Immunomodulatory Nanocarriers in Multiple Sclerosis: Dual Targeting of Microglia and Mitochondria for Precision Neurotherapy.

The chronic neuroinflammatory and neurodegenerative disease known as Multiple Sclerosis (MS) is characterized by mitochondrial dysfunction and ongoing microglial activation. Developments in nanomedicine have enabled the design of immunomodulatory nanocarriers that target mitochondria and microglia simultaneously, thereby addressing two key pathogenic characteristics. By delivering antioxidants and anti-inflammatory drugs straight to microglia and their mitochondria, these systems increase the effectiveness and specificity of treatment. This review summarizes preclinical research on nanocarrier…

The exercise hormone irisin has neuroprotective effects in a mouse model of multiple sclerosis.

Aerobic exercise is a disease-modifying intervention in multiple sclerosis (MS) that ameliorates several progressive neurological symptoms in people with MS. Here we show that the exercise hormone irisin mediates neuroprotective effects of exercise in the experimental autoimmune encephalomyelitis (EAE) mouse model of MS. We demonstrate that voluntary free-wheel running exercise protects against inflammation-induced neurodegeneration in EAE, but these neuroprotective effects are abrogated in mice lacking Fndc5/irisin. Peripheral delivery of irisin increases irisin plasma levels and reduces both…

Psychological resilience as a behavioral proxy for cognitive reserve in multiple sclerosis: A theoretical reappraisal.

Psychological resilience has emerged as a robust protective factor in multiple sclerosis (MS), consistently associated with reduced depression, lower fear of relapse, and improved health-related quality of life. However, resilience in MS has been predominantly conceptualized as an intrapersonal psychological trait, limiting its explanatory scope for cognitive and neurological outcomes. In this theoretical manuscript, we propose reconceptualizing psychological resilience as a dynamic behavioral-psychological system that may function as a behavioral proxy for cognitive reserve. This proposal is …

Bcl-2 as a Double-edged Sword for the Treatment of Multiple Sclerosis: A Systematic Review.

INTRODUCTION: Multiple sclerosis (MS) is a chronic immune-mediated neurodegenerative disease that involves inflammation, demyelination, and neuronal loss. The B-cell lymphoma 2 (Bcl-2) protein family regulates intrinsic apoptotic pathways and may impact disease progression via effects on immune cell survival and neuroprotection. METHODS: To review current evidence on the role of Bcl-2 and its modulators in MS. Englishlanguage, clinical and preclinical studies (1998-October 2025) were retrieved from PubMed, Google Scholar, and other databases. RESULTS: Clinical studies show that peripheral B …

Analysis of the comprehension and familiarity among the Saudi Arabian population regarding multiple sclerosis.

INTRODUCTION: Multiple sclerosis (MS) is a chronic autoimmune disease affecting the central nervous system, characterized by immune-mediated demyelination and axonal degeneration. Its development is influenced by genetic susceptibility, immune dysregulation, and environmental factors such as viral infections, vitamin D deficiency, and smoking. MS affects over 2.3 million people globally, with prevalence varying by region, including an increasing trend in Saudi Arabia. Despite the growing burden, public awareness of MS in Saudi Arabia remains limited, contributing to delayed diagnosis and poore…

Reprogramming immunity: the promise and challenges of CAR T cell therapy in multiple sclerosis.

INTRODUCTION: Chimeric antigen receptor T-cell (CAR-T) therapy was initially used to treat B-cell malignancies, and it is now considered an effective treatment option for multiple sclerosis (MS). CAR-T therapy selectively targets and depletes pathogenic B cells within lymphoid tissue and the central nervous system (CNS), showing promise for achieving deep, sustained remission and long-term treatment-free disease control in patients with refractory MS. METHODOLOGY: A comprehensive analysis was carried out by searching multiple keywords with combinations such as "CAR-T", "MS", "Demyelination", …

The use of routinely collected structural neuroimaging to identify cognitive impairment in multiple sclerosis.

BACKGROUND: Cognitive impairment is common in multiple sclerosis (MS), yet comprehensive cognitive assessment is not universally accessible, and patients need to be triaged for referral. Given the links between brain atrophy and cognition, this study investigated whether routinely collected neuroimaging markers could identify MS patients at risk of cognitive impairment. METHODS: Data were retrospectively analysed from adult MS patients assessed in a specialist cognitive neuroimmunology clinic, who had undergone MRI within 12 months prior to cognitive testing. Normalised brain volume (NBV), no…

Oligodendrocyte Piezo2 is a regulator of age-dependent myelin integrity and dysregulated in multiple sclerosis.

