Failure of the interosseous membrane to heal with immobilization, pinning of the distal radioulnar joint, and bipolar radial head replacement in a case of Essex-Lopresti injury: case report.

The authors report a case of a patient with complex radial head and ulnar shaft fractures with an associated tear of the interosseous membrane, which failed to heal despite open reduction internal fixation of the ulna, a bipolar radial head replacement, and cross pinning of the distal radioulnar joint. This case demonstrates that failure of the interosseous membrane can occur even in a properly replaced bipolar radial head with adequate immobilization of the forearm and pinning of the distal radioulnar joint.

Low‑Energy Fall With High‑Impact Consequences: A Case Report and Biomechanical Literature Review of Ogawa Type I Coracoid Process Avulsion Fracture With Acromioclavicular Joint Dislocation.

Coracoid process fractures are rare injuries, usually associated with high-energy trauma and disruption of the superior shoulder suspensory complex (SSSC). We report the case of a 36-year-old male who sustained an Ogawa type I coracoid avulsion fracture with concomitant acromioclavicular (AC) joint dislocation following a low-energy mechanical fall. Initial radiographs demonstrated AC joint displacement with an associated coracoid fragment, further characterised by computed tomography (CT) with three-dimensional reconstruction. Imaging confirmed a displaced basal fracture involving the coracoc

Retroperitoneal Migration of a Malpositioned Nasogastric Tube Following Panfacial Fracture Surgery.

A 63-year-old man with panfacial fracture was transferred to our hospital. Open reduction and internal fixation of the fracture was performed, followed by nasogastric tube insertion under general anesthesia. Location of the tube was checked by epigastric auscultation and chest radiography intraoperatively. Although the tube was displaced to the left on the radiography, its position was deemed appropriate because the tip was located below the diaphragm. Postoperative computed tomography revealed that the tube had entered into the trachea, penetrated the lung and diaphragm, and migrated into the

En-Bloc separation of the distal humerus articular appendage.

The unique anatomy of the distal humerus predisposes it to coronal shear injuries of its articular appendages. These injuries often require an open reduction and internal fixation of articular fragments. The existing classification systems often overlook the depth of sub-chondral bone involvement. This case report discusses a rare variant of a distal humerus articular injury which involved an 'en-bloc separation of the anterior articular cartilage'. A young male in his 20s presented with a coronal shear fracture of the distal humerus, fixed using combined lateral and medial surgical approaches

Axial-Radial-Ulnar Carpal Dislocation with Divergent Intermetacarpal Dislocations, Peritrapezoid and Scaphotrapezial Dislocations: A Case Report.

We report a patient with a severe axial-radial-ulnar (ARU) fracture dislocation of the carpus, involving multiple intermetacarpal dislocations as well as divergent carpometacarpal dislocations involving the index, long, ring and small fingers and peritrapezoid and scaphotrapezial dislocations. She also had a degloving injury involving the dorsal hand. Emergent debridement followed by open reduction and internal fixation of all injuries was performed, followed by soft tissue management. At 6-year follow-up, the patient had adequate active range of motion with the ability to make a full fist and

Subtrochanteric fracture after core decompression for osteonecrosis of the femoral head: a case report and literature review.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) is a common clinical disease. Improper treatment can lead to femoral head collapse and hip joint dysfunction. Core decompression is particularly important for early ONFH. However, subtrochanteric fractures after core decompression cause some clinical problems. CASE PRESENTATION: This article describes a 34-year-old male patient with early ONFH. After core decompression, he suffered a subtrochanteric fracture of the femur while bearing weight on the affected limb when going up stairs. He was subsequently treated with open reduction and intra

The sequence of the treatment of combined fractures of the pelvis, spine, and extremities in polytraumatized patients.

'Damage control' is the therapeutic strategy in the treatment of polytraumatized patients and aims at securing vital functions and controlling bleeding with a favorable effect on the post-traumatic immune response. The post-traumatic immune dysfunction is based on a disturbed balance between immunostimulatory and anti-inflammatory mechanisms. The extent of the immunological 'second hit' can be limited by delaying deferable surgical therapies until organ stabilization has been achieved by the treating surgeon. Pelvic sling is easy to apply and noninvasive with effective pelvic reduction. Pelvic

Fracture-Dislocation Dorsal of the Proximal Interphalangeal Joint: A Case Report and Focus on Volar Plate Injuries.

The proximal interphalangeal (PIP) joint is the articulating joint between the proximal and middle phalanges of the fingers. A dorsal fracture-dislocation of the PIP joint of the fingers with volar plate injuries is an uncommon injury. Few cases have been published in the literature. In this article, we report the case of a subluxation fracture of the PIP joint in a 27-year-old male patient, without pathological history, a manual worker, right-handed, diagnosed 28 days after the injury. The treatment was surgical with open reduction and fixation of the fragment of the base of P2 with osteosutu

Cannulated cancellous screw fixation for the management of sustentaculum tali fracture: A case report.

