Hemorrhagic infarction: A reperfusion injury following prolonged myocardial ischemic anoxia.

Sixteen canine myocardial pedicle preparations were divided into four groups. Group I received 60 minutes of reversible ischemic anoxia, Group II 90 minutes, Group III 120 minutes, and Group IV 180 minutes. Transmural ischemic infarction was present in Group I. With longer periods of anoxia, capillary disruption occurred, first in the subendocardium (90 minutes) and then transmurally (180 minutes). Reperfusion after ischemic, anoxic time periods of 90 minutes or greater produced hemorrhagic necrosis, the extent of which was directly proportional to the duration of the preceding ischemic, anoxi

Cerebral oximetry levels during CPR are associated with return of spontaneous circulation following cardiac arrest: an observational study.

OBJECTIVES: Cerebral oximetry using near-infrared spectroscopy measures regional cerebral oxygen saturation (rSO2) non-invasively and may provide information regarding the quality of cerebral oxygen perfusion. We determined whether the level of rSO2 obtained during cardiopulmonary resuscitation is associated with return of spontaneous circulation (ROSC) and survival in Emergency Department (ED) patients presenting with cardiac arrest. METHODS: We conducted a retrospective, observational study of adult ED patients presenting at an academic medical centre with cardiac arrest in whom continuous

Evidence of diminished coronary flow in pulmonary hypertension: explaining angina pectoris in this patient group?

BACKGROUND: Many patients with pulmonary hypertension (PH) have symptoms of angina without evidence of occlusive coronary artery disease. For the first time, this study addresses the influence of progressively increasing pulmonary artery pressure (PAP) on left anterior descending artery flow in a rat model of PH. The role of pulmonary artery dilatation, septal wall motion abnormality, cardiac output or diastolic blood pressure in determining coronary blood flow (CBF) during PH was determined. METHODS: Pulmonary hypertension was induced in 6-week-old female nude rats (n = 44) using monocrotali

Preliminary results of Tc-99m ECD SPECT to evaluate cerebral collateral circulation during balloon test occlusion.

PURPOSE: The authors obtained noninvasive quantitative measurements of regional cerebral blood flow using Tc-99m L,L-ethyl cysteinate dimer (Tc-99m ECD) SPECT to evaluate cerebral collateral circulation during balloon test occlusion (BTO) of the internal carotid artery and vertebral artery (VA). METHODS: Five cases associated with an aneurysm and a brain tumor were investigated. After 10 minutes of BTO, a bolus of Tc-99m ECD was injected intravenously and radionuclide angiography was performed. The balloon remained inflated for an additional 5 minutes. After radionuclide angiography was compl

Rationale and design of the quantification of myocardial blood flow using dynamic PET/CTA-fused imagery (DEMYSTIFY) to determine physiological significance of specific coronary lesions.

BACKGROUND: Coronary physiology assessments have been shown by multiple trials to add clinical value in detecting significant coronary artery disease and predicting cardiovascular outcomes. Fractional flow reserve (FFR) obtained during invasive coronary angiography (ICA) has become the new reference standard for hemodynamic significance detection. Absolute myocardial blood flow (MBF) quantification by means of dynamic positron emission tomography (dPET) has high diagnostic and prognostic values. FFR is an invasive measure and as such cannot be applied broadly, while MBF quantification is commo

Comparison of myocardial microcirculatory perfusion after catheter-administered intracoronary thrombolysis with anisodamine versus standard thrombus aspiration in patients with ST-elevation myocardial infarction.

OBJECTIVE: To evaluate efficacy, safety and feasibility of targeted intracoronary injection using pro-urokinase combined with anisodamine (TCA) versus thrombus aspiration (TA) in ST-elevation myocardial infarction (STEMI) patients with high thrombus loads. BACKGROUND: The best method of avoiding thrombus detachment and stroke in PCI patients with high thrombus loads has not yet been established. METHODS: STEMI patients receiving coronary artery angiography or percutaneous coronary intervention (CAG/PCI) with thrombus grade ≥ 3 from January 1, 2017 to June 30, 2018 were randomly assigned to t

Increased benefit of interleukin-1 inhibition on vascular function, myocardial deformation, and twisting in patients with coronary artery disease and coexisting rheumatoid arthritis.

BACKGROUND: We investigated the effects of anakinra, an interleukin-1 receptor antagonist, on coronary and left ventricular function in coronary artery disease (CAD) patients with rheumatoid arthritis. METHODS AND RESULTS: In a double-blind crossover trial, 80 patients with rheumatoid arthritis (60 with CAD and 20 without) were randomized to a single injection of anakinra or placebo and after 48 hours to the alternative treatment. At baseline and 3 hours after treatment, we assessed (1) flow-mediated dilation of brachial artery; (2) coronary flow reserve, ejection fraction, systemic arterial

Adenosine stress cardiovascular magnetic resonance with variable-density spiral pulse sequences accurately detects coronary artery disease: initial clinical evaluation.