Studies on the mechanosensitive ion channel Piezo2 largely focus on its role in the peripheral nervous system, particularly in touch and pai … stent with these findings, OL-encoded PIEZO2 expression is reduced in optic nerve lesion areas from multiple sclerosis patients, highlighting a convergent mechanism of small-caliber fiber vulnerability. Together, these data identify Piezo2 as an age-related regulator of OL function and myelin integrity, with potential relevance for preserving white matter structure …

Brain age gap in multiple sclerosis: associated with disability but independent of serum biomarkers.

BACKGROUND: Multiple sclerosis (MS) is influenced by age-related brain alterations and affects cellular aging mechanisms. Machine-learning models can estimate brain-predicted age from magnetic resonance imaging (MRI) to quantify these aging-related changes. OBJECTIVES: This study examines whether the difference between predicted and chronological age (BrainAGE) relates to clinical disability and biomarkers of neuro-axonal injury in MS. DESIGN: This study analyzed brain-predicted age from structural 3D T1-weighted MRI in 82 patients with relapsing MS enrolled in three prospective clinical tri…

Invisible symptoms in multiple sclerosis and their impact on social role participation: A multidimensional analysis.

BACKGROUND: People with multiple sclerosis (PwMS) frequently experience invisible symptoms, including cognitive impairment, fatigue, depression, anxiety, and psychosocial factors such as stigma, which can substantially affect social role participation (SRP). These factors are often under-recognized in clinical care despite their functional impact. OBJECTIVE: To examine the unique contributions of invisible MS-related symptoms to SRP after accounting for demographic characteristics and objective cognitive performance. METHODS: A cross-sectional sample of 434 PwMS was recruited from a large ou…

Circadian rhythm disruption and multiple sclerosis: The role of affective symptoms and temperamental traits.

Sleep and circadian rhythm disturbances are common clinical features in people with multiple sclerosis (PwMS) and related demyelinating conditions. Affective symptoms may shape vulnerability to circadian dysregulation. This pilot cross-sectional study examined how affective symptoms and underlying temperamental dispositions relate to circadian rhythm instability in a population of newly diagnosed PwMS and related demyelinating conditions, defined as patients evaluated within 30 days after discharge from the index hospitalization during which the diagnosis was established. Measures included Bec…

External validation of KFLC index threshold for multiple sclerosis diagnosis in a French cohort.

BACKGROUND: The kappa free light chains (KFLC) index was recently incorporated into the 2024 McDonald criteria as a practical alternative to oligoclonal bands (OCB) in multiple sclerosis (MS) diagnosis. Our objectives were to compare the diagnostic performance of KFLC index and OCB, evaluate the recommended KFLC index threshold of 6.1, and assess the correlation between KFLC index and disease activity. METHODS: We conducted a retrospective study on patients referred to the Lyon MS center who underwent lumbar puncture for OCB assessment from October 2019 to November 2023. KFLC index was measur…

Towards a biologically informed description of multiple sclerosis disease course - a roadmap for transition.

Contemporary management of multiple sclerosis is based on the clinical course descriptors of relapsing-remitting, secondary progressive and primary progressive. However, recognition is growing in the field that these clinical course descriptors do not capture the full disease spectrum of multiple sclerosis or reflect the underlying biological mechanisms of disease, and that biologically informed course descriptors are needed. In this Roadmap, participants in an International Advisory Committee on Clinical Trials in Multiple Sclerosis workshop discuss the problems with the existing course descr…

Increased education is associated with decreased odds of depression in females, not males, with multiple sclerosis.

BACKGROUND: Contrasting with general population trends, there is no sex difference in rates of depression among people with multiple sclerosis (pwMS). Although increased education is protective against depression in the general population, data are equivocal in pwMS. OBJECTIVE: This study sought to evaluate whether sex moderates the relationship between education and depression in pwMS. METHODS: In a consecutively assessed sample of 1711 pwMS at a Canadian neuropsychiatry clinic, data were gathered regarding educational years, sex, age, Expanded Disability Status Scale (EDSS) scores, MS subt…

Heart rate variability as a biomarker of autonomic (re)activity and fatigue in multiple sclerosis.

Fatigue is a debilitating symptom in multiple sclerosis (MS) associated with autonomic nervous system (ANS) dysfunction. Heart rate variability (HRV) provides a non-invasive measure of ANS function; yet the relative sensitivity of linear and nonlinear HRV parameters in capturing specific ANS (dys)function remains unclear. This study investigated the factorial structure of HRV parameters, their response to biopsychological interventions, and their relationship with MS-related fatigue. Data from two clinical trials (N = 78) were pooled. Exploratory factor analysis (EFA) was applied to time-domai…
