BACKGROUND: McCune-Albright syndrome (MAS) is a rare genetic, non-inheritable disease and is characterized by fibrous dysplasia, hyperendocrinism, and café-au-lait macules. Pituitary adenomas could be concurrent with this syndrome but clinicopathological features and the surgical management of such disorders is unclear.
METHODS: We retrospectively reviewed ten MAS-associated pituitary adenoma patients with follow-up in Beijing Tiantan Hospital and analyzed their clinicohistological data, surgical strategies, neuro-imaging, genetic mutations, and prognosis. Moreover, a critical review of the E

Nonsurgical treatment is the first option in patients with radiculopathy due to spinal metastasis. However, we have to consider surgical management for patients who are resistant to conservative treatment. There are few reports of surgical treatment for radiculopathy due to metastatic spine tumors. We present cases in three patients who underwent surgery for radiculopathy due to spinal metastasis. Case 1 was in an 82-year-old woman with lumbar foraminal stenosis at L5-S1 due to breast cancer metastasis to the right L5-S1 intervertebral foramen. She underwent subtotal tumor resection and poster

INTRODUCTION: Diffuse idiopathic skeletal hyperostosis (DISH) extended to the lumbar segments (L-DISH) reportedly has adverse effects on the surgical outcomes of lumbar spinal stenosis (LSS). However, the risk factors in patients with L-DISH have not been clarified. The purpose of this study was to investigate the long-term risk factors for reoperation at the same level after decompression surgery alone for LSS in patients with L-DISH in a retrospective cohort study.
METHODS: A postoperative postal survey was sent to 1,150 consecutive patients who underwent decompression surgery alone for LSS

BACKGROUND: Patient counseling and selection for surgical therapy in adult Chiari malformation type I (CM-1) remain debatable. We aimed to develop a clinical calculator predicting the risk of nonhome discharge and reoperation using the American College of Surgeons-National Surgical Quality Improvement Program database.
METHODS: The database from years 2011 through 2017 was queried to identify the subset of CM-1 patients undergoing suboccipital decompression. Univariable analysis was conducted to identify baseline factors associated with nonhome discharge and 30-day reoperation following the i

OBJECTIVE: To investigate the influence of modified clivoaxial angle (MCAA) on the surgical planning and outcome in adult patients with Chiari malformation type I.
METHODS: Clinical data of 268 adult patients undergoing posterior fossa decompression without (PFD) or with duraplasty (PFDD) or with resection of tonsils (RT) for Chiari malformation type I over 10 years were reviewed retrospectively, with outcome evaluated by Chicago Chiari Outcome Scale (CCOS) score. By means of receiver operating characteristic curve, the threshold of MCAA was calculated to stratify the patients.
RESULTS: MCAA

OBJECTIVE: Microvascular decompression (MVD) is considered the first choice for the surgical treatment of trigeminal neuralgia (TN). However, recurrence after MVD is still a problem and a challenge for neurosurgeons. In this study, we share our experience in the posterior fossa re-exploration of the recurrent cases.
METHODS: From January 2014 to January 2015, 15 recurrent TN patients who received fossa re-exploration were retrospectively studied. Surgeries were performed by retrosigmoid suboccipital approach. Brainstem auditory evoked potentials (BAEP) were monitored throughout the operation.

PURPOSE: While radiotherapy is generally an acceptable treatment for metastatic spinal cord compression, surgical intervention is controversial due to the invasiveness and diversity of diseases in the patients being considered. The ideal treatment, therefore, depends on the situation, and the most acute treatment possible is necessary in patients presenting with myelopathy. We compared the neurological outcomes between radiotherapy and surgery in patients with metastatic spinal cord compression presenting with myelopathy.
METHODS: A total 54 patients with metastatic spinal cord compression pr

INTRODUCTION: Risk factors associated with Carpal Tunnel Syndrome include repetitive use of hand and wrist, advanced age, obesity, pregnancy, diabetes mellitus and thyroid disease. Decompression of the median nerve is the last treatment of choice usually indicated when negative results to conservative treatments remain for three months. In this study, we aimed to find out whether hypothyroid patients would respond to CTS surgical decompression differently in comparison to healthy individuals.
METHODS: This case control study was conducted on patients with CTS in need of surgical release who w

We describe two surgical strategies for treating thoracic spinal stenosis (TSS) with ossification of the ligamentum flavum (OLF) and dural ossification (DO), and discuss their postoperative efficacy. From January 2004 to June 2008, 147 patients underwent TSS surgery. Thirty three of those with intraoperative evidence of OLF and DO were included in the present study. Based on the different intraoperative treatment of the dura, these 33 patients were divided into two groups: Group A, 17 patients who had their dura slit and the ossification excised, and Group B, 16 patients treated by floating th

