[Treatment Strategy for Infectious Pulmonary Artery Pseudoaneurysm Secondary to Lung Abscess in Which Bleeding Is Controlled by Transcatheter Embolization Followed by Surgery:Report of a Case].

We report a case of hemoptysis due to pulmonary artery pseudoaneurysms controlled by transcatheter arterial embolization (TAE) and surgery. An 83-year-old man presented with hemoptysis one month after treatment for pneumonia and referred to our hospital. Chest computed tomography( CT)revealed a lung abscess in the left lower lobe and two pulmonary artery pseudoaneurysms by the abscess. TAE was performed to control hemoptysis, and the hemoptysis improved. After his general condition improved, a left lower lobectomy was safely performed.

Remimazolam Use in Cardiac Anesthesia: A Narrative Review.

Remimazolam, a novel ultra-short-acting intravenous benzodiazepine, has garnered recent attention for its use as a general anesthetic. This narrative review aims to summarize and analyze the available literature on the effects of remimazolam use in cardiac surgical patients, including its effects on hemodynamics, safety in patients with baseline myocardial dysfunction, and impact on postoperative management including time to emergence and extubation. Finally, there is discussion regarding potential drawbacks of adopting remimazolam as a routine anesthetic for cardiac surgery.

Is splenectomy an option for multiple relapses in a child with visceral leishmaniasis? A case report.

Visceral leishmaniasis (VL) is an infectious disease caused by Leishmania spp. The recurrence of the disease occurs, in general, in patients with decreased or loss of T-cell function, whether due to the use of corticosteroids, immunosuppressive disease, or another cause. In some cases, splenectomy may be a therapeutic option. However, the effectiveness of splenectomy is not well defined. This report describes the evolution of a pediatric patient with seven recurrences of VL, who relapsed post-surgery after drug therapy and splenectomy.

Anaesthetic Challenges and Transesophageal Echocardiography-Guided Perioperative Management in a Patient with Uncorrected Adult Congenital Heart Disease Presenting for Emergency Laparoscopic Hysterectomy.

Anaesthetic management of a patient with adult congenital heart disease with a single ventricle physiology presenting for an emergency laparoscopic surgery is challenging. The importance of a multidisciplinary approach, astute understanding of the pathophysiology and optimisation of intraoperative hemodynamic goals cannot be overemphasised. The present report describes the anaesthetic challenges and the role of transoesophageal echocardiography in perioperative management of a patient with uncorrected tetralogy of Fallot with pulmonary atresia, who successfully underwent an emergency laparosco

Value of Surgery and Nonsurgical Approaches for Cervical Spondylotic Myelopathy: WFNS Spine Committee Recommendations.

Cervical spondylotic myelopathy (CSM) is a common cause of adult spinal cord dysfunction. Although the therapeutic options for moderate to severe CSM patients have been established well, the existing guidelines for therapeutic decisions in mild cases of CSM are unclear. We present a review of literature on conservative treatment and surgery for CSM and suggest general recommendations applicable in various clinical presentations and in different geographic locations across the globe, with due considerations to available resources and locally prevalent practices.

Successful Percutaneous Coronary Intervention for Chronic Total Occlusion of Left Anterior Descending Coronary Artery in a Patient with Dextrocardia.

Dextrocardia is a rare cardiac anomaly in which the heart is located in the right chest along with the cardiac long axis directing to the right and inferiorly. Although, it is a rare clinical phenomenon, coronary artery disease with dextrocardia is presumed to be of similar frequency as in the general population. Percutaneous coronary intervention has some technical difficulties in these patients. We hereby report a male patient with dextrocardia who was previously treated with coronary artery bypass graft surgery.

[Problems in complex hindfoot corrections].

This article discusses the intraoperative and postoperative problems seen in the treatment of posterior tibial tendon dysfunction. Problems associated with tendon transposition procedures, osteotomy and arthrodesis are discussed. The preoperative, intraoperative and postoperative problems and complications and how to avoid or treat them will be addressed. The individual procedures are often part of other complex hindfoot reconstructions. For this reason the general and special aspects of treating posterior tibial dysfunction can often be transferred to avoiding and resolving problems in hindfo

Adult congenital heart disease.

