Relevant factors determining the management of inaccessible retained lead airgun pellets are discussed in relation to 11 orbital cases. Although the injuries were of between one month's and 26 years' standing, serum lead levels were normal at under 350 micrograms/l in all instances. This is attributable to the modern alloy composition of the ammunition in question. It is concluded that surgical intervention may be indicated only where there is a mechanical disturbance of ocular motility allied with good visual function.

We treated exposure of the orbital implant with a dermis fat graft in two patients with an acquired anophthalmic orbit. During the first six postoperative months, a large polypoid tumor of the conjunctiva developed over the grafted tissue in each patient. Histologic examination of both lesions disclosed a pyogenic granuloma.

We have reported a case of acute orbital pseudotumor, an idiopathic inflammatory process that usually responds dramatically to corticosteroids. Untreated, severe proptosis and eventual optic neuropathy may result. This article begins with a discussion of normal orbital anatomy and the terminology involved in the description of abnormalities of the orbit. Case reports of patients with neurofibromatosis or with a craniofacial anomaly are presented, and the characteristic orbital findings on both CT and MRI are reviewed. Thus the radiologist's role and the value of CT and MRI in the overall treatment of these often complex abnormalities are elucidated.

A 13-year-old black female was referred with unilateral granulomatous uveitis and orbital inflammation of sudden onset. Hematologic, serologic, bacteriologic, and ultrasonographic studies along with high resolution CT scans of the orbits were employed to determine the diagnosis and appropriate treatment. Exophthalmos with lowering of the globe, eyelid enlargement and canthal displacement are the usual consequences of orbitopalpebral neurofibromatosis. A cerebral hernia into the orbit may be seen on the CT scan. An intracranial approach allows for pushing back the brain with a bone graft to create an enlarged orbital cavity, lifting the globe with a graft on the orbital floor, and fixing the canthi in proper position. An eyelid radical resection is performed in the same stage or later. This approach is discussed in the treatment of 17 cases of orbito-palpebral neuro-fibromatosis.

We report the first case of sino-orbital mucormycosis caused by Rhizopus rhizopodiformis. Therapy consisted of conservative surgical drainage and intravenous amphotericin B. The patient, an insulin-dependent diabetic, survived her infection with minimal residual deformity. This case and a literature review suggest that infection due to R. rhizopodiformis in diabetics might be more amenable to treatment than mucormycosis caused by other species of fungi. Speciation of such fungi would permit the determination of species specific mortality rates, and may be of therapeutic and prognostic value.

Two cases of orbital myositis simulating infectious cellulitis were reported, in which the diagnosis was made by CT scans, biopsy and laboratory findings of thyroid function. In both cases, ophthalmoscopy revealed partial occlusion of the central retinal vein and choroidal folds due to posterior scleritis. Histopathology of a biopsy specimen revealed infiltration of lymphocytes, eosinophils and plasma cells and perivasculitis which suggested an allergic origin of this disease.

A case of subperiosteal abscess associated with acute sinusitis is reported. The case is remarkable in that anaerobic organisms played a major role in the pathologic process. In addition, the unusual superolateral position of the abscess in the absence of any defect in the bony sinus walls suggests the hematogenous spread of the sinus infection. The use of Ct scanning proved to be essential in determining the true nature of the orbital pathology.

An emplaced plastic plate was demonstrated by computed tomographic scanning five years after surgical implantation because of fracture of the orbital floor, proving that it had remained in proper position and that the patient's complaints were unrelated to its presence in the orbit. To the best of our knowledge, this is the first instance in which this method had been used intentionally to achieve noninvasive localization of an orbital plastic plate. It is a valuable and accurate adjunct in ascertaining possible migration or early extension of the emplaced implant.
