Comparison of operations for coarctation of the aorta in infants.

During a 12-year-period, 33 infants 4 days to 1 year old underwent 36 operations for treatment of coarctation of the aorta. Associated cardiac anomalies were present in 27 infants (81.8%). All patients presented with congestive heart failure and underwent cardiac catheterization and aortography. Resection was performed in 11 infants, patch aortoplasty in 6, bypass in 2 and subclavian flap angioplasty in 17. Pulmonary artery banding was performed in 4 patients and ligation of a patent ductus arteriosus in 21 patients. Aortic crossclamp time was shorter in the 17 patients who had subclavian flap

Prevention and management of laryngeal stenosis in infants and children.

Acquired laryngeal stenosis is a well-documented complication of endotracheal intubation. Premature infants requiring ventilatory support for respiratory distress syndrome are at increased risk for developing laryngeal stenosis. Until recently, the techniques for repair of laryngeal stenosis have required a temporary tracheotomy which complicates the management and has an associated mortality. In 1980 the use of an anterior cricoid split was described to treat laryngeal stenosis in infants. The technique has been modified: the anterior cricoid split procedure as originally described required a

Total parenteral nutrition in very low birthweight infants: a controlled trial.

34 preterm infants with birthweights <1200 g were randomly assigned to total parenteral nutrition (TPN) or oral (Milk) feeding regimens for the first 2 weeks after birth. Infants in the TPN group were started on a modified Vamin-based glucose amino-acid infusion and Intralipid. The daily amounts of carbohydrate, amino-acids, and fat infusions were increased. In the Milk group, infants were started on intermittent gavage feeding, supplemented with a glucose-electrolyte infusion as necessary. The overall mortality rate did not differ in the two groups. Four infants in the Milk group developed ne

A Rare Case of Spontaneous Rupture of Giant Sacrococcygeal Teratoma: A Case Report From Muhimbili National Hospital, Tanzania.

A rare but potentially fatal neonatal emergency, ruptured sacrococcygeal teratoma (SCT) is characterized by abrupt distension of the abdomen, anemia, and shock from hemorrhage into the tumor or peritoneal cavity. To reduce mortality and enhance results, early detection and quick surgical intervention are essential. Clinicians who treat infants with large SCTs must be aware of this serious consequence.

Percutaneous closure of patent ductus arteriosus in premature infants: A French national survey: A Titanic movement in technology and technique- Beware the icebergs!

First national multicenter report of interventional closure of patent ductus arteriosus in preterm infants. Results describe commendable technical and procedural success of 99% with year-on-year tendency for intervention in smaller patients. Reported complications (8.9%) and outcome (1-year mortality 6.8%) are far superior to contemporary surgical intervention but indicate room for improvement.

Lethal chondrodysplasia punctata.

The radiological findings in five infants, with chondrodysplasia punctata, who died during the neonatal period are presented to illustrate the role of radiology in diagnosis and prognosis in this syndrome. Rhizomelic shortening of the limbs almost inevitably indicates an early demise. In the non-rhizomelic group, features such as excessive stippling and tracheal calcification are unreliable prognostic factors of early morbidity and mortality.

Hypothermia in Preterm Newborns: Impact on Survival.

Background. Globally, prematurity is the leading cause of neonatal mortality, and hypothermia is one of its contributing factors. The goal of this study was to determine the association between hypothermia and mortality. Methods. A prospective, multi-center, descriptive clinical study was conducted in 5 hospitals in Ethiopia. Axillary temperatures were taken at the time of admission to the newborn intensive care units (NICU) and followed during the NICU stay. Results. A total of 3852 premature neonates (<37 weeks) were admitted to the NICUs from July 2016 to May 2018. Of these infants, 1109 (2

Parental smoking. Its effects on fetus and child health.

Smoking is a risk factor in pregnancy. Maternal smoking reduces the birthweight by 150-250 g (dose-response relationship), shifting the birthweight (and length) distribution to the left. Reduction of birthweight without shortening of gestation creates a group of small-for-date babies. Intrauterine hypoxia seems responsible for the growth retardation in smokers' babies as in babies of mothers living at high altitudes. Cigarette smoking of the mother affects fetal oxygenation, due to high levels of COHb in the blood of both mother and child. This observation is strongly supported by animal exper

Fetal outcomes of critically ill pregnant women admitted to the intensive care unit for nonobstetric causes.

INTRODUCTION: The outcome of the fetus in critically ill mothers has been briefly reported as a part of descriptive studies focusing on maternal risk factors for admission to the intensive care unit. We evaluated the risk factors for adverse fetal outcomes in critically ill pregnant women admitted to the intensive care unit for nonobstetrical reasons. DESIGN: Retrospective cohort study of all critically ill pregnant patients >18 yr; admitted to four (medical, surgical, trauma, and mixed medical-surgical) intensive care units at the Mayo Clinic in Rochester, MN; during the period of January 19

Mortality and ventilatory effects of central serotonin deficiency during postnatal development depend on age but not sex.