For a decade now, it has been recognized that optimal management of adult congenital heart disease (ACHD) requires a skilled multidisciplinary team. The size and complexity of the population of adults with congenital heart disease (CHD) are increasing. This article reviews the general considerations for giving an anesthetic to an adult with CHD for cardiac or noncardiac surgery and provides further elaboration for a variety of complex patient types. Lastly, the advantages of an organized multidisciplinary approach to patients with ACHD are discussed.

Chronic osteomyelitis following an uncomplicated dental extraction.

The general dentist is usually the first health care practitioner to evaluate oral disease, initiate treatment and manage complications. Although rare, devastating outcomes of dental surgery can include Ludwig angina, mediastinitis, hemorrhage, necrotizing fasciitis, Lemierre syndrome and osteomyelitis. Osteomyelitis is a well known, but rare complication of dental extractions that can mimic multiple benign and malignant processes. In this case report, we review the diagnosis and management of an advanced postoperative mandibular osteomyelitis that developed following the removal of a mandibul

Benzocaine-induced methemoglobinemia.

A case is reported in which a patient developed methemoglobinemia-induced cyanosis while under general anesthesia during surgery for multiple fascial space infections. The cause of methemoglobinemia was 20% benzocaine spray used for local anesthesia before intubation. Acutely developing methemoglobinemia is infrequently encountered in clinical practice. When confronted with cyanosis in the absence of cardiac or pulmonary disease, one must seriously consider the diagnosis of methemoglobinemia. The etiology of methemoglobinemia, the causative agents, the diagnosis, and the emergency treatment re

[Antithrombotic therapy and dental surgery with bleeding].

The advantages of stopping or reducing antithrombotic therapy should always be weigh out against the disadvantage of the enlarged risk of thrombo-embolic complications. A review of the available literature showed that, in general, it is not necessary to stop the use of aspirin or clopidogrel in case of normal dental surgery. If in doubt and in all cases of other antithrombotic therapy the medical specialist should be consulted. The treatment of postoperative hemorrhage is described and the medical legal aspects are discussed.

Interventional radiology in the gynaecological oncology patient.

The use of radiologically guided percutaneous procedures has greatly reduced the morbidity and mortality of gynaecological oncology patients. Certain procedures, previously requiring major surgery, can now be performed with little patient discomfort, with conscious sedation instead of general anaesthesia, and frequently on an outpatient basis. In this chapter, diagnostic and therapeutic arteriography and venography will be discussed as well as radiologically guided genitourinary interventions. This represents an overview of what is available to the gynaecologist from the interventional radiolo

[Acute infectious endocarditis].

Acute infective endocarditis are due to highly virulent microorganisms, such as Staphylococcus aureus, able to invade normal endocardium, causing fulminant local damages (valvular destruction) as well as general damages (sepsis, metastasis), so that clinical manifestations are usually dramatic. Empirical antimicrobial therapy must be given immediately, using combinations active against the most commonly encountered pathogens. Other treatments are frequently necessary: medical therapy for heart failure, surgery of valvular reconstruction or replacement, respiratory assistance and other supporti

Brachial plexus injury during surgery--report of two cases.

Brachial plexus is the most commonly injured peripheral nerve by malposition during operation. We present two cases of transient brachial palsy after surgery under general anesthesia. Symptoms of the first case persisted about 60 min. Electromyography (EMG) and nerve conduction velocity (NCV) revealed no abnormal finding three days later. In the second case, axonal neuropathy was found at left axillary and suprascapular nerves by EMG and NCV three weeks later. Symptoms persisted for three months and had complete remission after conservative treatment.

First clinical experiences with an implantable bone conduction hearing aid at the University of Amsterdam.

A transcutaneous bone-conduction hearing aid was implanted in 11 patients who were not suitable for transcranial sound amplification. Audiological and surgical selection criteria were followed strictly. One device had to be explanted and minor revision surgery was needed in two cases for skin irritation and scarring. In general the aids were well tolerated but the amplification power of the external device proved to be insufficient in some patients, in whom bone conduction levels were on the borderline of the selection limits.
