Factitial leg ulcers associated with an unusual sleep disorder.

A 19-year-old woman presented with a 14-month history of multiple, factitially induced, full-thickness leg ulcers. Nocturnal polysomnography with video monitoring revealed a short sleep latency and frequent sleep stage changes. Rapid eye movement sleep was absent. All stages of non-rapid eye movement sleep were associated with leg rubbing. After treatment that included avoidance of daytime naps and administration of chlorpromazine at bedtime, affect, daytime behavior, nocturnal sleep, and the ulcers improved markedly. To our knowledge, there are no reports of factitial dermatosis associated wi

Motion of the body centre of gravity as a summary indicator of the mechanics of human pathological gait.

Abnormal movements of the body segments due to lowest level gait disorders such as musculoskeletal disorders, peripheral neuropathies and radiculopathies or middle-level disorders such as hemiplegia, paraplegia and dystonia influence the motion of the centre of gravity (CG) during walking. The translation of the CG can be studied by the work done by muscles (WExt) with respect to the ground. The efficacy of gait's mechanism can be quantified by the energy transferred between gravitational potential and kinetic energies (recovery). WExt and recovery were investigated in lowest and middle-level

Echography-guided Botulinum Toxin for Moving Ear Syndrome.

BACKGROUND: Moving Ear Syndrome is a rare hyperkinetic disorder. PHENOMENOLOGY SHOWN: This Video Abstract illustrates typical backward movements of the right ear associated with pain and discomfort in a man with Moving Ear Syndrome. EDUCATIONAL VALUE: Moving Ear Syndrome is effectively and safely treatable with EMG-US-guided botulinum toxin injections.

A case of intractable psychogenic essential palatal tremor.

Essential palatal tremor (EPT) is a rare disorder which shows rhythmic involuntary movement of the muscles of soft palate, especially tensor veli palatini muscle. EPT is classified by two subtypes, which is primary and secondary EPT. Secondary EPT includes psychogenic type. We describe a case of intractable psychogenic EPT.

Creating a Japanese version of the attitudes toward dreams scale: Attitude toward dreams may predict sleep disorders.

Previous studies have identified two factors that influence people's attitudes toward dreams: positive and negative. However, cultural differences may influence these attitudes. This study aimed to develop a Japanese version of a scale to assess attitudes toward dreams and to examine factors related to Japanese participants' attitudes toward dreams. These factors included dream attitudes, dream-recall frequency, and various sleep-related variables (Pittsburgh Sleep Quality Index, sleep apnea, restless legs syndrome, insomnia, and rapid eye movement sleep behavior disorder), personality traits,

First prospective European study for the feasibility and safety of magnetically controlled capsule endoscopy in gastric mucosal abnormalities.

BACKGROUND: While capsule endoscopy (CE) is the gold standard diagnostic method of detecting small bowel (SB) diseases and disorders, a novel magnetically controlled capsule endoscopy (MCCE) system provides non-invasive evaluation of the gastric mucosal surface, which can be performed without sedation or discomfort. During standard SBCE, passive movement of the CE may cause areas of the complex anatomy of the gastric mucosa to remain unexplored, whereas the precision of MCCE capsule movements inside the stomach promises better visualization of the entire mucosa. AIM: To evaluate the Ankon MCC

Tardive Dystonia.

Tardive dyskinesia is a movement disorder that develops after exposure to dopamine receptor blocking agents. Less well-appreciated are other, more recently described tardive syndromes that are phenomenologically distinct from tardive dyskinesia and respond to different treatments. Patients may simultaneously have more than one tardive syndrome. Major subtypes of tardive syndromes include tardive dyskinesia, characterized by orobuccolingual, truncal, or appendicular, choreiform movements; tardive dystonia, characterized by sustained, stereotyped muscle spasms of a twisting or turning character;

Charcot-Marie-Tooth disease: A case report initially manifested by bilateral vocal cord paralysis.

Charcot-Marie-Tooth is an inherited disorder involving multiple genes, causing progressive nerve damage affecting sensation and movement. The complexity of the condition often leads to various possible diagnoses along with neuropathic diseases, sometimes resulting in significant delays in diagnosis and treatment. Thorough clinical examinations, suspicion based on symptoms, electromyography, nerve conduction tests, and specific genomic testing can expedite diagnosis. Here, we will introduce a case of Charcot-Marie-Tooth disorder, initially presenting with stridor and hoarseness due to vocal cor

Risperidone in the treatment of behavioral disorders associated with autism in children and adolescents.

This is a review of the clinical trials investigating the efficacy and safety of risperidone in the treatment of children with autistic spectrum disorders (ASD). The main clinical characteristics are impairment in social skills, communication difficulties, repetitive movements and behaviors, including stereotypies. Pharmacotherapy is mainly directed at the so-called target symptoms, ie, behavioral disorders and the various kinds of repetitions associated with ASD. According to the available data, risperidone seems to be moderately efficacious and safe for treating behavioral disorders. 4 doubl

Benign neonatal sleep myoclonus mimicking status epilepticus.

Benign neonatal sleep myoclonus is a self-limited movement disorder characterized by neonatal-onset myoclonic jerks only during sleep, abrupt and consistent cessation with arousal, and absence of concomitant electrographic changes suggestive of seizures. It has a good outcome and was included in the differential diagnosis of neonatal seizures. A presumed transient serotonin imbalance and genetic factors may play a role in the pathogenesis of this disorder. We report a case of benign neonatal sleep myoclonus mimicking status epilepticus in an infant with a family history of nocturnal myoclonus,

[Rett syndrome: clinical and molecular aspects].

Rett syndrome is one of the leading causes of mental retardation and developmental regression in girls. It is characterized by a period of normal psychomotor development followed by the loss of acquired motor and communication skills, autistic features and stereotypic hand movements. Rett syndrome is the first pervasive developmental disorder with a known genetic cause. The majority of cases are caused by de novo mutations in an X-linked MECP2 gene. Its product, methyl-CpG-binding protein 2, plays an important role in the regulation of gene expression and chromatin structure. Because the neuro

Prenatal and Perinatal Morbidity in Children with Tic Disorders: A Mainstream School-based Population Study in Central Spain.

BACKGROUND: While current research suggests that genetic factors confer the greatest risk for the development of tic disorders, studies of environmental factors are relatively few, with a lack of consistent risk factors across studies. Our aim is to analyze the association of tic disorders with exposure to prenatal and perinatal morbidity. METHODS: This was a nested case-control study design. Cases and controls were selected and identified from a mainstream, school-based sample. The diagnosis of tic disorders was assigned by a movement disorder neurologist using 'Diagnostic and statistical ma

3D Gait Network Analysis of Ground Reaction Forces: A Case Study of Lower Limb Disorders and Walking Speeds.

OBJECTIVE: Previous research has indicated that human gait can be quantified using three-dimensional signals, each of which corresponds to distinct locomotor modules. However, no study has thoroughly investigated their interactions and the combined influence they exert on gait in a holistic way. METHODS: Three-dimensional ground reaction force (GRF) signals were recorded from 2,295 participants, including 211 healthy individuals and 2,084 patients with lower limb disorders- specifically those affecting the calcaneus, hip joint, knee joint, and ankle joint-for illustrative purposes. Intravenous anesthetics and opioid analgesics demonstrated the highest signal rates (44.4% each).
