Unexpected Prolonged Apnea Following Succinylcholine in a Medically Complex Patient: A Case Report.

Prolonged apnea after succinylcholine should prompt consideration of impaired drug metabolism, including reduced butyrylcholinesterase activity, while other causes of delayed emergence are evaluated. We report a 69-year-old, 72.7-kg man with severe systemic illness and hypoalbuminemia who underwent esophagogastroduodenoscopy and colonoscopy under total intravenous general anesthesia. Induction included propofol, fentanyl, and succinylcholine 100 mg. Approximately two hours after the initial succinylcholine dose, the patient was extubated but developed profound ventilatory weakness, with a resp

Therapeutic Dilemma in Epidural Catheter Removal During Dual Antiplatelet Therapy and Anticoagulation Following Hepatic Artery Injury: A Case Report.

Epidural catheter removal in the setting of concurrent dual antiplatelet therapy (DAPT) and anticoagulation poses a high risk of neuraxial hematoma. Current guidelines mandate prolonged interruption of antithrombotic therapy, which may be unfeasible in patients with recently implanted arterial stents at high risk of thrombosis. We present the case of a 56-year-old man who underwent combined epidural and general anesthesia for a partial hepatectomy. An inadvertent intraoperative injury to the proper hepatic artery required aborting the surgery and deployment of a rescue drug-eluting stent. Intr

Role of Therapeutic Plasma Exchange in Severe Hypertriglyceridemia-Induced Acute Pancreatitis Associated With Preeclampsia: A Case Report.

Acute pancreatitis during pregnancy is an uncommon but serious condition associated with significant maternal and fetal morbidity. Hypertriglyceridemia-induced acute pancreatitis (HTG-AP) is a severe etiology that presents unique diagnostic and management challenges, particularly when complicated by preeclampsia. Diagnosis may be difficult because clinical manifestations can overlap with those of other obstetric emergencies. We report the case of a 30-year-old woman at 33 weeks' gestation with a history of type 2 diabetes mellitus and chronic hypertension who presented with severe abdominal pa

Comparative effectiveness of dexmedetomidine and meperidine for the treatment of postanesthetic shivering: a propensity score-matched retrospective cohort study.

BACKGROUND: Postanesthetic shivering (PAS) is a common and distressing complication in the post-anesthesia care unit (PACU). Meperidine has traditionally been used for PAS treatment, but interest in non-opioid alternatives has increased. We compared the effectiveness and safety of dexmedetomidine versus meperidine for the treatment of PAS. METHODS: This retrospective cohort study included 484 adult patients who developed PAS in the PACU after surgery between January and December 2024 and received intravenous dexmedetomidine or meperidine as the initial treatment. The primary outcome was treat

Perioperative Airway Management in Treacher Collins Syndrome With Obstructive Sleep Apnea: A Case Report.

BACKGROUND Treacher Collins syndrome (TCS) is a congenital disorder resulting from maldevelopment of the first and second pharyngeal arches. A major high-risk comorbidity is obstructive sleep apnea (OSA), characterized by recurrent upper airway collapse during sleep, leading to intermittent hypoxia and sleep fragmentation. This condition requires a tailored anesthetic strategy due to an anticipated difficult airway during intubation and increased vulnerability to postoperative respiratory failure. CASE REPORT A 28-year-old man with TCS and polysomnography-confirmed mild OSA underwent orthognat

Efficacy and safety of preoperative intranasal dexmedetomidine in adults: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Airway manipulation during General Anesthesia (GA) can produce sympathetic activation and hemodynamic instability. Intravenous (IV) dexmedetomidine attenuates these responses but may cause bradycardia or hypotension. Intranasal (IN) administration is a noninvasive alternative with uncertain evidence in adults. We evaluated preoperative IN dexmedetomidine in adult patients undergoing GA. METHODS: We performed a PROSPERO-registered (CRD420251250492) systematic review and meta-analysis of randomized trials. MEDLINE, Embase, CENTRAL, and CDSR were searched from inception to November 2

Multisystemic Anesthetic Management and Patient Blood Management in Urgent Liver Transplantation for Acute-on-Chronic Liver Failure: A Case Report.

