Postoperative complications of distal humeral fractures. 27/96 adults followed up for 6 (2-10) years.

Of 96 adult patients with a distal intercondylar, intraarticular fracture of the humerus treated operatively during a 10-year period, 27 patients suffered a local complication, while the total number of individual complications were 34. There were six postoperative infections, 12 neural injuries, and 16 fixation failures. Because of the complication, a reoperation was performed in 12 of the 27 patients. The final outcome was assessed at a reexamination after a mean follow-up period of 6 (2-10) years. Three patients had a permanent dysfunction of the ulnar nerve. In another 3 patients, failure

The Use of the Fracture Table Post for Guidewire Placement in Hip Fracture Surgery.

Hip fractures, common among elderly patients, may result in significant disability and mortality. Fixation techniques often involve cannulated screws, sliding hip screws, or intramedullary nails. Optimal implant placement requires precise guidewire insertion from the lateral cortex into the femoral head. Variable femoral anatomy, particularly femoral version, can complicate guidewire placement despite fluoroscopy use, often requiring multiple screw passage attempts. The authors present a novel visual reference using a fracture table inguinal post to estimate femoral version and improve guidewi

Reduction Techniques for Calcaneus Fractures-Using a Femoral Distractor.

Displaced intra-articular calcaneal fractures disrupt hindfoot alignment and are associated with long-term morbidity. Although minimally invasive approaches reduce wound-related complications, they can limit reduction of a displaced calcaneal tuberosity. This report describes a 29-year-old man with a displaced intra-articular calcaneal fracture treated using a laterally applied femoral distractor to facilitate correction of coronal, sagittal, and axial malalignment while limiting soft-tissue disruption.

Surgical approaches, choice and positioning of implants in the surgical treatment of proximal tibia fractures in adults: a scoping review protocol.

INTRODUCTION: Tibial plateau fractures present a complex orthopaedic challenge and usually require surgical intervention to restore joint alignment and stability as well as to prevent complications. In practice, determining the most appropriate surgical approach, implant selection and positioning remains a challenge and depends on the surgeon's experience and patient-specific factors. Therefore, this scoping review aims to map the current evidence on surgical fixation methods for proximal tibia fractures in adults. In particular, we seek to first assess study types and their distribution acros

Variation in surgical indications across national distal radius fracture guidelines: A comparative review.

AIMS: Surgical indications for dorsally displaced distal radius fractures (DRFs) are guided by national guidelines, yet variation in clinical practice persists. This study aimed to compare contemporary national guidelines, examine differences in recommended intervention thresholds, and assess their potential impact on treatment decisions and resource utilization. METHODS: A structured search was conducted across biomedical databases, clinical knowledge platforms, guideline repositories, and national health agency websites. National guidelines were included if they provided surgical recommenda

Increased stiffness with medial column screw supplementation of lateral locking plate for distal femur fractures: a biomechanical study.

INTRODUCTION: We propose and assess the biomechanical stability of medial column screw supplementation in a synthetic distal femur fracture model. MATERIALS AND METHODS: Twenty-four low density synthetic femora modeling osteoporotic, intraarticular distal femur fractures with medial metaphyseal comminution were split into two fixation groups: (1) lateral locking distal femur plate (PA- plate alone) and (2) lateral locking distal femur plate with a 6.5 mm fully threaded medial cannulated screw (PWS- plate with screw). Cyclic biomechanical testing included 5 steps of 10,000 cycles with each ste

The Biomechanical Behavior of Fixation Options in Short Segment Radius and Ulna Diaphyseal Fractures.

PURPOSE: Plate fixation of radius and ulna shaft fractures has evolved due to a more complete understanding of the biology and mechanics. Screw fixation across six cortices has been adopted as the standard, purported to provide the optimal biological and mechanical environment for healing; however, recent investigations have proposed that four cortices of fixation may be sufficient. These recommendations are intended for segments of optimal length; however, it is less clear how these options perform in short segment fixation. Our objective was the determine the biomechanical behavior of fixati

Bioelectronic osteosynthesis plate to monitor the fracture bone healing using electric capacitive variations.

BACKGROUND: Bone fractures represent a global public health issue. Over the past few decades, a sustained increase in the number of incidents and prevalent cases have been reported, as well as in the years lived with disability. Current monitoring techniques predominantly rely on imaging methods, which can result in subjective assessments, and expose patients to unnecessary cumulative doses of radiation. Besides, they are costly and incapable of providing continuous daily detection of fracture healing stages. Technological advances are still required to design fixation systems with the ability

Biomechanical Evaluation of the Risk of Subtrochanteric Fracture After Femoral Neck Fixation in a Synthetic Model: The Femoral Neck System versus Cannulated Screws.

OBJECTIVES: The Femoral Neck System (FNS) is a fixed-angle side plate device approved for use in fixation of femoral neck fractures. The FNS perforates the lateral cortex of the subtrochanteric femur, which may increase the risk of postoperative subtrochanteric fractures compared with the inverted triangle cannulated screw (CS) construct. The purpose of this biomechanical study was to compare forces required to create subtrochanteric fractures in FNS and CS constructs in a synthetic bone biomechanical model. METHODS: Osteopenic sawbones were divided into five groups: Noninstrumented femurs, C

Distal Femur Fracture Above a High Below-The-Knee Amputation Managed With Lateral Locking Plate: A Case Report.

CASE: We report the case of a patient with a below-the-knee amputation who sustained a complex femur fracture ipsilateral to his residual limb after a fall from height. His fracture was managed with a lateral locking plate of the distal femur for fixation. He was followed for 16 months; at 7 months, he demonstrated return to pretrauma functional status. CONCLUSION: Fixation of the distal femur with a lateral locking plate did not interfere with the patient's ability to wear a prosthesis and should be considered for managing "periprosthetic" femur fractures in those with a below-the-knee amput

Digitally driven surgical guide planning.