BACKGROUND: Adenosine stress cardiovascular magnetic resonance perfusion imaging can be limited by motion-induced dark-rim artifacts, which may be mistaken for true perfusion abnormalities. A high-resolution variable-density spiral pulse sequence with a novel density compensation strategy has been shown to reduce dark-rim artifacts in first-pass perfusion imaging. We aimed to assess the clinical performance of adenosine stress cardiovascular magnetic resonance using this new perfusion sequence to detect obstructive coronary artery disease. METHODS AND RESULTS: Cardiovascular magnetic resonanc

Combined anatomical and functional imaging using coronary CT angiography and myocardial perfusion SPECT in symptomatic adults with abnormal origin of a coronary artery.

There has been a lack of standardized workup guidelines for patients with congenital abnormal origin of a coronary artery from the opposite sinus (ACAOS). We aimed to evaluate the use of cardiac hybrid imaging using multi-detector row CT (MDCT) for coronary CT angiography (Coronary CTA) and stress-rest myocardial perfusion SPECT (MPS) for comprehensive diagnosis of symptomatic adult patients with ACAOS. Seventeen symptomatic patients (12 men; 54 ± 13 years) presenting with ACAOS underwent coronary CTA and MPS. Imaging data were analyzed by conventional means, and with additional use of 3D imag

Cerebral cortical oxygenation changes during OPCAB surgery.

BACKGROUND: We investigated the changes occurring in cerebral cortical oxygenation during off-pump coronary artery bypass (OPCAB) surgery using near infrared spectroscopy (NIRS). METHODS: Measurement of cerebral cortical oxygenation changes included concentration of deoxygenated hemoglobin [HHb], oxygenated hemoglobin [O(2)Hb], changes in the redox status of the cytochrome c oxidase [Cyt-Ox], cerebral saturation as expressed by the tissue oxygenation index (TOI), and cerebral blood volume (CBV) as expressed by tissue hemoglobin index (THI). Measurements were performed in 23 consecutive patien

Thromboprophylaxis in congenital and acquired heart disease across the lifespan. A French consensus position statement: Part 2 - from adolescence to adulthood.

Patients with congenital and acquired heart disease are exposed to a lifelong thromboembolic risk that increases progressively from adolescence to adulthood because of residual lesions, arrhythmias, venous congestion, repeated interventions and acquired co-morbidities. Management is further complicated by concurrent bleeding risk. To provide practical expert consensus recommendations on thromboprophylaxis in congenital and acquired heart disease from adolescence to adulthood. A multidisciplinary expert panel from the Paediatric and Congenital Cardiology Branch of the French Society of Cardiolo

Perioperative Takotsubo Cardiomyopathy Revealed by Ventricular Arrhythmia After a Minor Surgery in a Young Woman.

Takotsubo syndrome (TTS) is an acute, reversible form of stress-induced cardiomyopathy typically triggered by intense emotional or physical stress. Perioperative presentations are uncommon but increasingly recognized, and may present abruptly with severe hemodynamic instability or malignant arrhythmias. These forms can mimic anesthetic complications or acute coronary syndromes, delaying diagnosis. We report the case of a healthy 32-year-old woman who developed sustained monomorphic ventricular tachycardia followed by cardiac arrest 15 minutes after an uncomplicated stapedotomy under general an

Abdominal vascular calcification at CT: opportunistic quantification and imaging-based risk stratification.

Abdominal vascular calcification is increasingly recognized as a clinically relevant marker of systemic atherosclerotic burden and regional vascular disease. Because the abdominal aorta and its major branches are frequently captured on routine abdominal, oncologic, vascular, and preoperative CT examinations, CT provides a practical opportunity for opportunistic assessment of vascular calcium beyond the coronary circulation. This review summarizes current approaches to CT-based evaluation of abdominal vascular calcification, with a focus on abdominal aortic, renal artery, mesenteric and celiac …

Air Embolism in a Case of Pulmonary Aspergillosis-A Case Report With Brief Review of Literature.

Air embolism is a life-threatening condition characterized by the entry of air into the vessels, causing circulatory collapse. Air embolism may be arterial or venous, with venous air embolism being the most common. Arterial air embolism is commonly due to trauma or right to left shunting of cardiac circulation in cases of patent foramen ovale, ventricular septal defects, or rarely due to pulmonary arteriovenous fistulas. We report a case of a 40-year-old male who was diagnosed with pulmonary aspergillosis and sarcoidosis. He presented to the hospital with acute hemoptysis, where he deteriorate

Thrombotic storm presenting with synchronous myocardial infarction, stroke and bowel ischaemia: A case report.

BACKGROUND: We present the first known case of simultaneous myocardial infarction, stroke, and bowel infarction, likely triggered by a thrombotic crisis. CASE SUMMARY: A 72-year-old man was found unresponsive in his car and was diagnosed with acute inferoposterior ST-elevation myocardial infarction (STEMI) and slow atrial fibrillation (AF). A computed tomography (CT) brain scan initially ruled out stroke, and the preliminary diagnosis was cardiogenic shock, slow AF, and Killip 4 acute STEMI, complicated by lactic acidosis and delirium. The patient underwent catheterization, revealing a comple