INTRODUCTION: An isolated sustentaculum tali fracture is a rare manifestation of orthopaedic injury involving the supporting bone of the middle calcaneal facet. Globally, the incidence is estimated to be less than 1% of all calcaneus fractures. PRESENTATION OF CASE: We reported a 42-year-old man with persistent ankle pain and instability on the right foot following a week after a motorcycle accident. The Computed tomography evaluation showed an avulsion of the medial sustentaculum tali on the right calcaneus bone. An open reduction and internal fixation procedure was performed using two cannu

Treatment of dorsal fracture-dislocations of the proximal interphalangeal joint using the shotgun approach.

UNLABELLED: This study assessed the outcome of open reduction and internal fixation of proximal interphalangeal joint fracture-dislocations through a shotgun approach, while keeping hemi-hamate arthroplasty as a back-up plan. After using the shotgun approach, fixation was carried out when the anterior fragment was large enough to accept two 1.5 mm screws. Sixty-three fracture-dislocations were treated in 61 patients, of whom 30 underwent internal fixation. Twenty-one of these were assessed in the final study. At a mean follow-up of 29 months, the mean range of joint motion was 80° and 102° for

Proximal humerus and humeral shaft nonunions.

The rate of nonunion is estimated to be 1.1% to 10% following closed treatment of proximal humerus fracture and 5.5% following closed treatment of humeral shaft fracture. Surgical management should be considered for fractures that demonstrate no evidence of progressive healing on consecutive radiographs taken at least 6 to 8 weeks apart during the course of closed treatment. In the case of proximal humerus nonunion, recent series have demonstrated union in >90% of patients treated with reconstruction using locking plates and autogenous bone graft. Shoulder arthroplasty is reserved as a salvage

Anterior interosseous nerve palsy after supracondylar fracture of humerus in adult. Case report.

We present a case of anterior interosseous nerve palsy after a supracondylar fracture of the humerus in a 24-year-old woman. The patient was diagnosed and treated in two hospitals where the impairment of function of the hand indicating nerve damage was not noticed during a standard examination. An open reduction of the fracture with internal fixation using LCP plates and additional screws was performed, ultimately producing a good therapeutic outcome. Anterior interosseous nerve palsy was detected after 14 days during the first follow-up visit. The decision was made to institute conservative t

Interscalene block for shoulder surgery.

Fracture dislocation of the shoulder is a common musculoskeletal injury following road traffic accident. Peripheral nerve block has become a recognized anesthetic technique due to the rapid onset of prolonged analgesia, sufficient for both pain and surgical management. However, interscalene block for shoulder surgery has not been reported as a primary anesthetic technique in our environment. We report its successful use in open reduction and internal fixation of left humeral surgical neck fracture dislocation. The interscalene brachial plexus were localized by a Polystim II nerve stimulator (t

Iliopsoas bursitis-associated femoral neuropathy exacerbated after internal fixation of an intertrochanteric hip fracture in rheumatoid arthritis: a case report.

We present the case of a 63-year-old woman with a six-year history of rheumatoid arthritis (RA) and a left iliopsoas bursitis. Radiography had detected destructive changes in her hip joint associated with her bursitis, and she had reported some paresthesia along the left anterior distal thigh. Her pain and numbness remained tolerable, and her disease activity was well controlled until she accidentally fell on the floor, which resulted in an unstable intertrochanteric fracture of left femur with displacement of the proximal portion. The fracture was successfully treated with open reduction and

Bladder entrapment after external fixation of traumatic pubic diastasis: importance of follow-up computed tomography in establishing prompt diagnosis.

A 30-year-old male was an unrestrained driver in a high-speed motor vehicle crash. On presentation, the patient was profoundly hypotensive with multiple injuries, including a 20-cm-deep perineal laceration with avulsion of the rectum, a diffusely tender abdomen, an unstable open-book pelvic fracture, and multiple rib fractures. Blood noted at the urethral meatus prompted a retrograde urethrogram and cystogram, which were within normal limits. A Foley catheter was placed with the return of clear urine. Closed reduction and external fixation of the pelvic fracture were performed emergently witho

Technique for facilitating closed reduction of difficult flexion type supracondylar humeral fracture in children.

The displaced flexion type supracondylar humeral fractures (SCHF) are inherently unstable and there is great intraoperative difficulty in obtaining and maintaining the fracture reduction by closed means. We introduced a technique for closed reduction and K-wires pinning of displaced flexion type SCHF. Fourteen patients with flexion-type SCHF (9 boys and 5 girls) underwent a reduction technique using a construct of three K-wires. The proximal wire was used for rotational control of the proximal fragment and the two distal wires were used for correction of the flexion and rotational deformity of
