Do clinical breast examination skills improve during medical school?

PURPOSE: To assess the effects of stage of training, gender, and specialty interest on medical students' breast cancer knowledge, attitudes, and clinical breast examination (CBE) skills as a case study of the progression of physical examination skills during medical education. METHOD: In 1996, questionnaires assessing breast cancer knowledge and attitudes were administered to 493 premedical and first-, second-, and third-year medical students at Northwestern University Medical School. Silicone breast models were used to evaluate the CBE proficiency of a subset of 151 students. RESULTS: Breas

Assessing condom use practices. Implications for evaluating method and user effectiveness.

BACKGROUND: Consistent and correct condom use remains important to human immunodeficiency virus (HIV) prevention. Although many studies evaluate consistent condom use, few examine how condoms are used during intercourse. GOALS: Assess how user practices affect exposure to risks of pregnancy and infection during condom use. STUDY DESIGN: A cross-sectional survey on condom behaviors in the past month was conducted among 98 male students attending two Georgia universities. RESULTS: Altogether, 35 of 270 total condom uses (13.0%, 95% confidence interval, 7.4-18.5) resulted in potential exposure

Condom and other contraceptive use among a random sample of female adolescents: a snapshot in time.

This study examined the sexual practices of 235 females aged 15 to 19 years and their readiness to use specific contraceptive methods for birth control and sexually transmitted disease (STD) prevention. The investigation was based on the stages-of-change construct from the Transtheoretical Model (Prochaska & DiClemente, 1983, 1984). Results demonstrated that despite the availability of newer contraceptive methods (e.g., Depo-Provera), most sexually active adolescents were least resistant to using condoms and were further along in the stages of change for condom use as compared with other contr

Self-reported factors influencing condom use among clinic attendees.

Data were collected from a convenience sample of 231 urban women seeking health care at a Midwestern women's health care center at a city medical center. Participants responded to 53 items, most taken from the Family Health Center Survey, which was designed to elicit descriptive data related to three major categories: background factors, factors that might influence the decision to use a condom, and knowledge and attitudes that might predict condom acquisition. Knowledge of HIV transmission and safer sex was not a predictor of condom use. Through education and demonstrations of proper condom u

Preliminary observations on factors responsible for long persistence and continued outbreaks of plague in Lushoto district, Tanzania.

Human plague has been an important public health problem in Tanzania for over a century. Recorded outbreaks of the disease have been reported from various parts of Tanzania, including Iringa, Kagera, Singida, Mbulu, Arusha and Kilimanjaro since 1886. Since 1980 however, only Lushoto, Singida and Karatu districts have experienced outbreaks of the disease. Of these areas, Lushoto has disproportionately high incidences of the disease and this has persisted for nearly 17 years. Efforts to curb the disease through conventional methods, including control of vectors and reservoirs, chemotherapy and c

Knowledge, attitude and practice of family planning among senior high school students in north Gonder.

Sexual experience, knowledge, attitude and practice on contraception was studied among 991 senior high school students in north Gonder in May 1993 using anonymous questionnaire. Three-hundred-four students (30.7%) answered that they had experienced sexual intercourse. Out of 83 sexually active female students 25(30.1%) reported to have been pregnant. Only four students admitted to have had abortion. Seven-hundred-fifty (75.7%) students claimed that they know at least one method of modern contraception. Of the variables considered in the study reported knowledge of pills and positive attitude t

Attitudes and practice of couples regarding sexual relations during the menses and spotting.

To determine attitudes and practices regarding sexual relations during menstruation and vaginal spotting, a cross-sectional descriptive survey was performed at the Instituto de Investigaciones Materno Infantil, Universidad de Chile, Santiago, Chile. A total of 287 women and 206 men were randomly selected from an urban population. Attitudes, beliefs, and practices regarding sexual relations during vaginal bleeding were surveyed and stratified by educational level and other demographic characteristics. Overall, 70% of women and 72% of men were found to avoid sexual relations during menstruation.

Agrochemicals exposure and health implications in Githunguri location, Kenya.

A study conducted in a rural agricultural community (Githunguri location) in Kenya between 1987 and 1990 investigated the extent of use of agrochemicals, especially pesticides, by the farmers; their level of awareness of the dangers posed by these chemicals and their attitudes towards agricultural chemicals in general. The findings showed that more than 95% of the farmers used pesticides extensively. More women than men were found to be at risk of agrochemicals exposure, while babies and children were at more risk of agrochemicals exposure than the women. In this community, knowledge and aware

[The history of family planning among women who opt for surgical sterilization in the Social Security Fund of Panama].

