Results of tenorrhaphy with early rehabilitation for acute tear of Achilles tendon.

BACKGROUND: For the treatment of acute Achilles tendon rupture, early rehabilitation after surgical repair is believed to be a useful method. The purpose of this study was to evaluate the outcome of open tenorrhaphy followed by early rehabilitation. METHODS: A total of 112 patients who underwent open repair and early rehabilitation for acute Achilles tendon rupture from January 2003 to March 2015 were retrospectively reviewed. These 112 patients (80 men, 32 women) had an average age of 43.1 years (22-62 years), and the mean follow-up period was 91.8 months (12-171 months). A short leg cast wa

Ultrasonographic diagnostics of pain in the lateral cubital compartment and proximal forearm.

Pain in the lateral compartment of the elbow joint and decreased strength of the extensor muscle constitute a fairly common clinical problem. These symptoms, occurring in such movements as inverting and converting the forearm, pushing, lifting and pulling, mostly affect people who carry out daily activities with an intense use of wrist, e.g. work on computer. Strains in this area often result from persistent overload and degeneration processes of the common extensor tendon and the radial collateral ligament. Similar symptoms resulting from the compression of deep branch of the radial nerve in

The role of forensic imaging in the allegations of torture in asylum seekers.

Nowadays, according to the Istanbul Protocol, diagnostic tests are not an essential part of the clinical assessment of a person alleging torture or ill-treatment. In many cases, a medical history and physical examination are sufficient. However, literature evidence suggests a helpful role of diagnostic imaging in the allegation of evidence of torture, especially in the case of musculoskeletal injuries. The purpose of this narrative review is to highlight the role of imaging tests in suspected torture victims, emphasizing the role of these methods in establishing legal evidence of mistreatment.

Does endoscopic piriformis tenotomy provide safe and complete tendon release? A cadaver study.

BACKGROUND: Endoscopic piriformis release (EPR) is among the available treatments for piriformis syndrome. This procedure typically involves dividing the muscle near the sciatic nerve in the sub-gluteal space, which contains numerous blood vessels and nerves. The objectives of this prospective cadaver study were: 1) to assess the reproducibility and quality of endoscopic piriformis tenotomy near the greater trochanter; 2) to detect iatrogenic injuries to the lateral hip rotators, nerves, and vessels; 3) and to define the surgical safety margins relative to the sciatic nerve and inferior glutea

Is MRI adequate to detect lesions in patients with ankle instability?

BACKGROUND: Chondral lesions, peroneal tendon tears, and other disorders in patients with chronic ankle instability may not be detected by preoperative MRI. Also, MRI often is obtained and interpreted at the referring institution, leading to variability in reading. QUESTIONS/PURPOSES: We assessed the accuracy of the radiologists' and orthopaedic surgeon's reading of preoperative MRI for diagnosing ankle lesions in patients with ankle instability warranting surgery. PATIENTS AND METHODS: We retrospectively reviewed 133 patients who underwent 135 surgeries for lateral ankle ligament reconstruc

Shoulder Reconstruction for Brachial Plexus Birth Injuries: An In-Depth Review and Case-Based Update.

Brachial plexus birth injuries can result in significant shoulder dysfunction with limitations in range of motion, decreased strength, and risk of glenohumeral joint deformity. This comprehensive review examines current approaches for management of the shoulder including surgical reconstruction following these injuries. Serial clinical exams and selective imaging are critical to determine optimal timing of surgery based on recovery potential and joint pathology. Biceps recovery is monitored monthly from birth and, if absent by 5-6 months, serves as an indication for nerve reconstruction with n

Comparative Effectiveness of Operative and Nonoperative Management Strategies for Acute Achilles Tendon Rupture: A Narrative Review.

Acute Achilles tendon ruptures (AATRs), an extremely prevalent injury amongst adults who remain active, have been treated with surgical intervention for decades. This choice was made due to several decades-old studies demonstrating reduced re-rupture rates as opposed to nonsurgical interventions utilizing casts; however, numerous surgeons and researchers acknowledged potential complications such as wound problems and nerve damage. Functional braces and early weight-bearing now challenge this 'surgical by default' model. An evidence-based, focused review of randomized clinical trials, conventio

Jumper's knee risk in elite volleyball: impact of training volume, sex, and position.

