Neck and shoulder pain: differentiating cervical spine pathology from shoulder pathology.

Problems of the cervical spine and shoulder frequently have very similar presentations and can be difficult to differentiate. However, with a careful history, physical exam, imaging studies, and judicious use of diagnostic injections, the true source of a patient's symptoms can be deciphered and treated. Cervical spondylosis not only causes pain in the neck and shoulder area, but can also cause radiating pain in the arm and forearm that can be confused with rotator cuff pathology, nerve compression in the shoulder area, or brachial neuritis.

Phaeohyphomycosis of subcutaneous tissue caused by Phaeoacremonium parasiticum.

Though Phaeoacremonium parasiticum is an unusual cause of human disease, subcutaneous infection, eumycetoma, osteomyelitis, arthritis and even disseminated diseases, such as fungemia and endocarditis have been reported. Here, we report a case of subcutaneous abscess on the forearm due to P. parasiticum in a 26-year-old woman. There were no obvious predisposing factors. The patient was treated with surgical debridement followed by intravenous amphotericin B and itraconazole to which she responded well. We report this case, being rare and the first from India.

Long-segment tracheal stenosis treated with vascularized mucosa and short-term stenting.

A free, vascularized radial forearm fascia flap lined with two full-thickness buccal mucosa grafts can be used in cases of acquired, long-segment tracheal stenosis. To compensate for the absence of supportive tissue, the reconstructive tissue has to be supported by a stent for a period of 4 weeks. A case with an airway stenosis with a length greater than 5 cm is presented. The vascularized mucosa allowed for primary healing of the augmented airway. The tracheostomy could be closed shortly after stent removal.

["Tingling feet," forgetfulness, and progressive personality changes in a 63-year-old patient].

A 63-year-old female was admitted to the hospital with leg and forearm paresthesias. We found progressive ataxia, dementia, and psychosocial deterioration. The clinical symptoms, the neurologic and psychiatric abnormalities together with the inflammatory cerebrospinal fluid alteration and the cerebral magnetic resonance imaging changes suggested a paraneoplastic etiology. It was confirmed by paraneoplastic antineuronal antibodies in the patient's serum and the histological diagnosis of a small cell bronchial carcinoma. The prognosis of patients with paraneoplastic symptoms is the better the ea…

Dermatomyositis with tuberculous fasciitis.

A case of dermatomyositis with tuberculous fasciitis is described. A 69-year-old Japanese man treated with prednisolone for dermatomyositis developed erythema and bilateral swelling of arms and forearms. A diagnosis of tuberculous fasciitis was confirmed by the presence of acid-fast bacilli in a biopsy specimen and the growth of Mycobacterium tuberculosis from tissue cultures. The unusual presenting clinical features resulted in delayed diagnosis. We emphasize that tuberculous fasciitis should be considered in an immunocompromised patient who is treated with steroids, especially if the results…

Case report. Primary cutaneous histoplasmosis in an immunosuppressed patient.

A case of cutaneous histoplasmosis is reported in an 80-year-old man with rheumatoid arthritis who had been treated with steroid therapy for 15 years. The patient developed a large ulcerative lesion on the back of the left hand and on the distal third of the left dorsal forearm after a slight trauma. Diagnosis was based on histological and mycological examination. Systemic involvement was not found. The lesions healed after 2 months of therapy with 100 mg day-1 fluconazole, confirmed at follow-up 1 year later.

Ipsilateral fractures of the proximal ulna and distal radius (Colles type).

A 43-year-old female fell backward outside her home while carrying a watering can. She sustained an ipsilateral fracture of the proximal ulna and distal radius (Colles fracture). The ulnar fracture was plated, and the Colles fracture was managed by closed reduction and percutaneous Kirschner wiring. The patient recovered without incident. This association of forearm fractures is unusual. Both fractures can be caused by a fall on the hand with the wrist in mid-extension and the elbow flexed, and they probably occurred in quick succession.

Preradial myxedema: a case report.

We describe a 51-year-old Chinese woman with a 2-year history of swelling of the right forearm and hand, which gradually became hyperpigmented and indurated. This had the typical features of myxedema. The diagnosis was confirmed on histology. She had a past history of thyrotoxicosis and rheumatoid arthritis for which she had undergone treatment. This case is reported as an unusual location of localised myxedema with a literature review and comment on the treatment options. In Singapore only rare similar cases have been reported in the past.

Aeromonas hydrophila myonecrosis accompanying mucormycosis five years after bone marrow transplantation.

A 36-year-old man, five years after bone marrow transplantation for aplastic anemia, was admitted with myonecrosis of the forearm after he had immersed his hand in sewage water several days prior to his admission. Blood cultures and specimens taken from the necrotic tissue of the arm all grew Aeromonas hydrophila. Following extension of the infection, the patient underwent amputation of the arm but ultimately died of cerebral mucormycosis. The epidemiology of Aeromonas infections is discussed and the literature of Aeromonas myonecrosis is reviewed.

Trigger wrist induced by finger movement. Pathogenesis and differential diagnosis.

Four cases of trigger wrist induced by finger motion are reported. The cause of trigger wrist induced by finger motion was a rheumatoid nodule in one case, giant cell tumor of the flexor tendon sheath in one case, partial laceration of the flexor digitorium superficialis tendon in one case, and lipofibroma in one case. Triggering or snapping at the wrist is also induced by motion of the wrist and forearm. Reported cases were also analyzed and the clinical entity of the true trigger wrist and differential diagnosis were discussed.

