Ultrasound-Guided Radiofrequency Ablation of the Intercostal Nerve for Traumatic Rib Fracture Pain: A Case Report.

BACKGROUND: Pain arising from traumatic rib fracture contributes to significant patient morbidity and mortality. CASE REPORT: A 68-year-old man with multiple comorbidities presented to the emergency department with a left-sided multiple rib fracture (ribs 6-8) and altered mental status after an unwitnessed fall. The patient was started on a standard of care oral analgesic regime, but continued to deteriorate because of inadequate pain relief in his pulmonary function. Given his altered mental status and insufficient pain control, to facilitate pulmonary physiotherapy, we performed an early (d

Automated Nociceptive Withdrawal Reflex Measurements Reveal Normal Reflex Thresholds and Augmented Pain Ratings in Patients with Fibromyalgia.

The nociceptive withdrawal reflex (NWR) is used to probe spinal cord excitability in chronic pain states. Here, we used an automated and unbiased procedure for determining the NWR threshold and compared the reflex thresholds and corresponding pain ratings in a well-characterized cohort of fibromyalgia (n = 29) and matched healthy controls (n = 21). Surface electrical stimuli were delivered to the foot in a stepwise incremental and decremental manner. The surface electromyographic activity was recorded from the ipsilateral tibialis anterior muscle. Fibromyalgia patients reported significantly h

Warming lignocaine reduces the pain of injection during local anaesthetic eyelid surgery.

The injection of local anaesthetic solutions is frequently a painful and unpleasant experience for patients. A double-masked randomised controlled trial was performed to study the potential benefit of warming lignocaine during local anaesthetic minor surgical procedures on the eyelids. The pain of subcutaneous injection of 1.5 ml of 2% lignocaine at room temperature (cold) and body temperature (warm) was compared in 60 patients during the surgical incision of solitary meibomian cysts of one eyelid. Patients were randomly allocated to receive either warm or cold lignocaine. Pain was assessed su

[Diagnosis and treatment of atypical facial pain-a review.].

Atypical facial pain is a collective term used for otherwise unclassifiable pain syndromes of the face. It describes mostly unilateral lancinating and burning pain, which is constantly present but fluctuating in intensity and may be accompanied by dys-or paraesthesias over a period of several years. Women are more often affected than men. A strikingly large number of invasive investigative procedures are performed before the final diagnosis is established. Psychopathological abnormalities are frequent and do not consist solely in masked depression. To date there is no cure for atypical facial

Retrobulbar bupivacaine irrigation for postoperative pain after scleral buckling surgery. A prospective study.

The authors conducted a prospective, randomized, double-masked clinical trial to determine if retrobulbar irrigation with bupivacaine hydrochloride 0.75% (Marcaine) has an effect on postoperative pain after scleral buckling surgery. Fifty consecutive patients undergoing scleral buckling under general anesthesia were randomized to receive either bupivacaine or balanced salt solution as a retrobulbar irrigation at the end of their retinal detachment procedure. Of the 25 patients who received bupivacaine, only three (12%) required parenteral pain relief in the first 24 hours after surgery. This w

Lumbar spinal stenosis: results of surgical treatment.

One hundred forty patients with surgically treated lumbar stenosis were evaluated. There were 70 men and 70 women with an average age of 63 years. The average duration of preoperative leg pain and/or claudication symptoms was 16 months. The average follow-up time was 42 months (range, 12-116 months). Average leg pain improvement was 82%, and average back pain improvement was 71%. Thirteen patients (9%) had no leg improvement, 15 (11%) between 25-74% improvement, and 112 (80%), 75% or greater improvement. Ninety-four patients (67%) had 75% or greater back pain improvement, 29 patients (21%) bet

Role of Cheilectomy and Minimally Invasive Surgery Shortening Osteotomies for Hallux Rigidus Treatment.

Hallux rigidus is a degenerative condition of the first metatarsophalangeal joint, resulting in pain, walking or sports disability, and joint stiffness. When conservative treatment fails, surgical treatment is the option. The main minimally invasive surgery (MIS) procedures indicated are cheilectomy, metatarsal shortening osteotomies, phalanx procedures, and arthrodesis. Recently, some authors recommended adding an arthroscopy to help improve the outcomes of the cheilectomy. This study presented 2 MIS decompression osteotomies that can be included after cheiletomies.

Complications in nail surgery and how to avoid them.

Complications in nail surgery are not that frequent. Apart from complications that are common to every hand procedure, intense pain is the major issue with nail surgery. To prevent complications, good knowledge of anatomy and physiology is required to avoid misdiagnosis or inappropriate technique. However, some complications, such as postoperative nail dystrophy, are unavoidable, and must be known and discussed with the patient prior to the procedure in order to prevent disappointment that may lead to medico-legal cases. This paper will discuss the most frequent complications encountered.

Carotid Subclavian Bypass for the Treatment of Coronary Subclavian Steal Syndrome.

Coronary subclavian steal syndrome is an uncommon cause of angina in patients with a previous coronary artery bypass graft procedure. The patient had chest pain with the exertion of the left upper limb, difference in blood pressure between the left and right arm, occlusion at the ostium of the left subclavian artery. He underwent carotid subclavian bypass surgery that was successful in relieving symptoms. On the other hand, the patient had an embolic stroke related to the procedure and further assessment may be necessary.

Optimizing Outcomes in the Virgin Penile Implant Patient.