To investigate the role of a fully digital process in the surgical treatment of mandibular fractures in children. We analyzed a complete dataset from 22 children with mandibular fractures treated with digital surgical assistance. The patient's treatment process included preoperative thin layer CT (Computed Tomography) scanning, computer-aided design (3D reconstruction, virtual reduction, and internal fixation device determination and shaping), and 3D printing (jaw model, bite plate). We used occlusal and shaping plates during surgery to assist in fracture reduction and fixation. During the fol

[Bilateral Dorsal Fracture-Dislocation of the Proximal Humerus].

Fractures of the proximal humerus constitute approximately 5% of all fractures. Shoulder joint injuries without any external mechanical impact during seizures with the occurrence of spasms occur only sporadically. The occurrence rate is reported in approximately 0.4% of patients. Very rarely they occur in the form of epileptic seizure-induced dorsal fracturedislocation impacting both sides. The case report describes a case of a 48-year-old woman with no treatment for epileptic seizures in her medical history. During the first seizure she sustained a bilateral dorsal fracture-dislocation caused

Cell-Based Therapy by Autologous Bone Marrow-Derived Mononuclear Cells for Bone Augmentation of Plate-Stabilized Proximal Humeral Fractures: A Multicentric, Randomized, Open Phase IIa study.

Proximal humerus fractures are common in an aging population. The standard operative treatment is open reduction internal fixation (ORIF) using an angular stable plate. However, this procedure has complications such as a relatively high rate of secondary dislocation, humeral head necrosis or nonunion caused by delayed bony consolidation. Autologous bone marrow mononuclear cells (BMC) combined with a β-TCP scaffold could support bone healing and is considered clinically safe. This multicentric, randomized, open phase IIa clinical trial (Clinical Trials. Gov Identifier: NCT02803177, Eudra CT No:

[Long-term anticoagulation treatment in proximal femoral fractures as obstacle to surgery in the 24h time window? : Results of an analysis of Quality Assurance data on 32,252 treatment courses with osteosynthesis].

BACKGROUND: Both in Germany and internationally there is a vehement controversy about the appropriate time for care of proximal femoral fractures in older patients. The effort to achieve high quality and uniform standards of care culminated in the German healthcare system in the strict requirement of delay-free surgery within 24 h. Until now, in view of their high vulnerability patients who were severely injured were too often operated on late with the reference to a general medical condition that could be improved preoperatively. In particular, the fear of complications due to a pre-existing

Robot-Assisted Percutaneous Lag Screw Osteosynthesis for C1 Lateral Mass Fractures: Case Series and Technical Note.

BACKGROUND: C1 lateral mass fractures (LMF) cause abnormal alignment of the upper cervical joints. Conservatively treated cases can develop into late cock-robin junction, requiring a reconstructive surgical procedure of the occipitocervical junction. Partial coronal C1 LMF could be effectively fixed with lag screws. Navigation and robot-assisted techniques have made percutaneous fixation possible and are gradually being used in the upper cervical spine. METHODS: Five consecutive patients with C1 LMF who underwent percutaneous lag screw osteosynthesis under the guidance of a new robotic system

Management of complex pilon fractures: Is it necessary to fix all the columns in AO/OTA type 43-C fractures?

INTRODUCTION: Surgical treatment of AO/OTA 43-C pilon fractures has not yet taken a standard form. We aimed to evaluate whether patients that appeared to be labelled as unsupported columns according to the four-column theory would affect long-term clinical and radiological outcomes. MATERIALS AND METHODS: Patients with AO/OTA 43-C type who were treated surgically between January 2010 and December 2019 were included in the study. 53 fractures in 52 patients who received osteosynthesis were categorized based on the fixed columns. These groups were formed as fully supported columns (FSC- Group A

If at first you don't succeed, should you try again? The efficacy of repeated closed reductions of distal radius fractures.

INTRODUCTION: A repeated closed reduction ("re-reduction") of a displaced distal radius fracture is a common procedure performed to obtain satisfactory alignment and avoid surgery when the initial reduction is deemed unsatisfactory. However, the efficacy of re-reduction is unclear. Compared to a single closed reduction, does a re-reduction of a displaced distal radius fracture: (1) improve radiographic alignment at the time of fracture union and, (2) decrease the rate of operative intervention? MATERIALS AND METHODS: Retrospective cohort analysis of 99 adults aged 20-99 years with extra-artic

Analysis of fracture-related infections from Swedish insurance claims between 2011 and 2021.

Fracture-related infections (FRI) pose a serious complication with an incidence of 1-2%. This study aimed to analyze compensation claims submitted to The Swedish National Patient Insurance Company (LÖF) because of FRI after closed/open reduction and internal fixation (C/ORIF) in the four most common fracture sites (proximal humerus, distal radius, hip, ankle). Patients registered in the LÖF database with a suspected FRI between 2011 and 2021 were identified by matching International Classification of Diseases and procedural codes indicative of a combination of fractures to the proximal humerus

Proximal fibular stress fracture in adolecent soccer player. A case report.

Fibular fractures are the third most common stress fractures in children and adolescents. Proximal fibular location is a very rare finding, with few reports in the literature and, frequently, careful investigations before a definitive diagnosis could be necessary. The authors report a case of an adolescent 13 years old soccer player with a proximal fibular fracture that was initially underestimated and misdiagnosed and ultimately confirmed as a stress lesion by MRI.