In this study, the previous history of family planning and prior usage of contraceptive methods are analyzed, in a sample of 175 women who voluntarily opted for surgical sterilization as a permanent method of fertility regulation, in the Panamanian Social Security System. A questionnaire was applied during the second trimester of pregnancy, by which information would be gathered in regard to knowledge, usage and adverse effects of the contraceptive methods used prior to the intervention. Results showed that 99% of the sample had heard, on some occasion about oral contraceptives (OC) and, in le

Effects of specially tailored information on Swedish university students' sexual behavior.

In independent surveys concerning sexual behavior among university students in Uppsala, Sweden, in 1989 and 1990, we found that condoms were infrequently used and that up to 25% of the sexually experienced students had a history of having had at least one sexually transmitted disease (STD). We targeted an information campaign toward the same students (approximately 20,000) in 1990. Our aims were (1) to increase the knowledge of STDs and alert the students to the high frequency of STDs in the student population, (2) to encourage students to have an STD checkup at the local STD clinic, and (3) t

Knowledge and practices among injecting-drug users of bleach use for equipment disinfection--New York City, 1993.

Sharing (i.e., multiperson use) of drug-injection equipment among injecting-drug users (IDUs) is a major risk factor in the transmission of human immunodeficiency virus (HIV) and other bloodborne pathogens. Abstaining from injection of drugs eliminates this risk; disinfection of needles and syringes with household bleach can reduce this risk. Because studies suggest the effectiveness of bleach disinfection may be limited, the March 1993 National Institute on Drug Abuse (NIDA) Community Alert Bulletin included recommendations that IDUs who do not stop injecting and sharing injection equipment u

Attitudes of teenagers toward sun exposure and sunscreen use.

A survey of 220 adolescents attending a multiphysician pediatric office in Virginia was conducted to determine the frequency with which they used sunscreens. Eighty-one percent of the teenagers in the study stated that they spent most weekends in the sun; however, only 9% always used sunscreen, while 33% never did. Factors found to be associated with increased sunscreen use included female sex (odds ratio = 4.5, P less than .0001), having a best friend who routinely used sunscreen (odds ratio = 3.0, P less than .001), having parents who insisted on sunscreen use when the teenagers were childre

[Smoking habits among adults--attitude and behavior in relation to prevention of cardiovascular diseases].

During the autumn 1989, a material of 1,330 men and 1,561 women aged 20-65 years and resident in the County of Vejle replied to a questionnaire about their smoking habits and knowledge in connection with the prophylaxis of cardiovascular disease. 47% of the men and 39% of the women stated that they smoked. During the past year, 44% of the male and 51% of the female smokers and ex-smokers had altered or considered altering their smoking habits. The main motives for altering smoking habits were health or financial reasons. The main reasons that an attempt to alter smoking habits did not succeed

Risk behaviors for HIV infection in clients of Pennsylvania family planning clinics.

Responses from a statewide survey of risk behaviors among clients of Planned Parenthood clinics in Pennsylvania indicate that a sizable proportion of these women are at intermediate or high risk for exposure to the human immunodeficiency virus (HIV). Overall, nearly five percent of the 15,499 women in the sample reported personal behavior, or a partner's behavior, that placed them at high risk, while an additional 20 percent were at intermediate risk. Among all women surveyed, 13 percent had had three or more partners in the past year, 12 percent had had a sexually transmitted disease, four pe

[Adolescence and contraception: 1. A study knowledge and use among women hospitalized for childbirth or abortion].

A study of the knowledge and utilization of contraceptive methods by adolescent is presented. An analysis was carried out based on data collected from interviews with and recorded case histories of 78 puerperal adolescents (childbirth or abortion), assisted by an obstetric service in the county of Cotia, SP, Brazil, between May 1 and July 31, 1986. Of all the adolescents studied, 61.5% had some knowledge of contraceptive methods; the findings showed that such knowledge was influenced by factors such as: age, school background, parity and marital status. The main sources of information on contr

Adolescent pregnancy: contributing factors, consequences, treatment, and plausible solutions.

Adolescence, although not always necessarily a period of extreme stress, is a time of profound physical changes which increase the adolescent's awareness of and interest in sexual behavior. This issue of sexuality, which begins in early adolescence, creates new challenges to personal and gender identity formation as the person matures. Sexual maturation is a three-fold process, requiring growth in understanding of oneself as a sexual being, in the ability to handle interpersonal relationships effectively, and in the capacity to plan behavior in view of future outcomes and present problems. Vanguard family planning acceptors in Senegal.

This study examines contraceptive use among clients at the three clinics providing family planning services in Dakar, Senegal in early 1983. Most clients first became interested in family planning following the birth of a child, and most are interested in spacing future pregnancies, although one-third state that they want no more children. The clinic itself was found to be an important determinant of the type of contraceptive used, with only the government-operated clinic providing a balance between IUDs, oral contraceptives, and barrier methods. Nearly half of the clients interviewed said tha