PURPOSE OF REVIEW: Patient satisfaction with inflatable penile prosthesis (IPP) is an elective procedure that restores erectile function. RECENT FINDINGS: While the return of usable erections is almost always achieved with IPP, patients may still be unhappy with the procedure if the outcome has not met their expectations. Optimizing patient satisfaction and outcomes should incorporate considerations for pre-, intra-, and postoperative factors. Length optimization regimens and improved pain control options have dominated the recent literature.

Spontaneous resolution of a gallstone ileus.

Gallstone ileus is an uncommon complication of cholelithiasis, usually associated with an internal biliary fistula. Management of gallstone ileus is surgical with enterolithotomy the procedure of choice, followed by fistula closure either as a one or two stage procedure. In this case a 66 year old female presented with colicky abdominal pain, computed tomography (CT) clearly showing a gallstone ileus and cholecystoduodenal fistula. Despite this the patient refused surgery and went on to have spontaneous resolution of the obstruction and passage of gallstones.

[Injection techniques in the cervical spine].

Spinal injections are a treatment option in the conservative management of degenerative spinal disorders. The indications must be critically reviewed for every patient. Treatment with injections for painful spinal disorders should be a part of interdisciplinary treatment regimes. The diagnostic value varies between injection procedures. A large number of imaging techniques are used to achieve a precise and safe needle placement in interventional procedures. The following article presents an overview of common fluoroscopy-guided spinal injection techniques for the cervical spine.

Female bladder catheterisation: step by step.

Catheterisation of the female urinary bladder is performed by midwives for a range of reasons. This article outlines and the main reasons for this procedure, including during labour and after the birth. It describes the equipment needed and the procedure undertaken, summarising the aseptic technique required to minimise the risk of urinary tract infection. The use of anaesthetic gels to minimise pain and trauma is considered. The article highlights the potentially embarrassing nature of catheterisation for women and urges midwives to perform this skill with maximum dexterity and minimum fuss.

First metatarsophalangeal arthrodesis for hallux rigidus.

Hallux rigidus leads to a restricted and painful motion at the first metatarsophalangeal (MTP 1) joint. Decision making of the appropriate surgical procedure mainly refers to the stage of hallux rigidus. If conservative measures fail, operative procedures can be taken into consideration. Arthrodesis of the MTP 1 joint is widely accepted as the gold standard for end-stage hallux rigidus. Despite the fusion of a key joint, there is little adverse effect on gait, and weight bearing of the first ray can be restored.

Complications of limited and total wrist arthrodesis.

Partial and total wrist arthrodeses have become common procedures for treating degenerative diseases arising from numerous conditions, including posttraumatic arthrosis, intercalated segment instability, inflammatory arthropathy, and carpal osteonecrosis. The goals of these two procedures are to provide pain relief and improved function by fusing arthritic or unstable joints. A thorough understanding of the complications and the best practices to avoid them is critical for the surgeon in the preoperative, intraoperative, and postoperative management of candidates for total or limited wrist art

Anesthesia.

Anesthetic agents are used in laboratory animals to prevent pain or distress due to an experimental procedure or for restraint to facilitate a technically difficult procedure. This unit provides three basic protocols: injectable anesthesia for mouse, rat, and hamster; inhalant anesthesia using methoxyflurane for mouse, rat, and hamster; and injectable anesthesia using ketamine/xylazine for rabbit. An Alternate Protocol describes sedation using butorphanol/acetylpromazine in the rabbit. The Commentary further describes and compares these methods of anesthesia for various applications.

Thoracic surgery: the irreversible evolution toward endoscopic surgery.

Endoscopic techniques have established themselves as an important means for reducing the traumatic impact of thoracic surgery on the patient. They considerably lighten postoperative pain and are cosmetically much more pleasing. Procedures such as lung biopsy, pericardial window, treatment of spontaneous pneumothorax, mediastinal benign tumour excision and sympathectomy have become routine, other such as oesophagectomy require further evaluation. Advanced procedures such as lobectomy and pneumonectomy require further clinical and technological development prior to a conclusive assessment.

Early and eighteen month clinical outcomes of first UK case of percutaneous deep vein arterialisation (pDVA) to treat "no option" chronic limb-threatening ischemia using the LimFlow system.

BACKGROUND: Chronic limb-threatening ischaemia (CLTI) in cases where there are no further standard treatment options for limb salvage represents the most advanced stage of peripheral arterial disease. For these "no-option" CLTI patients, an experimental treatment of foot vein arterialisation (FVA) was first described in 1912, however, it was never widely adopted as outcomes varied significantly most likely due to the complexity of the surgical intervention and lack of standardisation. In recent years there have been significant developments in performing FVA fully percutaneously and standardis

Office hysteroscopic morcellation service: Evaluation of women experience and factors affecting satisfaction.

OBJECTIVES: To evaluate the hysteroscopic morcellation service in office-setting in everyday practice outside the context of clinical trials. The primary objectives were to assess level of acceptability and factors affecting women satisfaction. The secondary objectives included assessment of complete resection rate, complications rate, pain score during the procedure and on discharge, and the correlation between the lesion size as subjectively estimated by the hysteroscopists versus the volume of morcellated tissues as semi-quantitively measured by the laboratory. METHOD: The clinical data wa

Long-term results after surgery for chronic pancreatitis.

AIM: To determine the early and late morbidity and mortality after surgical treatment of chronic pancreatitis. METHODS: We determined long-term outcome and early and late morbidity and mortality, respectively, in 484 consecutive patients undergoing surgery for chronic pancreatitis from 1976 through 1997. Sixty-five percent of the patients had small duct disease (main pancreatic duct <7 mm), whereas 35% had large duct disease. Indications for operation were pain (95%), suspicion of malignancy (28%), and complications involving adjacent organs (35%). Pseudocysts were present in 27% of patients.
