Development of Late Continence in Bladder Exstrophy and Epispadias Patients.

OBJECTIVE: To evaluate to what extend urinary continence develops during puberty in patients with classic bladder exstrophy (CBE) and epispadias. METHODS: The operative database was reviewed for surgical procedures and urinary continence in all 65 CBE and epispadias patients born in 1976-2005. Excluded were 2 patients who had insufficient data for evaluation. RESULTS: Thirty-five patients with volitional voiding were incontinent at the age of 10 years, 27 had daily incontinence, and 8 had rare incontinence. Sixteen patients (46%) became later fully continent without any major surgery (except

Sexual Function After Prolapse Repair and Retropubic Versus Single-incision Sling.

IMPORTANCE: Studies have shown that sexual function improves after surgical correction for stress urinary incontinence and pelvic organ prolapse. Current data suggest no significant difference in sexual function between transobturator and retropubic slings. However, there have been conflicting data regarding the newer, single-incision slings. OBJECTIVE: The objective was to evaluate sexual function after placement of single-incision versus retropubic slings for management of stress incontinence among patients undergoing concomitant native tissue vaginal repair. STUDY DESIGN: We performed a s…

Incontinence and voiding difficulties associated with prolapse.

PURPOSE: Prolapse is the protrusion of a pelvic organ beyond its normal anatomical confines. It represents the failure of fibromuscular supports. MATERIALS AND METHODS: A MEDLINE search was done using the keywords cystocele, uterine prolapse, vault prolapse, enterocele or rectocele in combination with urinary incontinence. We reviewed 97 articles. From this material the definition, classification, incidence, symptoms and evaluation are described. RESULTS: Prolapse and urinary incontinence often occur concomitantly and cystocele, rectocele, enterocele, uterine descent or vaginal vault prolaps…

Bone anchor 4-corner cystourethropexy: long-term results.

PURPOSE: We evaluate the long-term results of incision less transvaginal bone anchor cystourethropexy to treat genuine urinary stress incontinence. MATERIALS AND METHODS: Between August 1995 and January 1997, 31 women 36 to 81 years old (mean age 58) with types I and II genuine urinary stress incontinence were treated. Four miniature bone anchors (2 on each side of the urethra) attached to a suture were fired transvaginally into the retropubic bone using a bone anchor stapler. The ipsilateral sutures were tied, thus creating a Marshall-Marchetti colposuspension. RESULTS: Of the patients 3 we

Overactive bladder: evaluation and management in primary care.

Primary care physicians must initiate a discussion of overactive bladder and urinary incontinence with their patients who are at risk. A stepwise approach to evaluation and diagnosis and the use of systematic evaluation and treatment algorithms suitable to the primary care setting will improve identification and effective management of the incontinent patient.

Efficacy of tension-free vaginal tape with other pelvic reconstructive surgery.

OBJECTIVE: The tension-free vaginal tape procedure has been shown in several studies to be a safe and effective method of correcting stress urinary incontinence, but its efficacy when combined with other pelvic surgery has not been examined. STUDY DESIGN: Thirty-seven women with stress or mixed urinary incontinence and other pelvic floor defects underwent the tension-free vaginal tape procedure in conjunction with other pelvic floor reconstructive surgery. Preoperative and postoperative voiding diaries, standing stress test results, and patient satisfaction were evaluated. Patient outcome, op…

The sacrospinous vaginal vault suspension: Critical analysis of outcomes.

One hundred and three women with a preoperative diagnosis of a pelvic support defect underwent right sacrospinous fixation of the vaginal apex. The procedure was performed either therapeutically (in 63 subjects with vaginal vault eversion) or prophylactically (40 patients with severe uterovaginal prolapse), and was associated with other reconstructive procedures to repair the coexisting cystocele, enterocele or rectocele. Preoperative and postoperative assessments of each vaginal site were compared and the results in the cure of stress urinary incontinence, if present, were evaluated with rega…

[Pelvic and perineal sequelae of delivery].

Vaginal delivery especially with dystocia, may result in relaxation or disruption of fascial and ligamentous supports of pelvic organs. The relationship between first childbirth and obstetric trauma is strong but additional pregnancies and deliveries are aggravating factors as well as ageing and hormonal effects of the menopause. These anatomic changes are contributing to the development of stress urinary incontinence, anal incontinence and genital prolapse. Preventing obstetric trauma needs changes in current obstetric practice: reduction in the episiotomy rate, use of vacuum extractor in pre…

The treatment of autonomic dysfunction.

