report the methods and quality-assurance results for blood procurement, processing, shipping, storage, and sample analysis used during the first examination period (May 1989-June 1990). The most frequent difficulty in phlebotomy and processing was the requirement of more than one venipuncture (in 2.6% of the participants). A patient with systemic scleroderma is described who developed a cutaneous eruption of papules and vesicobullae. Over time, the latter turned into papules. The histopathological and immunofluorescence features of the papular lesions were unequivocally those of lichen planus, while those of the bullous lesions reminded of erythema multiforme. The case of a 32-year old woman with 36-week pregnancy presented at hospital because of spontaneous vaginal bleeding, anemia and mild hypotension is reported. Fetal mors in utero, abruptio placentae and diffuse intravascular coagulation were diagnosed. The patient subsequently underwent cesarean section and large retroplacental hematoma was removed and obviously fetum. The DIC was easily controlled by means of recently introduced method of determination of fibrin D-dimer.

In the period from 1980 till 1983 fibrin tissue glue was used in 30 rhinosurgical operations. The formation of haematomas and effusions was prevented. Two examples are presented. No haematoma was observed. The post operative recovery was improved.

Gluing of oesophageal anastomoses in atresia effected by means of three or four sutures will definitely produce better results than suturing alone. The results of endoscopic closure of tracheo-oesophageal fistulas by gluing with fibrin are excellent. Furthermore, it is possible with fibrin gluing alone to treat iatrogenic perforations of the oesophagus in the newborn, following a false passage of nasogastric tubes from the hypopharynx into the mediastinum or pleural cavity.

Under analysis was the course of the postoperative period in 216 patients subjected to closed mitral commissurotomy. The aim of the present study was to investigate the effect on blood coagulation and fibrinolysis of a natural oestrogen preparation, piperazine oestrone sulphate, prospectively in menopausal women. A case of a chronic remittant blistering eruption on the left temple in a 62-year-old woman is described. Only after repeated biopsies did direct immunofluorescence (DIF) studies reveal a linear staining of the epidermal basement membrane with anti-IgG, C1q, C4, C3 and C5. In addition, heavy deposits of fibrin were observed. The importance of repeated DIF examination in initially negative cases has been stressed. Localized chronic pemphigoid should be considered as a distinct subform of the pemphigoid syndrome.

This article reviews the mediation systems participating or potentially participating in inflammatory disease, especially in immunologic injury of the glomerulus.
