Central serotonergic stimulation of aldosterone secretion.

Serotonin stimulates aldosterone secretion both in vitro and in vivo, and serotonin antagonism decreases plasma aldosterone levels in patients with idiopathic aldosteronism. This study was designed to assess the effects of the serotonin precursor, 5-hydroxytryptophan (5HTP), upon aldosterone secretion in man, and to determine whether stimulatory effects of 5HTP are mediated through the central nervous system. Oral 5HTP, administered as a single 200-mg dose, increased plasma aldosterone levels from 4.7 +/- 0.6 to 13.3 +/- 2.8 ng/dl in dexamethasone-pretreated, normal volunteers. Peripheral inhi

Correction of increased prostacyclin synthesis in Bartter's syndrome by indomethacin treatment.

The role of prostacyclin and thromboxane A2 in the pathogenesis of Bartter's syndrome was investigated by measurement of the urinary excretion of 6-keto-PGF1 alpha and thromboxane B2, respectively, in five patients. The prostaglandin metabolites were extracted from urine by a reproducible method and measured by specific radioimmunoassays. The patients with Bartter's syndrome excreted about four-times as much 6-keto-PGF1 alpha as the normal controls. In contrast, there was no significant difference in the urinary excretion of thromboxane B2 between the patients and the controls. In a second par

Metabolic acidosis and alkalosis.

The factors controlling renal bicarbonate reabsorption and acid excretion under normal conditions and in the presence of metabolic acidosis and alkalosis are reviewed. The methods used to assess distal acidification and its limitations are also discussed. Measurement of urinary pCO2 in maximally alkaline urine (pH greater than 7.8) is a very useful qualitative method to assess distal acidification. The finding of a low urinary pCO2 in maximally alkaline urine indicates a distal acidification defect. We propose that both the secretory and gradient defect types of distal renal tubular acidosis a

Role of prostaglandins in the pathogenesis of Bartter's syndrome.

Increased renal prostaglandins activated by beta-catecholamines could produce renal tubular sodium wasting and angiotensin pressor resistance observed in Bartter's syndrome. We therefore measured plasma renin activity (PRA), aldosterone and prostaglandin A (PGA) by radioimmunoassay, and body composition by isotope dilution prior to and following beta-adrenergic blockade with propranolol (200 mg/day for 4 days) and prostaglandin synthesis inhibition by indomethacin (200 mg/day for 4 days) in a patient with Bartter's syndrome on a 250 meq sodium diet. After the administration of propranolol, bod

Hypokalemia: causes, consequences and correction.

Hypokalemia poses a common diagnostic challenge with many potential etiologies. Multiple factors may contribute to this electrolyte deficiency in any given patient. Gastrointestinal potassium wasting usually is identifiable by an associated increase in fluid losses via biliary tract or bowel. A urinary potassium excretion of 20 mEq or more per day in the presence of a low serum potassium (less than 3.5 mEq/L) suggests inappropriate potassium wasting. Although diuretic therapy (loop diuretics, thiazides) undoubtedly is the most common cause of a potassium deficit, one also must consider abnorma

Primary aldosteronism. A study in contrasts.

Two patient with primary aldosteronism, one with a solitary adrenal adenoma and the other with bilateral nodular hyperplasia, are described. Both patients showed the classic features of primary aldosteronism in electrolyte and hormone patterns, but there were important differences in the biochemistry of their excised adrenal tissue. In addition, the injection of plasma from the patient with bilateral adrenal hyperplasia into the sheep's transplanted adrenal gland elicited a definite aldosterone secretory response, but there was no aldosterone response to the injection of plasma from the patien

[Bartter's disease associated with hypercorticism, phosphate and magnesium deficiencies and familial renal tubular disease].

A new case of Bartter's syndrome is described. There is a context of other cases of familial renal tubular disease with a sex-linked heredity. In this case, the Bartter's syndrome is associated with magnesium deficiency and hypomagnesemia, with a ricket and severe phosphate deficiency, and finally with an hypercorticism. The basal secretion rate of cortisol agree with a Cushing's syndrome. This hypercorticism is corrected by aminogluthetimide. The influence of the hyperreninism on the hypercorticism is discussed.

Respiratory triggered 3D bSSFP MRI enables accurate noninvasive localization of the right adrenal vein confluence in primary aldosteronism.

Catheterization of the right adrenal vein (RAV) is a technically challenging step in adrenal venous sampling (AVS) for primary aldosteronism. Accurate preprocedural evaluation of RAV anatomy is essential for procedural success. To assess the feasibility of respiratory-triggered three-dimensional balanced steady-state free precession (RT-3D bSSFP) magnetic resonance imaging for visualizing the confluence of the RAV with the inferior vena cava (IVC), in comparison with contrast-enhanced computed tomography (CECT). In this retrospective study, we evaluated RT-3D bSSFP, CECT, and CV for RAV identi

Clinical implications of 68Ga-pentixafor PET/CT in surgically treated primary aldosteronism.

BACKGROUND: CXCR4-targeted positron emission tomography/computed tomography has demonstrated potential utility in functional characterization and lateralization assessment of primary aldosteronism. However, clinical implications for postoperative outcomes after adrenalectomy and biologic correlations with adrenal histopathology remain incompletely defined in surgical cohorts. METHODS: We retrospectively analyzed data from 188 patients with primary aldosteronism who underwent CXCR4-targeted 68Ga-pentixafor positron emission tomography/computed tomography before adrenalectomy. Quantitative posi
