Liver Transplantation as a Definitive Treatment for Homozygous Protein C Deficiency.

Protein C is a vitamin K-dependent anticoagulant glycoprotein that inhibits activated factors V and VIII once activated by thrombin while promoting fibrinolysis. Patients with homozygous protein C deficiency generally present shortly after birth with purpura fulminans, retinal hemorrhage, and signs of the central nervous system or renal thrombosis. This case reports the first case of homozygous protein C deficiency in the UAE successfully treated with liver transplantation. This article aims to emphasize the importance of early diagnosis of neonatal purpura fulminans based on clinical presenta

A pragmatic clinical trial of hearing screening in primary care clinics: cost-effectiveness of hearing screening.

BACKGROUND: Hearing loss is a high prevalence condition among older adults, is associated with higher-than-average risk for poor health outcomes and quality of life, and is a public health concern to individuals, families, communities, professionals, governments, and policy makers. Although low-cost hearing screening (HS) is widely available, most older adults are not asked about hearing during health care visits. A promising approach to addressing unmet needs in hearing health care is HS in primary care (PC) clinics; most PC providers (PCPs) do not inquire about hearing loss. However, no cost

The Integration of Clinical Decision Support Systems Into Telemedicine for Patients With Multimorbidity in Primary Care Settings: Scoping Review.

BACKGROUND: Multimorbidity, the presence of more than one condition in a single individual, is a global health issue in primary care. Multimorbid patients tend to have a poor quality of life and suffer from a complicated care process. Clinical decision support systems (CDSSs) and telemedicine are the common information and communication technologies that have been used to reduce the complexity of patient management. However, each element of telemedicine and CDSSs is often examined separately and with great variability. Telemedicine has been used for simple patient education as well as more com

Symptom Management Following Radioactive Iodine Treatment in Adults With Thyroid Cancer: Usability Evaluation and Proof-of-Concept Study.

BACKGROUND: Thyroid cancer is often labeled a "good cancer" due to high survival rates. However, many patients diagnosed with differentiated thyroid cancer (DTC) treated with radioactive iodine (RAI) experience short- and long-term symptoms as a result of treatment. These include salivary dysfunction, dry mouth, dry eyes, and nausea. Despite these challenges, patients report limited knowledge and unmet informational needs regarding RAI symptoms and symptom management. Evidence-based, patient-centered education to support symptom management during DTC survivorship is needed. Our team developed

Impact of nurse-led supportive care intensity on quality of life and symptom burden in patients undergoing palliative chemotherapy: A prospective cohort study

Nurse-led supportive care is integral to palliative oncology, yet the intensity of nursing contact is rarely quantified, and its clinical relevance remains incompletely defined. We prospectively examined the association between supportive care intensity (SCI) and patient-reported outcomes and survival in patients receiving palliative chemotherapy. This prospective observational cohort study enrolled adults with advanced solid tumors undergoing palliative chemotherapy. All participants received routine physician-led oncology care and standard chemotherapy nursing, while additional supportive se

Reinventing vitamin K antagonist therapy management: an implementation study of a home-based integrated monitoring model.

Vitamin K antagonist (VKA) oral anticoagulant therapies are crucial for preventing thromboembolic complications in cardiovascular diseases; however, their narrow therapeutic range necessitates frequent monitoring of the International Normalized Ratio (INR), posing significant challenges for elderly and frail patients. This article presents the implementation of a new home-based monitoring model for VKA therapy that integrates digital tools with clinical supervision. Since 2022, the Local Health Authority of Bologna (Italy) has implemented a home-based coagulation diagnostics model to improve V

Telmisartan: distinct pharmacological features, partial PPAR-γ modulation, and pleiotropic therapeutic potential.

Telmisartan, a selective angiotensin II type 1 (AT1) receptor antagonist, is widely used in the management of hypertension by selectively blocking AT1 receptor mediated signaling, thereby attenuating the deleterious effects of excessive renin angiotensin aldosterone system (RAAS) activation without directly inhibiting ACE2, AT2 receptors, or other components of the renin angiotensin system. This results in reduced vascular resistance, decreased aldosterone secretion, and improved cardiovascular and renal protection. This review outlines the broadened therapeutic scope of telmisartan, highlight

Rickettsia rickettsii encephalitis in Brazilian spotted fever: a case report.

Brazilian spotted fever, caused by Rickettsia rickettsii, is a life-threatening tick-borne disease endemic to southeastern Brazil and is associated with high mortality when treatment is delayed. Central nervous system involvement poses a particular diagnostic challenge because of nonspecific early symptoms and frequent misdiagnosis as dengue or leptospirosis. We report a case illustrating characteristic neuroimaging findings and therapeutic considerations in rickettsial encephalitis. We describe the clinical course, cerebrospinal fluid (CSF) findings, magnetic resonance imaging (MRI) findings,

Review of Recent Advances in Implantable Brain-Computer Interfaces for the Restoration of Motor Function in Patients With Paralysis.

Implantable brain-computer interfaces (BCIs) - positioned at the intersection of neuromedicine and clinical neurorehabilitation - have achieved notable advances in restoring motor function for individuals with paralysis. By using invasive electrodes to directly sample cortical neuronal activity and translating these signals into control commands for external effectors, BCIs offer a viable therapeutic pathway for severe motor impairment. On the mechanistic front, steady improvements in neural signal acquisition and decoding have enabled more precise capture of movement intent and real-time cont

Pharmacological Complexities of Herbal Medicinal Extracts: A Review of Bioavaila-bility, Interactions, and Standardization.

Herbal medicines have gained remarkable popularity due to their natural origins and potential medicinal value. Nevertheless, they are chemically complex and pose signifi-cant pharmacological challenges. This review focuses on the key aspects influencing their clinical use: their variable bioavailability, complex pharmacokinetics (ADME), and potential for interactions. A primary concern is herb-drug interactions, with special emphasis on the modulation of drug-metabolizing enzymes by specific phytoconstituents, which can alter drug concentrations to an extent that may be life-threatening, causi

Patients With Chronic Pain and Their Perspectives on Barriers and Opportunities for Earlier Opioid Tapering-A Qualitative Study Using Patient Journey Mapping.

PURPOSE: Long-term opioid use is associated at the population level with increased risks such as physical dependence, adverse effects and complex healthcare trajectories. While some patients report improved function and quality of life with carefully monitored opioid therapy, others experience diminishing benefits over time and substantial harms. While tapering is clinically recommended, little is known about patients' perspectives on barriers and opportunities for initiating earlier opioid tapering. This study explores how patients experience and reflect on these issues throughout their patie
