Preterm birth may be associated with hypoxic-ischaemic encephalopathy (HIE) showing a well recognised number of patterns, including neuronal karyorrhexis/eosinophilia mostly at the diencephalon and brain stem and leukomalacia at the periventricular white matter. To investigate whether programmed cell death or apoptosis plays a role in HIE, we examined human brains of preterm infants. Brain tissue samples from 12 consecutive infants (24-34 weeks of gestation) were available at post-mortem examination (1998-2000) after approval of the Ethics Committee. Two tissue sections were stereologically lo…

In this case report, a 71-year-old female developed headache and nausea and signs of brain stem involvement following lumbar surgery with intraoperative dural tear. A CT scan showed cerebellar haemorrhage and hydrocephalus, and the patient was transferred to a neurosurgical department and treated with an external ventricular drainage, haematoma evacuation, and posterior fossa decompression. This case highlights the importance of swift diagnostics in patients developing neurological symptoms following lumbar surgery, as well as limiting any cerebrospinal fluid leakage following intraoperative d…

Stroke, a debilitating brain insult, afflicts millions of individuals globally each year. In the last decade, researchers have investigated cell-based therapy as an alternative strategy to improve neurological outcome following stroke. This concise review critically examines preclinical reports using human adult and fetal stem/progenitor cells in rodent models of ischemic stroke. As we enter the second decade of study, we should aim to optimize our collective likelihood to translational success for stroke victims worldwide. We advocate international consensus recommendations be developed for f

Coach Syndrome is a rare cause of Congenital Hepatic Fibrosis associated with neurological features. COACH is a mnemonic comprising of Cerebellar vermis hypo/aplasia, Oligophrenia (developmental delay/mental retardation), Ataxia, Coloboma and Hepatic fibrosis. Here we describe a 12 years boy who presented with hepatic encephalopathy. He was subsequently found to have marked developmental delay, bilateral ptosis and ataxia. CT scan revealed brain stem molar tooth sign, ophthalmoscopy showed optic disc coloboma and elastography showed hepatic fibrosis to confirm him as a case of COACH Syndrome.

Epstein Barr virus (EBV) encephalitis is rare in children but can have severe neurological complications and sometimes fatal. It can manifest with varied neurological presentations like meningoencephalitis, brain stem encephalitis, GBS etc. This can appear alone or with clinical picture of infectious mononucleosis. Establishing a diagnosis of EBV encephalitis is difficult and consequently molecular, serological and imaging techniques should be used when investigating a child with encephalitis. To highlight this entity we report two fatal cases of EBV meningoencephalitis presenting with sole ne…

Allogeneic stem cell transplantation (SCT) for solid tumors remains under investigation. We report a case of extended disease stability after a nonmyeloablative peripheral blood SCT for metastatic, refractory Ewing sarcoma. Of note, the patient developed metastatic disease to 2 unusual sites-the brain and small intestine. The allogeneic SCT environment may alter typical metastatic patterns, and may represent an ideal platform to manipulate and enhance the antitumor immune response. Further clinical trials are needed to evaluate the role for allogeneic SCT for this disease.

A case of remitting-relapsing Bickerstaff encephalitis is reported. The article focuses on its imaging findings and their significance when a clinical differentiation between Bickerstaff encephalitis and Miller-Fisher syndrome is attempted. Signs and symptoms may occasionally overlap. However, because Miller-Fisher syndrome is related to the peripheral nervous system and Bickerstaff encephalitis is a central disease, the recognition of brain stem hypointense lesions on T1-weighted images, which are hyperintense on T2-weighted sequences, could be a reliable tool when the clinical diagnosis is u…

A rare case of multiple cavernous angiomas with repeated hemorrhages in a 9-month-old male infant is presented, together with sequential computed tomography (CT) and magnetic resonance (MR) images. The infant underwent hematoma evacuation for left cerebellar hemorrhage and his postoperative course was uneventful. The multiple cavernous angiomas receded gradually without any treatment except the hematoma evacuation. Two years following presentation, he continues to do well. To the authors' knowledge, this is the first report of multiple cavernous angiomas with brain stem involvement in an infan…

18FDG-positron emission tomography performed at different stages in the course of subacute sclerosing panencephalitis revealed a changing pattern of metabolic disturbance. In clinical stage II patients the inflammation in the basal ganglia appeared to lead to neuronal excitation accompanied by hypermetabolism. Widespread cortical functional inhibition of metabolism followed. The striatal inflammation ended with necrosis and hypometabolism, with resulting functional cortical disinhibition; later, deep midbrain structures and brain stem became hypermetabolic. A patient clinically in remission sh…