BACKGROUND: Acute-on-chronic liver failure (ACLF) is associated with high short-term mortality and frequently requires urgent liver transplantation. Perioperative management is challenging due to severe hemodynamic instability, coagulopathy, systemic inflammation, and multiorgan dysfunction. Optimized anesthetic strategies combined with Patient Blood Management (PBM) principles may improve outcomes and reduce transfusion-related complications. CASE PRESENTATION: We report the case of a 42-year-old woman with chronic cholestatic liver disease who developed ACLF with hepatic encephalopathy, sys

Conducting successful mini-invasive laparotomy assisted artificial insemination in African Lion(Panthera leo).

Assisted reproductive technologies (ART), including artificial insemination (AI), represent promising tools for the conservation of genetic resources of vulnerable wild felid species. This study reports the application of AI in African lions ( Panthera leo), using fresh semen collected via urethral catheterization under general anesthesia. Semen samples were obtained from two adult male lions and subsequently analyzed for volume, motility, concentration, and morphology. A qualified sample, measuring 0.4 mL in volume, was extended at a 1:4 ratio, using 1.6 mL of a Tris-egg yolk-based extender.

Impact of intraoperative ketorolac on postoperative pain in children undergoing adenotonsillectomy: a double blind, placebo-control trial.

PURPOSE: Postoperative pain control following adenotonsillectomy in the pediatric population poses a great challenge to care providers. Multi-modal pain management regimes including NSAIDs such as intraoperative ketorolac usage has been purposed for many years. However, the effectiveness of ketorolac to reduce post-tonsillectomy pain and opioid-related side effects is controversial. The study was to evaluate the opioid-sparing effect of an intraoperative intravenous single dose of ketorolac in children undergoing adenotonsillectomy. We also assessed the effectiveness of perioperative ketorolac

Impact of different anesthetic protocols during anesthesia for the establishment of a porcine model of acute kidney injury.

BACKGROUND: During the establishment of a model of acute kidney injury (AKI) in pigs, we observed a high prevalence of malignant hyperthermia (MH). These complications led us to refine the anesthetic protocol. This publication describes the impact of the choice of anesthetics on the results obtained. METHODS: Pigs were euthanized at the end of the procedure, without recovery from anesthesia. Three anesthetic protocols were used: sevoflurane inhalation (ProtocolA, n = 5), a combination of ketamine, medetomidine and diazepam by intravenous infusion (ProtocolB, n = 5), and a combination of ketam

Efficacy of iliopsoas plane block for patients undergoing hip arthroscopy: a prospective, triple-blind, randomized, placebo-controlled trial.

BACKGROUND: Intraoperative stretching of the hip joint capsule often generates severe pain during the first 3 hours after hip arthroscopy. The short-lived severe pain mandates high opioid consumption, which may result in adverse events and delay recovery. The femoral nerve nociceptors are located anteriorly in the hip joint capsule. A femoral nerve block reduces pain and opioid demand after hip arthroscopy. It impedes, however, ambulation and home discharge after outpatient surgery. The iliopsoas plane block selectively anesthetizes the femoral sensory nerve branches innervating the hip joint

Effect of systemic lidocaine infusion on optic nerve sheath diameter during laparoscopic hysterectomy: a randomized controlled study.

BACKGROUND: During laparoscopic hysterectomy (LH), the elevation of intra-abdominal and intra-thoracic pressures due to pneumoperitoneum (PP) results in an increase in intracranial pressure (ICP). The Trendelenburg position (TP) is an accentuating factor. This trial aimed to assess the effect of intravenous (IV) lidocaine infusion on optic nerve sheath diameter (ONSD), a widely accepted surrogate measure for ICP, during PP and TP. METHODS: A randomized, placebo-controlled study was conducted on 66 patients scheduled for LH, equally divided into a lidocaine group and a saline group. ONSD, the

Does Propofol Effect Site (Brain) Concentration Predicted by Target-Controlled Infusion Correlate with Propofol Measured in the Brain? An Exploratory Study in Neurosurgical Patients.

BACKGROUND: Total intravenous anesthesia with propofol can be administered by target-controlled infusion pumps, which work on the principles of pharmacokinetic modeling. While designing this model, neurosurgical patients were excluded as the surgical site and drug action site remained the same (brain). Whether the predicted set propofol concentration and the actual brain site concentration correlate, especially in neurosurgical patients with impaired blood-brain barrier (BBB), is still unknown. In this study we compared the set propofol effect-site concentration in the target-controlled infusi
