Surgical treatment of dysphagia.

The role of surgery in the management of dysphagia is clear in some areas and controversial in others. Evaluation for the causes of dysphagia can elucidate conditions in which surgery can improve safety, quality of life, or both. Surgical therapy, when indicated, is safe and effective for many causes of dysphagia. This article includes a general overview of the causes of dysphagia that can be addressed successfully with surgery as well as a discussion of why surgery may be less appropriate for other conditions associated with dysphagia.

Manifestation of congenital urethral diverticula in a 57-year-old male.

Congenital urethral diverticula are a rare finding in adult males. Most cases are diagnosed in childhood or adolescence because of voiding symptoms such as urinary dribbling. Diagnostic workup should include radiography and urethroscopy. The standard therapeutic approach is open surgical excision or endoscopic marsupialization. An unusual case of male congenital urethral diverticula that remained asymptomatic until age 57 is presented.

Endoscopic stapling of pharyngeal pouch.

The relatively new technique of endoscopic stapling in the surgical management of pharyngeal pouch has gained widespread popularity over the last few years. The technique seems to be the procedure of choice as it is easier and quicker to perform and has a lower morbidity compared with open techniques. We present here an overview of the current evidence provided by a literature search performed by Ovid MEDLINE on literature published between January 1993 and May 2003 on endoscopic stapling of pharyngeal pouch. Several case series have been reported on various aspects of the surgery. The modific

Behçet's disease and diverticulosis.

Behçet's disease (BD) is a multisystem disorder characterized by vasculitis. The aim of this report is to present a patient with BD and diverticular disease of the colon and discuss the possible association between BD and diverticulosis. To our knowledge, diverticular disease of the colon has not been previously reported in a patient with BD. We conclude that the significance of this association between BD and diverticulosis needs to be clarified.

Diverticular disease of the colon.

Diverticular disease of the colon is quite common in developed countries, and its prevalence increases with age. Although present in perhaps two thirds of the elderly population, the large majority of patients will remain entirely asymptomatic. Nonetheless, an estimated 20% of those affected may manifest clinical illness, mainly diverticulitis, with its potential complications of abscesses, fistulas, and obstruction, as well as lower intestinal hemorrhage. The purpose of this report is to review our understanding of the epidemiology, pathophysiology, clinical presentation, and treatment option

Lower gastrointestinal tract bleeding.

Lower GI bleeding can be slow and chronic or massive and fatal. ED evaluation of these patients begins with history directed at determining the severity and amount of bleeding, and eliciting symptoms of volume depletion. Physical examination determines orthostasis, exclusion of an upper GI source for bleeding, and rectal examination. Laboratory evaluation is directed at determining baseline status of hemoglobin/hematocrit and platelet adequacy, as well as assessing concomitant medical problems. Although many of these patients are elderly, resuscitation is vigorous and should not be deterred by

The role of nuclear medicine in acute gastrointestinal bleeding.

In most patients with upper gastrointestinal (GI) bleeding endoscopy will locate the site and cause of bleeding, and also provide an opportunity for local therapy. The cause of lower GI bleeding is often difficult to attribute, even when pathology is found by colonoscopy or barium enema. Nuclear medicine techniques can be used to identify the site of bleeding in those patients in whom the initial diagnostic procedures are negative or inconclusive. Methods using transient labelling of blood (e.g. 99Tcm-sulphur colloid) produce a high target-to-background ratio in positive cases, give quick resu

Usefulness of colonic motility study in identifying patients at risk for complicated diverticular disease.

Colonic motility study was performed on a total of 145 patients. Of these, 55 were patients with symptomatic complicated diverticular disease, 30 had symptomatic uncomplicated diverticular disease, 30 had asymptomatic diverticular disease, and 30 were controls. The pressure sensors were positioned in the descending and the true sigmoid colon. Colonic motility index was significantly higher in symptomatic rather than in asymptomatic diverticular disease in the resting (P less than 0.001) and postprandial (P less than 0.001) states. This confirmed the association between symptomatic diverticular

Ileal diverticulosis in a black patient. A case report.

A 54-year-old black man presented to hospital with diarrhoea containing blood and mucus and marked loss of weight over a 1-year period. Barium enema examination revealed cobblestoning and rose-thorn ulceration with possible stricture formation of the terminal ileum. A right hemicolectomy was performed to remove inflamed sections of small and large bowel. Histological examination of the resected specimen demonstrated features of ileal diverticular disease. This is the first documented case of this disease occurring in a black patient.

Diverticulosis of the appendix.

Diverticulosis of the appendix is rarely reported. Two cases with this condition, including a unique example of appendiceal diverticulosis presenting at herniorrhaphy, are described. The aetiology of the condition and factors involved in incidental appendicectomy are considered.

Carcinoid tumors in Meckel's diverticula.

Coexisting carcinoid tumors within Meckel's diverticula have been described rarely. Our experience with an elderly patient with this unusual pathologic combination led to an extensive review of the world's literature. This review documents that carcinoid tumors are the most commonly reported tumors of Meckel's diverticula, with risks of symptoms and metastases similar to those described with primary small intestinal carcinoid tumors.