Magnetic resonance imaging has become the study of choice for the diagnosis of retrocochlear and central vestibular disease. Three groups of lesions, each characterized by a specific site of origin, have been recognized: 1) petrous apex lesions such as congenital cholesteatomas and cholesterol granulomas involving the eighth cranial nerve within the internal auditory canal; 2) cerebellopontine angle tumors, mainly acoustic neuromas and meningiomas; and 3) CNS disease involving the brain stem and the central auditory and vestibular pathways such as tumors, multiple sclerosis, infarcts, and hemo…

Following maternal chicken pox in the 14th week of pregnancy, a male infant was born with low birth weight, muscle wasting and limb contractures, hypotonia and areflexia. A rising titre of varicella-zoster-specific IgM (by enzyme-linked immunoabsorbent assay) confirmed congenital infection, and electromyogram showed widespread denervation. Death occurred at 8 days. Neuropathological examination revealed destructive and inflammatory lesions of cerebral cortex and white matter, thalamus, brain stem and spinal cord. In addition there were cerebellar heterotopias and bilateral polymicrogryic malfo…

Traumatic brain injury (TBI) triggers complex secondary pathologies that lack effective treatments. While mesenchymal stem cell (MSC) transplantation is promising, it is severely limited by poor cell retention and survival. To address these challenges, we engineered a combinatorial platform comprising an injectable, self-healing supramolecular gelatin hydrogel (iGel) loaded with CRISPR-SAM-engineered "Super MSCs" (SPMSCs). These cells were programmed to endogenously multiplex the activation of neuroprotective factors IL-10 and FGF21. Our results demonstrate that the biomimetic iGel niche enhan

OBJECTIVE: To investigate the utility of median nerve somatosensory evoked potentials (SEPs) and auditory brainstem responses (ABRs) in the early diagnosis of Bickerstaff brainstem encephalitis (BBE).
METHODS: Data from patients diagnosed with BBE between 2001 and 2022 were extracted. Patients who underwent SEPs and ABRs within two weeks of onset were included. Normal P13/14 onset (P13/14o) latency with lost or depressed N20, or with prolonged P13/14o-N20 onset interval, was interpreted as indicating an intracranial lesion. The P14b subcomponent, which has been attributed to the thalamocortic

This comprehensive longitudinal study explored for the first time the interrelations between neonatal brainstem abnormalities, focused attention (FA), and learning-following a preterm cohort (N = 175; 46.3% female; predominantly White) from birth (2003-2006) to 17 years. The findings indicated that FA during early childhood was associated with language outcomes in toddlerhood (n = 131) and academic and attention self-report indices in late adolescence (n = 44). Pilot assessments indicated that FA at 17 years (n = 25) was also associated with concurrent academic and attention functioning. Struc

OBJECTIVE: The objective of this study was to use machine learning in the form of a deep neural network to objectively classify paired auditory brainstem response waveforms into either: 'clear response', 'inconclusive' or 'response absent'.
DESIGN: A deep convolutional neural network was constructed and fine-tuned using stratified 10-fold cross-validation on 190 paired ABR waveforms. The final model was evaluated on a test set of 42 paired waveforms.
STUDY SAMPLE: The full dataset comprised 232 paired ABR waveforms recorded from eight normal-hearing individuals. The dataset was obtained from

Neural responses to sensory inputs in a complex and natural environment must be weighted according to their relevance. To do so, the brain needs to be able to deal with sudden stimulus fluctuations in an ever-changing acoustic environment. Stimulus-specific adaptation (SSA) is a phenomenon of some neurons along the auditory pathway that show a reduced response to repetitive sounds while responsive to those that occur rarely. SSA has been shown from the inferior colliculus to auditory cortex, but has not been detected in the cochlear nucleus. To discover where SSA is first generated along the a

Adult barn owl hearing is acute, but development of this sense is not well understood. We, therefore, measured auditory brainstem responses in barn owls from before the onset of hearing (posthatch day 2, or P2) to adulthood (P69). The first consistent responses were detected at P4 for 1 and 2 kHz, followed by responses to 0.5 and 4 kHz at P9, and 5 kHz at P13. Sensitivity to higher frequencies increased with age, with responses to 12 kHz appearing about 2 months after hatching, once the facial ruff was mature. Therefore, these altricial birds achieve their sensitivity to sound during a prolong

OBJECT: The authors performed a retrospective review of prospectively collected data to evaluate the safety and efficacy of stereotactic radiosurgery (SRS) for the treatment of patients harboring symptomatic solitary cavernous malformations (CMs) of the brainstem that bleed repeatedly and are high risk for resection.
METHODS: Between 1988 and 2005, 68 patients (34 males and 34 females) with solitary, symptomatic CMs of the brainstem underwent Gamma Knife surgery. The mean patient age was 41.2 years, and all patients had suffered at least 2 symptomatic hemorrhages (range 2-12 events) before ra