BACKGROUND: Superior orbital fissure syndrome is a rare complication that occurs in association with craniofacial trauma. The characteristics of superior orbital fissure syndrome are attributable to a constellation of cranial nerve III, IV, and VI palsies. This is the largest series describing traumatic superior orbital fissure syndrome that assesses the recovery of individual cranial nerve function after treatment.
METHODS: In a review from 1988 to 2002, 33 patients with superior orbital fissure syndrome were identified from 11,284 patients (0.3 percent) with skull and facial fractures. Seve

INTRODUCTION: Demographic changes create an expansion of the elderly population segment of society. Demands of mobility and quality of life have increased in this age group over the last decades. A rising number of elderly patients suffering from degenerative "de novo" lumbar scoliosis (DLS) may be eligible for surgical options to treat their spinal deformities. Therefore, we compared the clinical and radiographic results of conservative and surgical treatment.
MATERIALS AND METHODS: During 1990-2003, 55 patients with DLS were treated and followed up for a minimum of 24 months. Group 1 patien

OBJECTIVE: Rheumatoid arthritis frequently affects the craniovertebral junction (CVJ) and may lead to severe neck pain, quadriparesis, and respiratory dysfunction. Surgery in rheumatoid nonambulatory (Ranawat Class IIIb) patients carries a significant risk. This study presents the surgical outcome of Class IIIb patients with CVJ rheumatoid myelopathy and reviews the literature.
METHODS: One hundred twelve consecutive patients with rheumatoid cervical myelopathy underwent surgical decompression and stabilization. Thirty-two of the patients (mean age, 66.81 +/- 10.25 yr) with CVJ rheumatoid art

OBJECTIVE: Bile leaks are known complications of cholecystectomy. The combination of sphincterotomy and nasobiliary tube (NBT) drainage is effective for the immediate decompression of bile ducts and provides access for follow-up cholangiography. Our objective was to study, retrospectively, 19 patients who had undergone treatment for bile leaks with this combination between October 1991 and December 1995.
METHODS: Nineteen patients (15 F, 4 M) ages 23-83 yr (mean 50 yr) presented with bile leaks secondary to open cholecystectomy (n = 1) and laparoscopic cholecystectomy (n = 18). All patients h

The optimal treatment of "burst" fractures is one of the more controversial topics in spinal reconstructive surgery. While it is generally considered a stable fracture, recent trends toward operative treatment of burst injuries have raised questions regarding the necessity of stabilization and decompression. A retrospective review was conducted of all patients who presented at the University of Minnesota Hospitals from 1970 to 1980 with closed thoracolumbar spinal fractures. In 354 of 399 patients, records and roentgenograms were adequate for review. Using strict x-ray criteria for classificat

The optimal timing of surgical intervention in cervical spinal cord injuries has not been defined. The goals of the study were to investigate changes in neurologic status, length of hospitalization, and acute complications associated with surgery within 3 days of injury versus surgery more than 3 days after the injury. All patients undergoing surgical treatment for an acute cervical spinal injury with neurologic deficit at two institutions between March 1989 and May 1991 were reviewed retrospectively. Forty-three patients initially were evaluated. At one institution, patients with neurologic s

OBJECTIVELumbar spinal stenosis (LSS) is the most common spinal disease in the geriatric population, and is characterized by a compression of the lumbosacral neural roots from a narrowing of the lumbar spinal canal. LSS can result in symptomatic compression of the neural elements, requiring surgical treatment if conservative management fails. Different surgical techniques with or without fusion are currently treatment options. The purpose of this study was to provide a description of the long-term clinical outcomes of patients who underwent bilateral laminotomy compared with total laminectomy

BACKGROUND: Concurrent trigeminal neuralgia (TN) and hemifacial spasm (HFS), historically termed painful tic convulsif, is a rare manifestation of cranial nerve hyperactive dysfunction associated with neurovascular compression. The operative anatomy may involve independent conflicts affecting cranial nerves V and VII or shared compression, particularly in vertebrobasilar dolichoectasia.
METHODS: We report a case of medication-refractory TN with ipsilateral HFS treated by microvascular decompression (MVD) and integrate the findings with a focused review of 23 studies addressing combined crania

BACKGROUND: Kerrison rongeurs are frequently used during decompressive procedures but carry risks such as incidental dural tears and occupational strain for spine surgeons. High-speed drills are often used for bony decompression. In other surgical specialties, ultrasonic aspirators have been shown to safely remove bone near critical structures like the dura and nerves. However, their application in spine surgery remains underexplored. The aim of this pilot study was to evaluate the safety and efficacy of ultrasonic aspirators as an alternative to high-speed drills and Kerrison rongeurs in mini
