Drivers with obstructive sleep apnea syndrome (OSAS) have an increased risk of motor vehicle crashes. Unfortunately, neither clinical nor polysomnographic features allow clinicians to reliably identify high-risk drivers. One potential means of identifying these drivers is with the use of driving simulators. Several investigators have shown that OSAS patients perform worse than healthy control drivers and results from our studies have demonstrated declines in driving performance during EEG-defined "microsleeps." The use of simulators, and in-vehicle detection and alerting devices may mitigate s…

Simple sleep-related movement disorders must be distinguished from daytime movement disorders that persist during sleep, sleep-related epilepsy, and parasomnias, which are generally characterized by activity that appears to be simultaneously complex, goal-directed, and purposeful but is outside the conscious awareness of the patient and, therefore, inappropriate. Once it is determined that the patient has a simple sleep-related movement disorder, the part of the body affected by the movement and the age of the patient give clues as to which sleep-related movement disorder is present. In some c…

The hypocretins (also known as orexins) and their receptors are the focus of many investigators as sites for therapeutic intervention in a number of endocrinologic, neurologic and sleep disorders. The interest for the hypocretin system is highlighted by a recent discovery that a human sleep disorder, narcolepsy, is tightly linked with the deficiency of hypocretin peptides. This finding suggests that hypocretin replacement is a promising new therapeutic intervention for human narcolepsy and related disorders, but this will only become possible when small-molecule (i.e., non-peptide) hypocretin …

Sleep is an active and regulated process with restorative functions for physical and mental conditions. Based on recordings of brain waves and the analysis of characteristic patterns and waveforms it is possible to distinguish wakefulness and five sleep stages. Sleep and the sleep stages modulate autonomous nervous system functions such as body temperature, respiration, blood pressure, and heart rate. These functions consist of a sympathetic tone usually related to activation and to parasympathetic (or vagal) tone usually related to inhibition. Methods of statistical physics are used to analyz…

Taste disorder is a rare complication of uvulopalatopharyngoplasty, and may have a significant impact on quality of life. Herein, we report a case of obstructive sleep apnea syndrome in a 51-year-old man who experienced taste disturbance after palatopharyngeal surgery using electrocautery for developing a uvulopalatal flap. Gustatory function test using three-drop-method with solutions of highest concentration was implemented to assess the deficiency of four basic tastes. The results showed deficit of sweet taste associated with phantom of bitter taste. The patient reported constant spontaneou…

Novel findings on the effects of inflammatory molecules on neuronal circuits, and on molecular interactions between immunity and sleep, in health and disease, shed light on the pathogenesis of disorders of past (encephalitis lethargica) and present concern (human African trypanosomiasis and narcolepsy), which share alterations in sleep-wakefulness transitions. Although these three disorders differ in etiology, synaptic interactions with immune-response-derived molecules could play a pathogenetic role. Knowledge obtained on neural-immune interplay during senescence also has implications for age…

The objectives were to explore the association between self-reported adverse childhood experiences (ACE) and sleep in adults suffering from primary insomnia and to examine the impact of presleep stress on this relationship. Fifty-nine patients with primary insomnia, aged 21-55 years, were administered the Childhood Trauma Questionnaire (CTQ) and then divided into two groups according to the achieved scores: with moderate/severe or low/no reports of ACE. The participants spent three consecutive nights in the sleep laboratory in order to record polysomnographic and actigraphic sleep parameters. …

This is a short review paper presenting hypothesis to explain the mechanism that may be involved in the genesis of sleep bruxism (SB). In humans, SB is a repetitive sleep movement disorder mainly characterized by rhythmic masticatory muscle activity (RMMA) at a frequency of 1Hz and by occasional tooth grinding. Until recently, the mechanism by which RMMA and SB episodes are triggered has been poorly understood. It is reported that during light sleep, most SB episodes are observed in relation to brief cardiac and brain reactivations (3-15s) termed "micro-arousals". We showed that RMMA are secon…

INTRODUCTION: Although substantial research has been completed on the effects of sleep deprivation on performance, very little research has focused on language-based tasks. The purpose of the current study was two-fold: 1) to determine the extent to which short-term sleep deprivation affects language performance; and 2) to examine whether relatively short and easy-to-administer "probe" tasks could signal decrements in language performance under sleep deprivation conditions.
METHODS: There were 38 non-native English-speaking students who were paid to complete a 28-h sleep deprivation study. Th…

Obstructive sleep apnea syndrome (OSAS) is a common disease characterized by repetitive partial or complete closure of the upper airway during sleep. Cardiovascular disturbances are the most important complications responsible for increased morbidity and mortality. It is suggested that daytime somnolence, chronic fatigue, and nocturnal hypoxemia may further impair muscle function and decrease exercise fitness. The aim of this study was to evaluate cardiopulmonary response to exercise in OSAS patients. One hundred and eleven middle aged (50.2+/-10 yr), obese (BMI 31.0+/-4.6 kg/m2) patients (109…

Over the last decade the pathophysiology of obstructive sleep apnea (OSA)-related cardiovascular disease has been further elucidated in animal models employing dogs and rats/mice. It was demonstrated that, under the conditions of chronic intermittent hypoxia (CIH), endothelial dysfunction, i. e., a reduction in endothelial-dependent vasorelaxation, occurs. Furthermore, animals were shown to develop arterial hypertension when subjected to CIH for some weeks. Other aspects of the cardiovascular morbidity linked to OSA such as pulmonary hypertension, heart failure and atherosclerosis were also fo…

Obstructive sleep apnea (OSA) is a recognized cause of secondary hypertension. OSA episodes produce surges in systolic and diastolic pressure that keep mean blood pressure levels elevated at night. In many patients, blood pressure remains elevated during the daytime, when breathing is normal. Contributors to this diurnal pattern of hypertension include sympathetic nervous system overactivity and alterations in vascular function and structure caused by oxidant stress and inflammation. Treatment of OSA with nasal continuous positive airway pressure (CPAP) abolishes apneas, thereby preventing int…

STUDY OBJECTIVES: The interaction of sensory physiology and sleep has been studied for various sensory systems. Nevertheless, the question whether chemosensory (especially olfactory) stimuli may lead to arousals during sleep remains under discussion. Specifically, the central processing of olfactory information shows fundamental differences compared to other sensory systems.
DESIGN: Prospective controlled trial.
SETTING: Sleep research facility, University Hospital.
PARTICIPANTS: Five young healthy, normosmic volunteers.
INTERVENTION: Intranasal chemosensory stimulation during sleep was ba…

INTRODUCTION: Nocturnal sleep-related eating disorder is a non-REM sleep parasomnia that is associated to other sleep disorders, especially sleepwalking. It becomes chronic, is not remitting and consists in episodes of compulsive eating during the night, which are then partially or completely forgotten by the patient. This condition must be differentiated from night-eating syndrome, which is far more common and is linked to endocrinological and psychiatric disorders, as well as to other disorders involving eating behaviour during sleeping hours.
CASE REPORT: A 28-year-old male who had suffere…