BACKGROUND: Jumper's knee, also known as patellar tendinopathy, is a common overuse injury among athletes involved in sports that require repetitive jumping and high-impact activities, such as volleyball. Understanding the risk factors contributing to this injury is essential for implementing effective prevention strategies. The aim of this study was to clinically and ultrasonographically evaluate the patellar tendons of senior volleyball players at national division one in Portugal (>18 years old) and investigate the relationship between weekly training volume (in hours), sex and players' pos

Extensor Pollicis Longus Entrapment on CT in Acute Distal Radius Fracture May Be a Predictor of Tendon Rupture.

BACKGROUND: Extensor pollicis longus (EPL) rupture is a well-described complication after volar plate fixation of distal radius fractures. Although protrusion of screw tips through the dorsal cortex of the distal radius is likely the cause of a substantial proportion of EPL ruptures, it is probably not the sole cause for EPL rupture after distal radius fracture. The purpose of this study was to evaluate preoperative computed tomography (CT) scans of distal radius fractures to characterize the incidence of EPL damage at the time of injury. METHODS: This retrospective study included adults with

Gastric pentadecapeptide body protection compound BPC 157 and its role in accelerating musculoskeletal soft tissue healing.

There is a current need for a therapy that can alleviate the social and economic burden that presents itself with debilitating and recurring musculoskeletal soft tissue injuries and disorders. Currently, several therapies are emerging and undergoing trials in animal models; these focus on the manipulation and administration of several growth factors implicated with healing. However, limitations include in vivo instability, reliance on biocompatible and robust carriers and restricted application procedures (local and direct). The aim of this paper is therefore to critically review the current l

Peroneal tendinosis as a predisposing factor for the acute lateral ankle sprain in runners.

PURPOSE: A painful episode in the region of the peroneal tendons, within the retromalleolar groove, is a common precipitating event of an acute lateral ankle sprain. A forefoot striking pattern is suspected to cause peroneal tendinosis. The aim of this study is to analyse the role of peroneal tendinosis as a predisposing factor for ankle sprain trauma in runners. METHODS: Fifty-eight runners who had experienced acute ankle sprain trauma, with pre-existing pain episodes for up to 4 weeks in the region of the peroneal tendons, were assessed clinically. Fractures were excluded by conventional ra

Rotator cuff injury as a complication of portal placement for superior labrum anterior-posterior repair.

BACKGROUND: An accessory trans-rotator cuff portal is commonly used in shoulder arthroscopy, primarily in the repair of SLAP (superior labrum anterior-posterior) lesions. Improper placement of the trans-rotator cuff portal can result in damage to the rotator cuff near its attachment site. METHODS: Six patients were studied, having been referred to our clinic after previous shoulder arthroscopy with SLAP repair. Review of operative notes showed that the rotator cuff had been described as normal in 5 patients and having a mild partial-thickness tear of the supraspinatus in 1 patient at the time

Patellar Tendon Ruptures after Total Knee Arthroplasty.

Patellar tendon (PT) rupture following total knee arthroplasty (TKA) is a rare, but devastating complication. These injuries occur most frequently in the acute period following TKA due to trauma to the knee. PT ruptures that disrupt the extensor mechanism create a marked functional deficit, impacting every facet of daily life. In complete ruptures of the PT, repair or reconstruction is typically indicated; however, complication rates following intervention remain high. Operative intervention remains the mainstay of treatment, with only certain specific situations where nonoperative interventio

Tendon transfers for the management of irreparable subscapularis tears.

Rotator cuff pathology is the main cause of shoulder pain and dysfunction in older adults. When a rotator cuff tear involves the subscapularis tendon, the symptoms are usually more severe and the prognosis after surgery must be guarded. Isolated subscapularis tears represent 18% of all rotator cuff tears and arthroscopic repair is a good alternative primary treatment. However, when the tendon is deemed irreparable, tendon transfers are the only option for younger or high-functioning patients. The aim of this review is to describe the indications, biomechanical principles, and outcomes which ha

Editorial Commentary: Shoulder Long Head of the Biceps Tendon Transposition Without Tenotomy May Be a Beneficial Augment for Massive Rotator Cuff Tear Repair.

Although many surgeons discard the long head of the biceps (LHB) tendon during arthroscopic rotator cuff repairs, several authors have argued that the LHB tendon may serve as a biologic and structural augment for massive repairs. There are multiple benefits of incorporating the LHB tendon autograft in difficult-to-repair massive rotator cuff tears. First, by maintaining the glenoid origin, the tendon likely retains a significant native blood supply, as well as tenocytes, which may support the healing process. In addition, there are several biomechanical benefits. The LHB may act as a dynamic h