Tinea corporis due to Trichophyton verrucosum: report of a patient from the San'in District.

A 70-year-old Japanese housewife, a resident of Shimane Prefecture, developed an erythematous, annular skin lesion on her right forearm. Mycological examinations revealed that it was tinea corporis caused by Trichophyton verrucosum (T. verrucosum). This fungus was then shown to have been transmitted to her from young dairy cattle introduced from the Hokkaido District to the ranch of her son. To the best of our knowledge, she is the first reported patient with tinea due to T. verrucosum who was infected in the San'in District.

[The ultrastructural changes in the clinically unaffected skin of patients with pemphigus vulgaris].

Analysis of the results of light optic, immunofluorescent, and electron microscopic studies of biopsy specimens of intact skin of forearms and of the skin adjacent to lesions in 10 patients with untreated pemphigus vulgaris has lead the authors to a conclusion on the uniform pattern of ultrastructural changes in all sites of the epidermis in pemphigus patients. A combination of the signs of involvement of prickle and basal-cell structures (desmosomes and mitochondria) and of signs of cellular metabolic activation (euchromatism, active status of the nucleolus) anticipate the development of intr…

Fractures and dislocations of the distal radioulnar joint.

Fractures and dislocations of the distal radioulnar joint are frequently visualized as a secondary problem in comparison to the more apparent radius fractures. Frequently, in the long-term follow-up of patients with radius fractures, ulnar wrist pain secondary to distal radioulnar joint incongruity is the final outcome. Therefore, in the evaluation of the injured forearm the distal radioulnar joint must be assessed clinically and radiographically. In this assessment if distal radioulnar joint instability or incongruity is present then joint stabilization or reduction, respectively, must be att…

Symptomatic, lateral synovial fringe (plica) of the elbow joint.

A series of patients with elbow symptoms suggesting intraarticular loose body(s) is presented. The symptoms included aching, stiffness, intermittent loss of motion, and catching. The clinical picture was seen in individuals without significant antecedent trauma. Arthroscopy in each case failed to demonstrate the anticipated loose body but did demonstrate a fibrotic synovial fringe that impinged between the radial head and the capitellum on repetitious elbow flexion and extension, particularly with the forearm in pronation. Arthroscopic excision of the fringe led to prompt recovery.

Open reduction and internal fixation of radial head fractures associated with olecranon fracture or dislocation.

Six fracture dislocations or dislocations of the olecranon associated with radial head fractures were treated with open reduction and internal fixation of the radial head. The olecranon was reduced and if fractured (three cases) was internally fixed. Postoperatively, early active range of motion was achieved in all cases. Twelve to 48 months after surgery, there was an average loss of 18 degrees of elbow extension and 56 degrees of forearm rotation. An average of 132 degrees of flexion was maintained. There was no elbow instability or wrist pain.

Hypertrichosis lanuginosa acquisita. Report of a case and review of the literature.

Hypertrichosis lanuginosa acquisita was observed in a female patient with small-cell follicular lymphoma stage IVB. The patient had additional complaints of a burning sensation on the tip of the tongue and the sudden appearance of several seborrheic keratoses on hands and forearms. The findings are well in agreement with those reported in the literature. It is concluded that the sudden appearance of down in an area previously perceived as hairless by the patient is highly indicative of widely disseminated internal malignancy and that this phenomenon can be seen in a wide range of cancers. Poss…

Transient idiopathic periosteal reaction associated with dysproteinemia.

Periosteal reaction with new bone formation in a child is a radiographic finding with an extensive differential diagnosis. The present case is that of a child who presented with a clinical syndrome of fever, bone pain in the forearm and leg, bony tenderness in these areas, radiographic evidence of a periosteal reaction in both tibiae and ulnae, and an abnormality of serum proteins. It was a self-limited disease process requiring no specific treatment, with eventual return to normal of both the radiographic and serum protein abnormalities. Its relationship to Caffey's disease remains to be defi…

[Muscle abnormalities on the back of the hand and their differentiation from soft-tissue tumors].

Anomalous muscles at the hand and forearm are described in several studies. They may be confused with ganglia or with other soft-tissue tumours. Two rare cases of atavistic muscles in the area of the back of the hand are described which caused intense pain of the wrist during some months. During the operation an enlongated muscle directly below the extensor tendon, was found which was covered by the extensor retinaculum. It is an extensor digiti brevis manus, which exists in amphibia and is replaced in man by the long tendonous part of the extensor digitorum.

[Carpal tunnel syndrome. Cosmetic incision (author's transl)].

The median nerve compressed in the carpal tunnel can be released by an incision in the "line of life", a natural fold along the axis of the third metacarpal bone. By sectioning the carpal tunnel in the palm of the hand, on the cubital side of the median nerve, division of the external branch and of the sensory cutaneous rami of the nerve can be avoided. Hypertrophic scars are prevented. The part of the carpal tunnel situated in the forearm can be freed by a subcutaneous incision if needed, but compression at that level is much less frequent than in the palm of the hand.

Diagnosis and operative treatment for the brachial plexus injury.

It is shown that the electrophysiological diagnosis of brachial plexus injury is becoming more precise than ever. The intraoperative use of electrodiagnosis especially aids the obtaining of better results for brachial plexus injuries. When nerve transfer is undertaken, biceps function is more usefully restored by the accessory nerve than by the intercostal nerves. Protective sensation was achieved in almost all cases by intercostal nerve transfer. When nerve grafting was undertaken, functional motor restoration was regained in the upper arm muscles in one case, but not in the forearm.
