An interpretable artificial intelligence approach to differentiate between blastocysts with similar or same morphological grades.

STUDY QUESTION: Can a quantitative method be developed to differentiate between blastocysts with similar or same inner cell mass (ICM) and trophectoderm (TE) grades, while also reflecting their potential for live birth? SUMMARY ANSWER: We developed BlastScoringNet, an interpretable deep-learning model that quantifies blastocyst ICM and TE morphology with continuous scores, enabling finer differentiation between blastocysts with similar or same grades, with higher scores significantly correlating with higher live birth rates. WHAT IS KNOWN ALREADY: While the Gardner grading system is widely u

The epidemiology of pregnancy loss: global burden, variable risk factors, and predictions.

STUDY QUESTION: Is the incidence of pregnancy loss correlated with various geographic, socio-demographic, and age stratifications at the societal and national levels, and what are the risk factors associated with pregnancy loss at the individual level? SUMMARY ANSWER: The epidemiological trends and disease burden of pregnancy loss were correlated with various geographic, socio-demographic, and age stratifications, and we identified that poor health condition, smoking, sedentary behaviour, lower educational level, and lower maternal birth weight may significantly increase the risk of pregnancy

Prevention of self-harm and suicide in young people up to the age of 25 in education settings.

BACKGROUND: In 2016, globally, suicide was the second leading cause of death amongst those aged 15 to 29 years. Self-harm is increasingly common among young people in many countries, particularly among women and girls. The risk of suicide is elevated 30-fold in the year following hospital presentation for self-harm, and those with suicidal ideation have double the risk of suicide compared with the general population. Self-harm and suicide in young people are significant public health issues that cause distress for young people, their peers, and family, and lead to substantial healthcare costs.

Mindfulness-enhanced parenting programmes for improving the psychosocial outcomes of children (0 to 18 years) and their parents.

BACKGROUND: Emotional and behavioural difficulties (EBD) in children are common, characterised by externalising or internalising behaviours that can be highly stable over time. EBD are an important cause of functional disability in childhood, and predictive of poorer psychosocial, academic, and occupational functioning into adolescence and adulthood. The prevalence, stability, and long-term consequences of EBD highlight the importance of intervening in childhood when behavioural patterns are more easily modified. Multiple factors contribute to the aetiology of EBD in children, and parenting pl

Implementing the 2009 Institute of Medicine recommendations on resident physician work hours, supervision, and safety.

Long working hours and sleep deprivation have been a facet of physician training in the US since the advent of the modern residency system. However, the scientific evidence linking fatigue with deficits in human performance, accidents and errors in industries from aeronautics to medicine, nuclear power, and transportation has mounted over the last 40 years. This evidence has also spawned regulations to help ensure public safety across safety-sensitive industries, with the notable exception of medicine. In late 2007, at the behest of the US Congress, the Institute of Medicine embarked on a year

Safety of immune checkpoint inhibitors: A systematic review of disproportionality analysis studies.

BACKGROUND: Immunotherapy has fundamentally changed the treatment landscape for both solid and hematologic malignancies over the past decade. However, an evolving body of literature has reported immune-related adverse events (irAEs) involving several organs in patients treated with immune checkpoint inhibitors (ICIs). Our study aimed to systematically review published disproportionality analyses on ICIs, to summarize the current state of methodology and potential strengths of the analysis, while highlighting safety signals generated for these pharmacological groups. METHODS: A PubMed, Scopus,

[Latent class analysis on characteristics of self-reported sexual behavior and associated factors in college students in Tianjin].

Objective: To identify latent categories of HIV infection risk in college students with self-reported sexual behaviors in Tianjin, analyze the associated factors, and provide evidence for targeted HIV/AIDS prevention interventions in universities. Methods: Based on cross-sectional study design, an anonymous online questionnaire survey was conducted among college students selected by stratified cluster sampling from 54 colleges or universities in 16 districts of Tianjin from September to December 2024. In principle, the survey was required to cover no less than 10% of the total enrolled student

The spread of sexually transmissible drug-resistant shigellosis in England: a genomic epidemiology study.

BACKGROUND: Shigellosis, caused by Shigella bacteria, is a leading cause of diarrhoeal disease globally. In the last two decades, Shigella circulation in high-income countries has expanded from being a sporadic travel-associated illness to also being an endemic sexually transmissible illness among men who have sex with men (MSM). We aimed to characterise the nature and drivers of this multimodal Shigella transmission in a high-income setting. METHODS: In this genomic epidemiology study, we used Shigella sonnei isolates referred for national surveillance from 138 laboratories across 15 UK heal

Performance of the Palliative Prognostic Index for cancer patients: A systematic review and meta-analysis.

BACKGROUND: Clinician predicted survival for cancer patients is often inaccurate, and prognostic tools may be helpful, such as the Palliative Prognostic Index (PPI). The PPI development study reported that when PPI score is greater than 6, it predicted survival of less than 3 weeks with a sensitivity of 83% and specificity of 85%. When PPI score is greater than 4, it predicts survival of less than 6 weeks with a sensitivity of 79% and specificity of 77%. However, subsequent PPI validation studies have evaluated various thresholds and survival durations, and it is unclear which is most appropri

Comparing the efficacy of different types of exercise for the treatment and prevention of depression in youths: a systematic review and network meta-analysis.

PURPOSE: Depression disorder is the most commonly diagnosed type of mental illness among youths. Although a plethora of evidence suggests a positive relationship between exercise and lower levels of depression in youths, the findings regarding the variation in magnitude of this relationship are inconclusive with respect to the preventive and therapeutic effects of different types of exercise. This network meta-analysis aimed to determine the best type of exercise for the treatment and prevention of depression in youths. METHODS: A comprehensive search of databases, including PubMed, EMBASE, T

Therapeutic exercises for affecting post-treatment swallowing in people treated for advanced-stage head and neck cancers.

