Examination of cervical spine kinematics in complex, multiplanar motions after anterior cervical discectomy and fusion and total disc replacement.

BACKGROUND: The biomechanical behavior of total disc replacement (TDR) and anterior cervical discectomy and fusion (ACDF) incomplex multiplanar motion is incompletely understood. The purpose of this study was to determine whether ACDF or TDR significantly affects in vitro kinematics through a range of complex, multiplanar motions. METHODS: Seven human cervical spines from C4-7 were used for this study. Intact cervical motion segments with and without implanted TDR and ACDF were tested by use of unconstrained pure bending moment testing fixtures in 7 mechanical modes: axial rotation (AR); flex…

A general approach for prediction of motional EPR spectra from Molecular Dynamics (MD) simulations: application to spin labelled protein.

A general approach for the prediction of EPR spectra directly and completely from single dynamical trajectories generated from Molecular Dynamics (MD) simulations is described. The approach is applicable to an arbitrary system of electron and nuclear spins described by a general form of the spin-Hamiltonian for the entire motional range. It is shown that for a reliable simulation of motional EPR spectra only a single truncated dynamical trajectory generated until the point when correlation functions of rotational dynamics are completely relaxed is required. The simulation algorithm is based on…

Short range of motion isokinetic testing of wrist flexor and extensor strength in normal subjects and patients with carpal tunnel syndrome.

The purpose of this study was to compare the utility of measuring wrist flexors and extensors strength derived from 'full' range of motion (FRoM): 60 degrees of flexion to 30 degrees of extension, and 3 equally spaced short ranges of motion: SRoM1, 60-30 degrees and SroM2, 30-0 degrees of flexion and SRoM3, 0-30 degrees of wrist extension. Fifteen apparently healthy subjects and 8 patients suffering from unilateral carpal tunnel syndrome (CTS) participated in the study. In all participants, SRoM1 findings closely resembled those obtained from FRoM. In the patient groups, the muscular strength …

Analysis of the origin of cardiac wall motion that constitutes myocardial velocity-time curves in patients with left bundle branch block.

Septal and lateral wall myocardial velocity-time curves from tissue Doppler imaging were analyzed to determine wall motion from which the velocity originated in 34 patients with left bundle branch and systolic dysfunction (ejection fraction < 45%). Longitudinal strain rate by speckle tracking imaging was assessed to identify whether corresponding wall motion was active or passive. All lateral peak velocities during the ejection period were derived from delayed active movement. However, septal peak velocities were more numerous and complex. Septal peak velocities during pre-ejection were derive…

Venous ulcers: A reappraisal analyzing the effects of neuropathy, muscle involvement, and range of motion upon gait and calf muscle function.

Chronic venous insufficiency is a complex disease that can result in severe sequelae including venous ulceration. Though the exact progressi … sult in neuropathic pain and muscle dysfunction, alterations in mobility and a decrease in range of motion may lead to gait alterations all affecting calf muscle pump function. In this paper, we analyze these cofactors and discuss possible treatment options to target them. Physicians treating this disease should be aware of the numerous factors involved …

Peripheral neuritis and increased spinal cord neurochemicals are induced in a model of repetitive motion injury with low force and repetition exposure.

Performance of high repetition tasks with or without force is associated with peripheral tissue inflammation, decreased nerve function and motor dysfunction. Here, we examined whether a low repetition task with negligible force (LRNF) produces fewer tissue and behavioral pathologies than previously observed with high repetition tasks using our rat model of repetitive motion injury (RMI). Thirty-seven rats were randomized into control or LRNF groups, the latter reaching and grasping a 45 mg food pellet at a rate of 3 reaches/min. This task was performed in 4, 0.5 5 h sessions with 1.5 5 h rest …

Observer agreement in assessing flexion-extension X-rays of the cervical spine, with and without the use of quantitative measurements of intervertebral motion.

