TachoSil® for prevention of postpancreatectomy hemorrhage after distal pancreatectomy: a propensity-weighted pilot study.

PURPOSE: Postpancreatectomy hemorrhage (PPH) is a rare but potentially lethal complication after distal pancreatectomy (DP), often precipitated by clinically relevant postoperative pancreatic fistula (POPF). While the fibrin sealant patch TachoSil® has been evaluated for POPF prevention, its role in mitigating PPH remains underexplored. METHODS: We conducted a single-center, retrospective cohort study of consecutive adult patients undergoing DP (2015–2024). Patients were classified by intraoperative use of TachoSil® in addition to standard stump closure. The primary endpoint was PPH, defined p

Usefulness of D-dimer Assay to Confirm the Course of Overt Venous Thromboembolism (VTE) in Cancer Patients.

Venous thromboembolism (VTE) is a serious complication in patients with cancer. In this population, the presence of thrombi is often assessed at cancer diagnosis by measuring D-dimer levels, which have high sensitivity but low specificity for identifying VTE at this clinical time point. However, the usefulness of D-dimer measurement during anticoagulation therapy has not been fully established, despite its widespread use. In this retrospective observational study, we investigated whether D-dimer measurement during anticoagulation therapy in cancer patients could predict overt VTE at follow-up.

Admission clinical features associated with disease severity in psittacosis pneumonia: A retrospective study.

OBJECTIVES: Psittacosis pneumonia caused by Chlamydia psittaci is often difficult to diagnose. Objective severity risk stratification at hospital admission is crucial for clinical management but remains challenging. This retrospective study aimed to evaluate admission baseline clinical characteristics for severity risk stratification in patients with psittacosis pneumonia. METHODS: We analyzed 60 patients diagnosed with psittacosis pneumonia, comprising 31 severe and 29 non-severe cases. All clinical and laboratory data were obtained from the initial evaluation within 24 h of admission. Firth

Impact of body mass index on D-dimer diagnostic utility for deep vein thrombosis in patients with cancer: a single-center retrospective analysis.

BACKGROUND: Deep vein thrombosis (DVT) is a common complication in cancer patients associated with significant morbidity and mortality. D-dimer is a widely used screening tool for suspected DVT; however, its accuracy may be influenced by body mass index (BMI). We evaluated whether BMI modified the diagnostic utility of D-dimer levels in patients with cancer and DVT. METHODS: We retrospectively analyzed 439 patients with newly diagnosed solid cancers suspected of having DVT between January 2013 and December 2020. DVT was confirmed or excluded by using computed tomography or echography. D-dimer

Comparison of D-dimer levels using coagulometer and immuno-nephelometer in Covid-19.

Covid is one of the most lethal viruses in the human respiratory system. Previous outbreaks of Covid19 include a severe respiratory problem (SARS) Cov2 and the Middle East (MERS) respiratory syndrome has recently been identified as one of the most serious health risks. D-dimer is a result of fibrin depletion which increases during a large thrombotic dose attributed to the selective activation of fibrinolytic structure. Measuring D-dimer levels is important in identifying and assessing risk of blood clots and related conditions such as deep vein thrombosis (DVT), pulmonary embolism (PE), and di

Evaluation of inflammatory-thrombosis panel as a diagnostic tool for vascular Behçet's disease.

OBJECTIVES: Vascular Behçet's disease (VBD) is prevalent in 40% of BD, but lacks laboratory biomarker for timely diagnosis. We aimed to establish a diagnostic panel for discerning VBD and non-VBD patients and identify hemostatic-thrombotic markers most related to VBD pathogenesis using machine learning algorithm. OBJECTIVES: A total of 338 BD patients comprising 123 VBD and 215 non-VBD were enrolled. Twenty-six clinical and laboratory features selected from LassoCV were included in multiple classifier to choose the optimal model for VBD differentiation. The Shapley Additive exPlanations (SHAP

Novel insights into pulmonary embolism with negative D-dimer results.

A patient in his mid-40s presented with exertional dyspnoea and pleuritic chest pain persisting for 6 weeks. Despite repeated normal investigations, including D-dimer tests, chest X-rays, serial troponins and ECGs, a CT pulmonary angiogram (CTPA) was performed to rule out pulmonary embolism (PE) or other pulmonary abnormalities, revealing a left main pulmonary artery thrombus with no evidence of right ventricular strain. The patient was managed with oral rivaroxaban 15 mg two times per day for 21 days followed by 20 mg once daily for 6 months. The patient showed full recovery at the 6-month fo

Assessment of Some Genetic thrombophilias in Nigerian patients with Venous Thromboembolism.

Venous thromboembolism (VTE) is a leading cause of mortality globally, resulting from genetic risk factors and/or acquired risk factors like oral contraceptives, smoking, diabetes, immobilization, cancer, trauma, fracture, and surgical procedures. This study aims to assess the involvement and prevalence of some genetic thrombophilias in Nigerian VTE patients. A total of 107 participants were recruited, comprising 67 individuals with VTE from three tertiary hospitals in Southwest Nigeria and 50 apparently healthy controls. Parameters included the absolute platelet count, prothrombin time (PT),

Optimizing resource allocation: Cost-effectiveness of specified D-dimer cut-offs in cancer patients with suspected venous thromboembolism.

An accurate diagnosis of venous thromboembolism (VTE) is crucial, given the potential for high mortality in undetected cases. Strategic D-dimer testing may aid in identifying low-risk patients, preventing overdiagnosis and reducing imaging costs. We conducted a retrospective, comparative analysis to assess the potential cost savings that could be achieved by adopting different approaches to determine the most effective D-dimer cut-off value in cancer patients with suspected VTE, compared to the commonly used rule-out cut-off level of 0.5 mg/L. The study included 526 patients (median age 65, IQ

Predictive value of D-dimer in the clinical outcome of severe COVID19 patients: Are we giving it too much credit?

BACKGROUND: COVID-19 is a new form of acute respiratory failure leading to multiorgan failure and ICU admission. Gathered evidence suggests that a 3-fold rise in D-dimer concentrations may be linked to poor prognosis and higher mortality. PURPOSE: To describe D-dimer admission profile in severe ICU COVID19 patients and its predictive role in outcomes and mortality. METHODS: Single-center retrospective cohort study. All adult patients admitted to ICU with COVID19 were divided into 3 groups: (1) Lower-values group (D-dimer levels < 3-fold normal range value [NRV] [500ng/mL]), Intermediate-valu