BACKGROUND: Head and neck cancer treatment has developed over the last decade, with improved mortality and survival rates, but the treatments often result in dysphagia (a difficulty in swallowing) as a side effect. This may be acute, resolving after treatment, or remain as a long-term negative sequela of head and neck cancer (HNC) treatment. Interventions to counteract the problems associated with dysphagia include swallowing exercises or modification of diet (bolus texture, size), or both. OBJECTIVES: To determine the effects of therapeutic exercises, undertaken before, during and/or immedia

Comparative outcomes and prognosis in patients with ureteral upper tract urothelial carcinoma undergoing segmental ureterectomy versus radical nephroureterectomy: a multicenter cohort study.

BACKGROUND: Although radical nephroureterectomy (RNU) with bladder cuff excision remains the gold standard treatment for upper tract urothelial carcinoma (UTUC), segmental ureterectomy (SU) may offer a nephron-sparing alternative. Studies comparing oncologic outcomes and renal functional outcomes between SU and RNU have yielded controversial, often conflicting results. Furthermore, investigations specifically involving ureteral UTUC patients in northern China are scarce. METHODS: We retrospectively enrolled 546 ureteral urothelial carcinoma patients (282 SU and 264 RNU cases) from three hospi

Optimized personalized management approach for moderate/severe OHSS: development and prospective validation of an OHSS risk assessment index.

STUDY QUESTION: Can a simplified ovarian hyperstimulation syndrome (OHSS) risk assessment index be developed and validated with sufficient discrimination of moderate/severe OHSS from those without OHSS? SUMMARY ANSWER: This easy-to-use OHSS risk assessment index shows good discriminative power and high calibration accuracy in internal and external validation cohorts. WHAT IS KNOWN ALREADY: An early alert and risk stratification is critical to prevent the occurrence of OHSS. We have previously developed a multi-stage smartphone app-based prediction model to evaluate the risk of OHSS, but app

Clinical outcomes of untreated adults living with chronic hepatitis B in The Gambia: an analysis of data from the prospective PROLIFICA cohort study.

BACKGROUND: Expanding antiviral therapy to people with chronic hepatitis B virus (HBV) infection who are ineligible to receive treatment under current international criteria has been increasingly debated. Evidence to support this approach is scarce, especially in Africa. We aimed to address this knowledge gap by analysing the clinical outcomes of people with chronic hepatitis B in The Gambia who were untreated and ineligible for antiviral therapy at diagnosis. METHODS: Between Dec 7, 2011, and Jan 24, 2014, we implemented the prospective PROLIFICA cohort study in The Gambia. Participants with

Home- or community-based programmes for treating malaria.

BACKGROUND: Malaria is an important cause of morbidity and mortality, in particular among children and pregnant women in sub-Saharan Africa. Prompt access to diagnosis and treatment with effective antimalarial drugs is a central component of the World Health Organization's (WHO) strategy for malaria control. Home- or community-based programmes for managing malaria are one strategy that has been proposed to overcome the geographical barrier to malaria treatment. OBJECTIVES: To evaluate home- and community-based management strategies for treating malaria. SEARCH METHODS: We searched the Cochra

Group B streptococcus colonization in pregnancy and neonatal outcomes: a three-year monocentric retrospective study during and after the COVID-19 pandemic.

BACKGROUND: Group B Streptococcus (GBS) is a major cause of sepsis and meningitis in newborns. The Centers for Disease Control and Prevention (CDC) recommends to pregnant women, between 35 and 37 weeks of gestation, universal vaginal-rectal screening for GBS colonization, aimed at intrapartum antibiotic prophylaxis (IAP). The latter is the only currently available and highly effective method against early onset GBS neonatal infections. Since the onset of the coronavirus disease 2019 (COVID-19) pandemic, the preventive measures implemented to mitigate the effects of SARS-CoV-2 infection led to

Non-pharmacological interventions for preventing delirium in hospitalised non-ICU patients.

BACKGROUND: Delirium is an acute neuropsychological disorder that is common in hospitalised patients. It can be distressing to patients and carers and it is associated with serious adverse outcomes. Treatment options for established delirium are limited and so prevention of delirium is desirable. Non-pharmacological interventions are thought to be important in delirium prevention.  OBJECTIVES: To assess the effectiveness of non-pharmacological interventions designed to prevent delirium in hospitalised patients outside intensive care units (ICU). Evidence based evaluation of immuno-coagulatory interventions in critical care.

UNLABELLED: Cochrane systematic reviews with meta-analyses of randomised trials provide guidance for clinical practice and health-care decision-making. In case of disagreements between research evidence and clinical practice, high quality systematic reviews can facilitate implementation or deimplementation of medical interventions into clinical practice. This applies especially to treatment of critically ill patients where interventions are most often costly and the clinical conditions are associated with high mortality. OBJECTIVES: To assess the potential benefits or harms of 1) antithrombin

[Orofacial myofunctional reeducation assisted by a prefabricated reeducation appliance: a systematic review of the literature].

INTRODUCTION: Several cross-sectional studies have shown the association of a dysfunctional orofacial environment with a greater prevalence of malocclusions. Orofacial myofunctional reeducation (OFMR) is the rehabilitation of the muscles, functions and resting postures of the orofacial complex. It is used in the therapeutic management of orofacial dysfunction in patients of all ages and with a wide range of disorders and comorbidities. RMOF mainly uses isotonic and isometric exercises targeting the oral and oropharyngeal structures, combined with specific exercises for ventilation, swallowing
