Subscapularis-sparing deltopectoral approach in reverse total shoulder arthroplasty.

PURPOSE: This study compared the clinical and radiologic results of reverse shoulder arthroplasty (RSA) using either the subscapularis-sparing deltopectoral approach (SSDA) or traditional deltopectoral approach (TDA) in cuff tear arthropathy patients. MATERIALS AND METHOD: We retrospectively evaluated 71 patients who underwent RSA for cuff tear arthropathy between July 2014 and December 2018. Patients were divided into two groups according to the surgical approach: TDA (34 cases) and SSDA (37 cases). The mean patient age was 78.6 years, and the mean (range) follow-up period was 23.5 (12-48) m

Combined Bankart and SLAP repair: patient-reported outcome measurements after a minimum 5-year follow-up.

BACKGROUND: Anterior shoulder instability is typically characterized by detachment of the anteroinferior labrum (Bankart lesion). Some patients also sustain a superior labrum anterior-to-posterior (SLAP) injury. The purpose of this study was to compare the medium-term clinical results of isolated anterior Bankart repairs (ABR) with those of combined Bankart and SLAP repair (ABR + SLAP). METHODS: Data on all patients treated surgically for recurrent anterior shoulder instability between 2006 and 2011 were retrospectively collected from medical charts. The minimum follow-up was 5 years. Patient

Plate Osteosynthesis or Figure-of-Eight Brace: Which One Is Better in Midshaft Clavicle Fractures?

Introduction Fracture of the clavicle bone is a very common injury owing to its subcutaneous location. Controversy exists about the optimal treatment of midshaft clavicle fractures in the presence of significant displacement and comminution of the fracture. Traditionally, non-surgical management was considered the first treatment option for most clavicle fractures. However, recent evidence shows that the non-surgical option causes more complications than previously reported. The purpose of this study was to compare the clinical and radiological outcomes of conservative treatment and surgical t

Arthroscopic treatment of type II superior labral anterior to posterior (SLAP) lesions in a younger population: minimum 2-year outcomes are similar between SLAP repair and biceps tenodesis.

PURPOSE: Compared to a relatively older population over 30-40 years of age, the efficacy of biceps tenodesis for type II SLAP lesions in a younger population is not well studied. The purpose of this study was to compare outcomes between biceps tenodesis and labral repair for type II SLAP lesions in a young active population. METHODS: Patients aged 15-40 who underwent primary arthroscopic biceps tenodesis or SLAP repair for type II SLAP tears between 2009 and 2016 were included. Shoulders with intraarticular chondral damage, full thickness rotator cuff tear, rotator cuff repair, labral repair

Evaluation of Fixation for Distal Humeral Diaphyseal Fracture by Locking Compression Plate.

Treatment of extra-articular distal humeral shaft fractures with plating techniques is often difficult. The recent development of LCP has improved the surgical treatment of fractures by overcoming the few drawbacks of older fixators. The aim of this prospective observational study was to assess the effectiveness of osteosynthesis of extra-articular diaphyseal fractures of the distal third of the humerus using a single locking compression plate (LCP) and was conducted from July 2016 to June 2018 at the National Institute of Traumatology and Orthopedic Rehabilitation (NITOR), Dhaka, Bangladesh.

Supracondylar humerus fractures in children treated with closed reduction and percutaneous pinning.

Supracondylar fractures of the humerus in children are important for frequency and type of associated serious complications. The management of this kind of fractures is still controversial (Skaggs et al. in J Bone Joint Surg Am 86:702-707, 2004; Kalllio et al. in J Pediatr Orthop 12:11-15, 1992). We are going to present our experience in the treatment of supracondylar humeral fracture in children. In the Orthopedic Department of Pisa, we treated 150 cases from 1989 to 2006. We are used to perform, emergency or within 12 h, reduction and two lateral-entry percutaneous pins fixation. The mean ag

Plain radiographs are inadequate to identify overlengthening with a radial head prosthesis.

PURPOSE: A correctly sized radial head implant helps restore more normal elbow motion and load distribution and allows the collateral ligaments to heal in an anatomic position. No single method of measurement has been agreed on. We hypothesized that plain radiographs could be used to evaluate the appropriate length of a radial head prosthesis when we simulated different patterns of ligament disruption. METHODS: Osteotomies of the medial and lateral condyles were created in 6 cadaver specimens to simulate 4 conditions: ulnar collateral ligament (UCL) and lateral ulnar collateral ligament (LUCL

Tips for Successful Ulnar Collateral Ligament Reconstruction.