Serotonin (5-HT) influences brain development and has predominantly excitatory neuromodulatory effects on the neural respiratory control circuitry. Infants that succumb to sudden infant death syndrome (SIDS) have reduced brainstem 5-HT levels and Tryptophan hydroxylase 2 (Tph2). Furthermore, there are age- and sex-dependent risk factors associated with SIDS. Here we utilized our established Dark Agouti transgenic rat lacking central serotonin KO to test the hypotheses that CNS 5-HT deficiency leads to: (1) high mortality in a sex-independent manner, (2) age-dependent alterations in other CNS a

Cardiopulmonary Bypass Strategy to Facilitate Transfusion-Free Congenital Heart Surgery in Neonates and Infants.

Priming the cardiopulmonary bypass (CPB) circuit without the addition of homologous blood constitutes the basis of blood-saving strategies in open-heart surgery. For low-weight patients, in particular neonates and infants, this implies avoidance of excessive hemodilution during extracorporeal circulation. The circuit has to be miniaturized and tubing must be cut as short as possible to reduce the priming volume to prevent unacceptable hemodilution with initiating CPB. During perfusion, measures should be taken to prevent blood loss from the primary circuit to avoid replacement by additional vo

Elective high-frequency oscillatory ventilation in preterm neonates: a preliminary investigation in a developing country.

BACKGROUND: Elective high-frequency oscillatory ventilation (HFOV) in preterm infants is known to be as effective as conventional ventilation in the prevention of bronchopulmonary dysplasia without the risk of increased mortality or brain damage. OBJECTIVE: To document the feasibility and safety of early use of HFOV in preterm neonates with respiratory distress syndrome (RDS) in a resource-limited setting. The primary outcome was survival or death, and the secondary outcome included complication rates observed among the two groups, HFOV and synchronised intermittent mandatory ventilation (SIM

Therapeutic hypothermia for newborn infants with hypoxic-ischaemic encephalopathy.

Peripartum asphyxia complicated by moderate or severe hypoxic-ischaemic encephalopathy is a devastating global health issue. A therapeutic 'window of opportunity' exists after resuscitation of the asphyxiated newborn and before the delayed phase of neuronal loss. Animal studies demonstrated that neuronal injury following hypoxia-ischaemia can be prevented or reduced by a mild reduction in brain temperature. Human infant pilot studies confirmed feasibility, without major adverse effects. Randomised trials and systematic reviews comprising term infants with moderate or severe encephalopathy and

Animal-derived surfactants: where are we? The evidence from randomized, controlled clinical trials.

Animal-derived surfactants, as well as synthetic surfactants, have been extensively evaluated in the treatment of respiratory distress syndrome (RDS) in preterm infants. Three commonly available animal-derived surfactants in the United States include beractant (BE), calfactant (CA) and poractant alfa (PA). Multiple comparative studies have been performed using these three surfactants. Prospective as well as retrospective studies comparing BE and CA have shown no significant differences in clinical or economic outcomes. Randomized, controlled clinical trials have shown that treatment with PA is

Short-term outcome of very low birth weight infants in a developing country: comparison with the Vermont Oxford Network.

OBJECTIVES: To determine the outcome of very low birth weight infants (VLBWI) admitted to a level III NICU in UAE and compare the results to percentiles published by the Vermont Oxford Network (VON). METHOD: Outcome data were collected retrospectively, using standard definitions, on a cohort of VLBWI 500-1500 g admitted between January 2004 and December 2006. RESULTS: Of the 173 infants weighing 501-1500 g at birth, 85.6% survived until discharge, which corresponds to the 50th percentile (P50) of VON. Chronic lung disease (CLD) occurred in 12.1% (<P25), death or CLD 26.6%, necrotizing entero

Myeloid hematopoietic growth factors and their role in prevention and/or treatment of neonatal sepsis.

Sepsis continues to be an important cause of morbidity and mortality among both full-term and preterm infants, secondary to an immaturity in neonatal host defense. The incidence of neonatal sepsis ranges from 30% in very low birth weight infants to 0.4% in preterm neonates and 0.1% in term neonates. The dysregulation of the expression and production of hematopoietic cytokines in the neonate contributes to quantitative and qualitative deficiencies in neonatal myeloid progenitor activity and decreased availability and function of mature effector neutrophils. These abnormalities contribute in lar

Does breast feeding have any impact on non-infectious, non-allergic disorders?

Feeding of breast milk in the first weeks of life appears to have a strong protective effect against necrotising enterocolitis. Nevertheless breast milk also seems to be positively linked to the development of jaundice and to late haemorrhagic disease in infants who have not received vitamin K supplements. There is no consistent evidence that other childhood conditions such as insulin dependent diabetes or cancer are less prevalent among children who have been breast fed. Among adult conditions suggested to be less prevalent in the breast fed, only single reports of significant findings for mu

Neonatal listeriosis: a summer outbreak.

Neonatal listeriosis, a condition associated with high morbidity and mortality, has been reported infrequently in Australia, with only seven isolated cases appearing in the literature. This paper describes 12 cases of neonatal listeriosis diagnosed in two Western Australian hospitals in the 22 months after January, 1978. These were the only cases diagnosed at these hospitals between 1970 and December, 1980. The early onset form of the disease was diagnosed in 11 infants, a group characterized by the frequent associations of low birthweight (mean, 1805 g; range, 1200 g to 2670 g), preterm deliv
