Responses of dorsal spinal cord blood flow to noxious mechanical stimulation of the skin in anesthetized rats.

In urethane-anesthetized, artificially ventilated rats, alterations in dorsal spinal cord blood flow (SCBF) at the L4-6 level were measured with laser Doppler flowmetry in response to noxious mechanical cutaneous stimulation (pinching) of either a forepaw or a hindpaw. The stimulation was delivered ipsilaterally or contralaterally to the site of blood flow measurement. Pinching of the forepaw or the hindpaw on either side increased mean arterial pressure (MAP) to the same degree. However, the SCBF response to pinching of the ipsilateral hindpaw was significantly greater than that to other stim

Stable in vivo imaging of densely populated glia, axons and blood vessels in the mouse spinal cord using two-photon microscopy.

In vivo imaging has revolutionized our understanding of biological processes in brain physiology and pathology. However, breathing-induced movement artifacts have impeded the application of this powerful tool in studies of the living spinal cord. Here we describe in detail a method to image stably and repetitively, using two-photon microscopy, the living spinal tissue in mice with dense fluorescent cells or axons, without the need for animal intubation or image post-processing. This simplified technique can greatly expand the application of in vivo imaging to study spinal cord injury, regenera

The effects of extracorporeal shockwave therapy on the lumbar spine of osteoporotic rat models.

PURPOSE: Extracorporeal shockwave therapy (ESWT) is a noninvasive treatment known to promote angiogenesis and tissue regeneration. While preclinical studies on osteoporotic animal models have suggested its potential to increase bone mineral density (BMD) in long bones, the specific effect of ESWT on the osteoporotic spine has not been previously investigated. Therefore, the purpose of this study was to evaluate the effects of focused ESWT on BMD and bone formation parameters in the lumbar vertebrae using an ovariectomized (OVX) osteoporotic rat model. METHODS: Eighty female Sprague-Dawley rat

The Effects of Preoperative Oral Dextrose Loading on Mobility Independence and Perioperative Comfort in Patients Undergoing Orthopedic Surgery: A Randomized Controlled Trial.

PURPOSE: Orthopedic surgery induces trauma and metabolic disturbance, delaying recovery. Preoperative oral carbohydrate loading (POCL), integral to Enhanced Recovery After Surgery protocols, may alleviate these effects. This study aimed to assess the effects of preoperative oral dextrose loading (PODL/POCL) on mobility independence, perioperative comfort, and hospitalization outcomes (intensive care unit [ICU] and total length of stay) in patients undergoing orthopedic surgery. DESIGN: The design was a single-blind randomized controlled trial. METHODS: In this randomized controlled trial, 70

Physiologic and Clinical Sequelae After Pneumatic Tourniquet Release in Frail Patients Undergoing Total Knee Arthroplasty Under Regional Anesthesia: A Prospective Observational Study.

BACKGROUND: Tourniquet deflation during total knee arthroplasty (TKA) produces abrupt reperfusion physiology, including hyperkalemia, acidosis, and hemodynamic instability. Frail patients are particularly vulnerable; however, this population has not been prospectively studied. METHODS: We conducted a prospective, observational study of frail adults (Clinical Frailty Scale ≥ 4) undergoing elective, unilateral, cemented TKA under spinal anesthesia with a pneumatic thigh tourniquet. There were 40 frail patients (mean age, 76 years [range, 65 to 88]; 65% women) included. Tourniquet duration avera

Continuous Epidural Analgesia (CEA) via Single Catheter Providing Profound Analgesia for Pediatric Patients Following Posterior Spinal Fusion (PSF) in Adolescent Idiopathic Scoliosis (AIS).

INTRODUCTION: Posterior spinal fusion (PSF) is a commonly performed orthopedic procedure to correct scoliosis in children. Continuous epidural analgesia (CEA) is a proposed means of providing analgesia following PSF. Whether a single epidural catheter with the tip in the upper thorax can provide adequate analgesia for PSF, which often spans the upper thoracic to lower lumbar regions, is unresolved in the literature. METHOD: In this single-center, retrospective study, we reviewed 69 consecutive patients undergoing PSF for adolescent idiopathic scoliosis (AIS) with CEA at our institution from O

Regional Anesthesia Associated With Decreased Inpatient and Outpatient Opioid Demand in Tibial Plateau Fracture Surgery.

BACKGROUND: Regional anesthesia (RA) has been used to reduce pain and opioid usage in elective orthopedic surgery. The hypothesis of this study was that RA would be associated with decreased opioid demand in tibial plateau fracture surgery. METHODS: Inpatient opioid consumption and 90-day outpatient opioid prescribing in all patients ≥18 years of age undergoing tibial plateau fracture surgery from July 2013 to July 2018 (n = 264) at a single, level I trauma center were recorded. The presence or absence of perioperative RA was noted. Of 60 patients receiving RA, 52 underwent peripheral nerve b

Clinical efficacy of transforaminal endoscopic lumbar discectomy for lumbar degenerative diseases: A minimum 6-year follow-up.

BACKGROUND: Transforaminal Endoscopic Lumbar Discectomy (TELD) is widely applied for lumbar degenerative disease (LDDs) and satisfactory short-term outcomes have been achieved. However, the mid-term and long-term follow-up of this technique is still lacking. OBJECTIVE: To retrospectively analyze the mid-term clinical efficacy of TELD for single-level LDD and its effect on intervertebral disc degeneration with a minimum of 6-year follow-up. METHODS: 64 patients with single-level LDDs (lumbar disc herniation, lumbar spinal stenosis) who underwent TELD under local anesthesia in our department f

Intracranial hematoma as the cause of headache after subarachnoid anesthesia for cesarean section--a case report.

BACKGROUND: Intracranial subdural hematoma is an exceptionally rare but life-threating complication of epidural and spinal anesthesia. The diagnosis is rather difficult because the initial symptoms mimic post-dural puncture headache. CASE REPORT: A 33-year-old primipara was admitted to the hospital at 38 weeks gestation for a cesarean section due to premature rupture of membranes and meconium stained amniotic fluid. During the procedure a single puncture between L2 and L3 vertebrae was made with the use of a 26-gauge, pencil-point needle. The amount of 2.8 ml of analgesic solution was adminis

Ultrasound guided peritubal infiltration of 0.25% ropivacaine for postoperative pain relief in percutaneous nephrolithotomy.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is a common endourologic procedure with less morbidity than open surgery. However, pain around the nephrostomy tube requires good post operative analgesia. So we hypothesize that infiltration of local anesthetic from the renal capsule to the skin around the nephrostomy tract would relieve the pain in the initial postoperative period. METHODS: 60 adult patients of either sex with ASA physical status I to III and undergoing percutaneous nephrolithotomy were randomized for a prospective double-blind controlled study.