BACKGROUND: Ulnar collateral ligament (UCL) reconstruction has become an increasingly common procedure in overhead-throwing athletes because of overuse and repetitive valgus stress placed on the inside of the elbow or valgus stress during an acute traumatic event2. Athletes with a deficient UCL often report a decrease in pitch velocity and accuracy in addition to an increase in fatigue, pain, and instability4. As such, UCL reconstructive procedures have been increasing in prevalence in throwing athletes to address their symptoms and regain their throwing capabilities, particularly in young bas

[Significance of computerized tomography in the diagnosis of post-traumatic proximal carpal instability].

INTRODUCTION: Proximal carpal instability is a painful condition characterized by early or late loss of radioulnar joint (RUJ) congruence not affecting the normal bone alignment of the two carpal rows. The joint incongruence or (incomplete) dislocation which leads to proximal instability is caused by many traumatic and nontraumatic events. The diagnosis of (incomplete) dislocation of the distal RUJ may be extremely difficult to make at conventional radiography because such injuries can be seen only when the lateral joint projection is perfect; otherwise the diagnosis is not reliable. CT is the

Deformity after fracture of the lateral humeral condyle in children.

We studied 28 displaced (Jacob types II and III) fractures of the lateral humeral condyle in 28 children. There were only two Milch type I fractures. Twenty-one fractures were treated by open reduction and internal fixation with K-wires, Palmer nails, or sutures. Seven patients were treated with a plaster cast, five following closed reduction, and the primary displaced position being accepted in two. Anatomic reduction was obtained in 18 patients. All but one fracture were united at review 2 years to 16 years later. The distal humerus was wider on the injured side in all patients. Six patients

The outcome of early revision of malaligned pediatric supracondylar humerus fractures.

BACKGROUND: Supracondylar humerus fractures (SCHF) are the most common elbow fractures requiring surgical treatment in the pediatric age group. Most fractures are reduced and stabilised adequately. Yet, post-surgical malunion may occur. The purpose of this study is to evaluate our results of early revision surgery in 21 surgically treated pediatric SCHF with immediate postoperative loss of alignment and compare them with previous reports of late corrective osteotomies. METHODS: Twenty-one pediatric SCHF patients that underwent revision surgery for malalignment within 3 weeks of the initial re

The role of the upper extremity surgeon in the management of tetraplegia.

Despite its demonstrated benefits, the majority of patients with tetraplegia do not have upper extremity surgery. Many upper extremity surgeons are unfamiliar with the techniques, long-term care providers are often unaware of the options or are biased against surgery, and patients might resist surgery based on lack of awareness, misinformation, or hope of a cure. Although it is not without its challenges, some consensus as to a general algorithm for the care of the tetraplegic patient does exist. At the recent 10th International Meeting on Surgical Rehabilitation of the Tetraplegic Upper Limb,

Biomechanical factors affecting the peak hand reaction force during the bimanual arrest of a moving mass.

Fall-related wrist fractures are among the most common fractures at any age. In order to learn more about the biomechanical factors influencing the impact response of the upper extremities, we studied peak hand reaction force during the bimanual arrest of a 3.4 kg ballistic pendulum moving toward the subject in the sagittal plane at shoulder height. Twenty healthy young and 20 older adults, with equal gender representation, arrested the pendulum after impact at one of three initial speeds: 1.8, 2.3, or 3.0 m/sec. Subjects were asked to employ one of three initial elbow angles: 130, 150, or 170

[Surgical treatment of para-articular ossifications after craniocerebral trauma].

This article reports on 45 cases in which extra-articular ossifications were removed following cranial-cerebral traumata. Radiologically detectable relapses occurred in 6% of the patients only; however, in none of these cases did the relapses lead to reankylosis of the joint. In the sagittal plane, the average gain in mobility was an increase of 84 degrees in the hip joint and of 100 degrees in the elbow joint. Considerable complications were caused by four fractures in extremely osteoporotic bones. To reduce the fracture rate, surgery should be performed as soon as possible after bone maturat