BACKGROUND CONTEXT: Flexion-extension X-rays are commonly used to identify abnormalities in intervertebral motion, despite little evidence for the reliability of the information that clinicians derive from these test. PURPOSE: Quantify observer agreement on intervertebral motion abnormalities assessed with and without the use of computer-assisted technology. STUDY DESIGN: Assess interobserver agreement among clinicians when they evaluate cervical flexion-extension X-rays using the methods they now use in clinical practice, and compare this to observer agreement when the same clinicians reass…

Systolic outward motion of the left ventricular apical wall as detected by magnetic resonance tagging in patients with apical hypertrophic cardiomyopathy.

Patients with apical hypertrophic cardiomyopathy (APH) associated with paradoxic jet flow (ie, diastolic flow away from the apex) may gradually develop an apical aneurysm, which often leads to arrhythmia and mural thrombus formation. We observed systolic outward motion of the left ventricular apical myocardium in patients with APH using a magnetic resonance tagging procedure and examined the relationship of the outward motion to echocardiographic and scintigraphic findings and to cardiac events. Systolic displacement of the myocardial tags of the apical region perpendicular to the long axis in…

The initial safe range of motion of the ankle joint after three methods of internal fixation of simulated fractures of the medial malleolus.

BACKGROUND: Previous studies have demonstrated the safe passive range of ankle motion for inter-bone stiffness after internal fixation under load but there is a lack of information about the safe range of ankle motion for early rehabilitation in the absence of loading. The present study was designed to assess the effect of ankle movement on inter-bone displacement characteristics of medial malleolus fractures following three types of internal fixation to determine the safe range of motion. METHODS: Five lower legs obtained during autopsy were used to assess three types of internal fixation (t…

Effects of fast head turns on head, trunk and pelvis motions during standing and walking in patients with unilateral vestibular deficit.

Patients with unilateral vestibular deficit (UVD) report difficulties with maintaining balance while executing fast head turns. Our aim was to determine whether head, trunk, and pelvis angular displacements were symmetrical in patients with UVD as they executed voluntary yaw rotation of the head towards or away from the side of the vestibular lesion, during standing and walking. Eight patients who underwent surgical resection of an acoustic neuroma stood with feet together or walked at comfortable pace across a 10-meter walkway. They turned the head as quickly and as fast as possible in the di

A syndrome of transient left ventricular apical wall motion abnormality in the absence of coronary disease: a perspective from the United States.

BACKGROUND: The syndrome of chest pain associated with characteristic anterior electrocardiographic changes, moderate increases in cardiac enzymes, and a reversible apical wall motion abnormality in the absence of coronary artery disease has been documented in Japan, but has received relatively little attention in other countries. METHODS: The clinical and echocardiographic data of 12 patients (11 women, mean age 64 +/-14 years) who presented with chest symptoms, electrocardiographic (ECG) changes indicative of an acute anteroapical myocardial infarction, abnormal cardiac enzyme levels and ec…

Dap-SL: a new site-directed nitroxide spin labeling approach for determining structure and motions in synthesized peptides and proteins.

A new approach for site-directed placement of nitroxide spin labels in chemically synthesized peptides and proteins is described. The scheme takes advantage of a novel diaminopropionic acid scaffold to independently control backbone and side chain elongation. The result is a spin-labeled side chain, referred to as Dap-SL, in which an amide bond forms a linker between the nitroxide and the peptide backbone. The method was demonstrated in a series of helical peptides. Circular dichroism and nuclear magnetic resonance showed that Dap-SL introduces only a minor perturbation in the helical structur

Characterization of the internal motions of a chimeric protein by 13C NMR highlights the important dynamic consequences of the engineering on a millisecond time scale.

By transferring the central curaremimetic beta hairpin of the snake toxin alpha into the scaffold of the scorpion charybdotoxin, a chimeric protein was constructed that reproduced the three-dimensional structure and partially reproduced the function of the parent beta hairpin, without perturbing the three-dimensional structure of the scaffold [1]. Picosecond to hour time scale motions of charybdotoxin and the engineered protein were observed, in order to evaluate the dynamic consequences of the six deletions and eight mutations differentiating the two molecules. The chimeric protein dynamics w…
