Clinical and resource burden of acute kidney injury among adults hospitalized with sepsis: a retrospective cross-sectional study.

BACKGROUND: Acute kidney injury (AKI) is a frequent complication of sepsis and is associated with worse clinical outcomes. However, contemporary national estimates of its broader clinical and health-system burden, including discharge disposition, remain limited. We used a large nationally representative database to characterize the epidemiology and associated outcomes of AKI among adults hospitalized with sepsis in the United States. METHODS: We conducted a cross-sectional study of adult sepsis hospitalizations in the 2022 National Inpatient Sample (NIS). AKI was identified using ICD-10-CM cod

Enhancing the Prospective Acceptability of a Paediatric Speech and Language Therapy Intervention With Parents From Under-Represented Communities.

BACKGROUND: Integrating the perspectives of people with lived experience is integral to developing new healthcare interventions. However, individuals from under-represented communities may face additional barriers to taking part in such patient and public involvement (PPI) activities. Within paediatric speech and language therapy (SLT), under-represented communities might include families from lower SES backgrounds and/or families where the majority language for that country is not spoken. Excluding families from under-represented communities poses the risk of new interventions not being accep

An implementation study of text invitation, mailed at-home human papillomavirus (HPV) self-testing and telehealth management in Aotearoa New Zealand, with a nested randomised controlled trial that compared offering an incentive vs. no offer with a repeat test kit.

INTRODUCTION: In Aotearoa New Zealand, human papillomavirus (HPV) self-testing was introduced simultaneously with HPV primary screening in September 2023 to improve access and reduce inequities for priority populations, including Indigenous Māori, Pacific and under-screened people. To contribute policy-relevant information, we implemented non-standard engagement and screening strategies, including text message invitation, mailed test kits, at-home self-testing, telehealth support and follow-up by a central nurse-led co-ordination team. METHODS: We partnered with an Auckland primary health org

Informal Caregivers Connecting on the Web: Content Analysis of Posts on Discussion Forums.

BACKGROUND: About 53 million adults in the United States offer informal care to family and friends with disease or disability. Such care has an estimated economic value of US $600 million. Most informal caregivers are not paid nor trained in caregiving, with many experiencing higher-than-average levels of stress and depression and lower levels of physical health. Some informal caregivers participate in web-based forums related to their caregiving role. OBJECTIVE: This study aimed to explore how informal caregivers use easy-to-access caregiving web-based forums, including the types of informat

App-Controlled Treatment Monitoring and Support for Patients With Head and Neck Cancer Undergoing Radiotherapy: Results From a Prospective Randomized Controlled Trial.

BACKGROUND: Head and neck cancers (HNCs) are very common malignancies, and treatment often requires multimodal approaches, including radiotherapy and chemotherapy. Patients with HNC often display a high symptom burden, both due to the disease itself and the adverse effects of the multimodal therapy. Close telemonitoring of symptoms and quality of life during the course of treatment may help to identify those patients requiring early medical support. OBJECTIVE: The App-Controlled Treatment Monitoring and Support for Patients With Head and Neck Cancer (APCOT) trial aimed to investigate the feas

Laboratory Abnormalities in Acne Patients Treated With Oral Isotretinoin: A Retrospective Epidemiological Study.

Background Isotretinoin has been used to treat moderate to severe acne. It is well known that isotretinoin can cause an elevation in liver enzymes, triglycerides, and cholesterol. Laboratory monitoring is indicated while patients are on isotretinoin, but the frequency of laboratory monitoring is very variable among physicians who prescribe it. Study objectives This study aimed to determine the frequency of laboratory abnormalities of triglycerides, cholesterol, and liver aminotransferases in acne patients treated with oral isotretinoin in order to assess the need for frequent laboratory monito

A Mobile Health Intervention for Fetal Alcohol Spectrum Disorders (Families Moving Forward Connect): Development and Qualitative Evaluation of Design and Functionalities.

BACKGROUND: Fetal alcohol spectrum disorders (FASD) affect approximately 2% to 5% of the US population. However, most families are unable to access FASD-informed interventions. Barriers to care include the lack of a knowledgeable and skilled workforce and family-level barriers such as limited financial resources, inability to access childcare, and stigma. As a result, families often try peer-to-peer and self-help support strategies. However, they often take these strategies from disparate sources, which have quite variable intervention quality and empirical support. OBJECTIVE: This study aime

[Study of the impact of an assertive community program on the families of patients with severe mental disorders].

OBJECTIVES: Assertive Community Treatment (ACT) is known to have a positive impact on the number and length of inpatient stays. Yet, research is needed in order to help understand how ACT programs may ease off families' burden, e.g. in terms of economic expenditures. Indeed, many families with siblings suffering from chronic mental illness, who disengaged from psychiatric services, report needs related to ACT. This paper aims to describe the impact of a new ACT program in Geneva on patients and their families' burden. METHODS: Out of 91 patients consecutively treated by the ACT program for at

Cancer CARE (Connected Access and Remote Expertise) Beyond Walls - Safety, Feasibility, and Patient Experience of Home-Based Chemotherapy.

As patients live longer with cancer, traditional treatment models centered around hospitals and infusion centers increasingly burden patients, caregivers, and health care systems alike. These burdens are logistical, financial, and emotional, and they are especially profound for older adults and individuals in rural or underserved areas, who face added challenges in accessing centralized care. To address these growing inequities and systemic pressures, Mayo Clinic piloted an innovative model of home-based chemotherapy through its Cancer CARE (Connected Access and Remote Expertise) Beyond Walls

Promoting and inhibiting factors for the implementation of evidence-based non-invasive treatment programs for patients with knee- and/or hip-osteoarthritis: a rapid review.

BACKGROUND: Osteoarthritis of the knee and/or hip joint are common conditions among older adults. Although multimodal therapy combining patient education, exercises and weight loss has proven effective, only a few countries offer implemented treatment programs. While some Scandinavian countries have achieved nationwide implementation, similar initiatives remain limited to research settings elsewhere. To date, limited evidence exists on factors that facilitate or hinder the successful implementation of these programs. This study therefore aims to synthesize scientific evidence on the implementa

Speeding up respiratory diagnosis: clinical drivers, barriers, and system readiness for rapid molecular point-of-care testing in Saudi Arabia.

BACKGROUND: Acute respiratory tract infections place substantial strain on healthcare systems, particularly during seasonal respiratory surges and mass-gathering events. Delayed confirmation of respiratory pathogens contributes to diagnostic uncertainty, unnecessary antimicrobial use, delayed infection prevention decisions, and inefficient patient flow. Rapid molecular point-of-care testing (mPOCT) offers near-PCR-level diagnostic accuracy with turnaround times compatible with real-time clinical decision-making; however, implementation and integration into routine clinical pathways remain vari

Mapping the Health Management Journey of Patients With Urostomy: A Qualitative Study.

BACKGROUND: The treatment of bladder cancer often involves radical cystectomy and urostomy, which increase survival rates but cause significant physical, psychological and social distress. While existing evidence emphasizes treatment and interventions to improve the quality of life, few studies have explored the continuous health management experience and needs of urostomy patients from diagnosis through recovery. OBJECTIVE: To construct a journey map of the healthcare management needs of urostomy patients to explore key pain points in their rehabilitation process and provide a reference for