Autonomic dysfunction is responsible for much of the morbidity associated with frequently encountered neurological disorders, such as Parkinson's disease, multiple sclerosis, cerebrovascular disease, and peripheral neuropathies, as well as with the rarer primary autonomic nervous system degenerations. We review the treatment of those aspects of autonomic dysfunction that often present to the neurologist, including orthostatic hypotension, urinary incontinence and retention, and bowel dysmotility syndromes. Pathophysiology is discussed in each instance as it relates to a rational approach to th…

Urodynamic studies in patients undergoing bladder replacement surgery.

A high incidence of nocturnal incontinence has been a problem in patients undergoing continent urinary diversion when intact bowel segments are used. Detubularization has been advocated to solve this problem. Fifteen patients underwent continent urinary diversions and 4 underwent bladder augmentation with a variety of intestinal segments. Detubularization of the right colon anastomosed to the urethra was used in the first 3 patients, all of whom experienced nocturnal incontinence. Urodynamic studies showed high pressure contractions of the intestinal pouch of 60 to 100 cm. water pressure after

Pubovaginal sling procedure for stress incontinence.

Urinary stress incontinence associated with poor urethral sphincter function and indicated by a urethral pressure of less than 10 cm. water was treated in 52 cases with a pubovaginal autogenous fascial sling. No urethral sphincter function could be measured in 7 patients. Of these 52 patients 42 had undergone a previous operation for stress incontinence. The uninhibited detrusor dysfunction that accompanied the stress incontinence in 29 cases ceased after operation in 20 but persisted in 9. Postoperative urethral pressure measurements indicated that while the sling increased urethral pressure

Urodynamic changes after endoscopic correction of vesicoureteral reflux.

Massive bilateral vesicoureteral reflux (VUR) in a 7-year-old girl with spinal scoliosis was successfully treated by endoscopic correction. She was admitted due to a febrile urinary tract infection and urinary incontinence. A cystometrogram demonstrated normal detrusor function during storage. The endoscopic subureteric injection of polytetrafluoroethylene (Teflon) was performed, resulting in the disappearance of the VUR. A postoperative cystometrogram demonstrated overactive detrusor function during storage, necessitating anticholinergic medication. She has been free of febrile urinary tract …

Correlation between urinary symptoms and urodynamic findings: Is the bladder an unreliable witness?

OBJECTIVE: The role of urodynamics as the gold standard to investigate bladder function has recently been questioned. We aimed to evaluate the agreement of lower urinary tract symptoms and urodynamic diagnosis and to build predictive models. STUDY DESIGN: Patients who underwent urodynamics for pelvic floor disorders between 2008 and 2016 were retrospectively analyzed. Clinical evaluation investigated the presence of genital prolapse, stress urinary incontinence (SUI), overactive bladder (OAB), urge urinary incontinence (UUI), voiding symptoms (VS), and bulging symptoms. The degree of concorda…

Pediatric Lower Urinary Tract Dysfunction: A Comprehensive Exploration of Clinical Implications and Diagnostic Strategies.

Lower urinary tract dysfunction is clinically important because it may cause urinary tract infections, mainly due to accumulation of residual urine, and adversely affect renal function. In addition, it may cause urinary incontinence, strongly affecting the child's quality of life. The function of the lower urinary tract is closely associated with function of the bowel because constipation is commonly present with bladder dysfunction. The interplay between the lower urinary tract and bowel function, coupled with common conditions such as detrusor overactivity and voiding dysfunction, requires a…

Physical and sexual abuse in patients with overactive bladder: is there an association?

The known sequelae of sexual abuse include acute and chronic injury. The purpose of this study was to evaluate the association of overactive bladder symptoms (OABs) with a history of physical or sexual abuse. Two hundred and forty-three women who attended the gynaecological out-patient clinic or the urogynaecological clinic were recruited for our study. Based on their clinical examination, they were assigned to three groups of patients with either OAB or with stress urinary incontinence (SUI) without concomitant urgency symptoms (SUI), or without history of incontinence (control group). Afterw…

Anterior urethropexy according to lapides: how effective is it in the treatment of genuine female stress incontinence?

OBJECTIVES: Anterior urethropexy, introduced by Lapides, is a retropublic suspension procedure in which the anterior urethral wall is secured to the posterior surface of the symphysis pubis by full thickness trans-urethral sutures. Little information exists in the literature about its effectiveness and morbidity as a surgical treatment option for female stress incontinence. In this study, the outcome of this technique in the treatment of anatomic genuine stress urinary incontinence has been evaluated. METHODS: Thirty women with urodynamically-proven anatomic stress urinary incontinence underw…
