Electrocorticography and stimulation.

Although acute electrocorticography (ECoG) is routinely used during epilepsy surgery there is little agreement as to its value nor criteria for its interpretation. Specific issues are reviewed on the basis of the literature and personal studies: does failure to resect the entire irritative zone prejudice seizure control, and are residual discharges predictive of failure; does activation of the ECoG by intravenous barbiturates provide information of clinical value; does intraoperative electrical stimulation help to improve localisation or avoid postoperative deficits; is the ECoG of value for m

A comparison of combined glottography in men and women.

Out-patient combined glottography has shown potential in the assessment of the vocal cord cycle in laryngeal abnormality. We compared this technique in men and women with normal laryngeal function, to evaluate the influence of patient sex on the results. Ten adult men were compared with 10 age matched women. The open quotient was significantly greater in women. This indicates the woman's glottis is open for a greater proportion of the glottic cycle, as a result of the anatomical and physiological differences of the larynx between the sexes. Analyses are made of the glottic cycle differences wh

Adaptor for continuous stimulation (SACS) with the WR-S8 monitor-stimulator.

An adaptor for continuous stimulation of the facial nerve (SACS) to be used with the WR-S8 Monitor/Stimulator during otologic surgery has been developed. This device allows the surgeon to use the electrified air drill and microsurgical instruments instead of a probe-tip during dissection. The SACS saves surgical time, is easy and convenient to use, and is safe. Routine monitoring of facial nerve function using SACS has helped prevent iatrogenic facial nerve injuries and has improved our ability to save the facial nerve during otologic and neurotologic surgery.

Chromatophore motor fields in the squid, Lolliguncula brevis.

Chromatophore motoneurones in Lolliguncula brevis are known to originate in the suboesophageal lobes of the brain and to project directly to the mantle and fin through bilateral stellate ganglia and fin nerves. The chromatophore motor fields of stellar and fin nerves were investigated by stimulation of the cut end of individual nerves in a semi-intact preparation. This elicited expansion of yellow and brown chromatophores in distinct motor fields. Brown chromatophores extended over the entire mantle, whereas yellow chromatophores were limited to the dorsal and lateral mantle areas. Combined ne

[Vagotomy and lower esophageal sphincter (LES) (author's transl)].

Selective proximal vagotomy does not influence the function of the cardia. Only high intrathoracic vagotomy results in a decrease of LES pressure. Esophagitis, which is found in a high percentage of preoperative duodenal ulcer patients, heals after SPV.

Neurostimulation of the celiac plexus.

Effective relief of abdominal pain lasting from 2 hours to 25 days was achieved in eight patients by neurostimulation of the celiac plexus. The procedure is judged to be superior to the nerve block.

Extra-cardiac stimulators: what do cardiologists need to know?

For several decades, treating patients with pacemakers has been the privilege of cardiologists. However, in the last 30 years, researchers have found new targets for electrical stimulation in different clinical subspecialities, such as deep brain stimulation (for the treatment of Parkinson's disease, essential tremor, dystonia, and some psychiatric illnesses); spinal cord stimulation (for refractory angina, chronic pain, and peripheral artery disease); and sacral (for diverse urologic and proctologic conditions), vagal (for epilepsy), and phrenic nerve stimulation (for sleep apnoea). The purpo

Sympathetic skin response. Glabella stimulation may be more useful than peripheral nerve stimulation in clinical practice.

The aim is to verify whether glabella electrical stimulation evokes sympathetic skin responses (SSR) without inter-side differences in latency and area of the responses and is more useful in mononeuropathies than peripheral nerve stimulation. SSRs were recorded in 25 healthy subjects from right palm, third (M3SSR) and fifth fingers and contralateral third finger. The inter-side differences of grand mean area and mean of largest area of M3SSR were significant only by ulnar nerve and not by glabella stimulation. Therefore glabella stimulation may be used in mononeuropathies comparing SSR area re

Direct evidence for a parietal-frontal pathway subserving spatial awareness in humans.

Intraoperative electrical stimulation, which temporarily inactivates restricted regions during brain surgery, can map cognitive functions in humans with spatiotemporal resolution unmatched by other methods. Using this technique, we found that stimulation of the right inferior parietal lobule or the caudal superior temporal gyrus, but not of its rostral portion, determined rightward deviations on line bisection. However, the strongest shifts occurred with subcortical stimulation. Fiber tracking identified the stimulated site as a section of the superior occipitofrontal fasciculus, a poorly know
