Interdisciplinary approach to treat dyskeratosis congenita associated with severe aplastic anemia: a case report.

This paper reports on a 4-year-old male who had dyskeratosis congenita and who acquired severe aplastic anemia. The patient developed hyperpigmentation of the face, neck and chest region, arms, shoulders and legs. In addition, he had dry skin, deformed fingernails and toenails, sparse hair and eyebrows and hyperkeratosis of the dorsum of the hands and feet. Laboratory and histological analysis revealed severe pancytopenia and dyserythropoiesis of red blood cells, hypocellularity of white blood cells and decreased megakaryocytes with dysplasia. The intraoral examination identified bleeding gums

[Phlegmon of the forearm due to therapy with Anakinra (interleukin-1 receptor-antagonist)].

INTRODUCTION: Biological modifiers (BM) are very effective in a high percentage of patients with rheumatoid arthritis being resistant to disease modifying antirheumatic drugs (DMARD). In the beginning, the therapy with the BMs was thought to be of low risk. However, due to the increase of application, a few lethal septical complications have been reported. CASE REPORT: A 38 year old male with rheumatoid arthritis and insulin deficiency diabetes mellitus achieved remission under the therapy with Anakinra within three days. Due to a spontaneous disrupture of the finger extensor tendons operativ

Wrist arthroscopy in distal radial fractures using a modified horizontal technique.

Wrist arthroscopy has today become an important adjunct in the management of displaced intra-articular distal radial fractures, with reduction of joint incongruencies as well as detection and treatment of associated soft-tissue injuries. However, standard upright arthroscopy makes it difficult to combine arthroscopic-assisted reduction with additional treatment of the often comminuted, extra-articular fracture component. This article describes a modified arthroscopic technique in which the arm is blocked in pronation with the traction horizontally over a handle on a normal hand table, without

Lunate plasty for Kienböck's disease: use of a pedicled vascularised radial bone graft combined with shortening of the capitate and radius.

We treated eight patients with Kienböck's disease (two patients each with stage 1, 2, 3a and 3b disease by Lichtman's classification) by removing a pedicled, vascularised bone segment from the dorsal aspect of the distal radius and engrafting it into the lunate. Additional shortening of the radius was performed in patients with the ulna-minus or null variant. Shortening of the capitate and capito-hamate fusion were also performed in patients with stage 3 disease. All patients were relieved of their wrist pain at rest and during movement, and the mean grip strength increased from 37% of that in

Retrospective analysis of surgical outcome, symptom changes, and hearing improvement following myringoplasty.

This study examines the surgical and audiologic results of myringoplasty and what factors affect these outcomes. It also examines whether patients experience any improvement in their main symptoms following surgery. This was a retrospective analysis of case notes, patient questionnaires, and audiograms of all patients undergoing myringoplasty between 1990 and 1995 at Walsgrave Hospital, Coventry, UK. Overall success of myringoplasty for consultants was 81% and for trainees was 79%, with an overall success rate of 86% in children. Posterior and inferior perforations had a 90% success rate for r

First European exposure to syphilis: the Dominican Republic at the time of Columbian contact.

Recognition of syphilis in Europe in the late 15th century and its prior absence suggest New World origin. Skeletal populations were examined from sites with documented Columbian contact in the Dominican Republic. Examination of 536 skeletal remains revealed periosteal reaction characteristic of treponemal disease in 6%-14% of the afflicted population. Findings were identical to that previously noted in confirmed syphilis-affected populations and distinctive from those associated with yaws and bejel: it was a low population frequency phenomenon, affecting an average of 1.7-2.6 bone groups, oft

Decapitation as a result of suicidal hanging.

A report is presented on a 47 year old man who committed suicide by hanging himself from a staircase bannister of an apartment house. The man, weighing 144 kg jumped with the noose of a 2 cm thick and 2 m long hemp rope around his neck and was completely decapitated. Death from typical "normal" suicidal hanging is usually due to cerebral ischaemia caused by compression of the carotid (and vertebral) arteries. Except for bleeding at the clavicular insertions of the sternocleidomastoid muscles there are only occasional injuries to the cervical soft parts or hyoid bone and/or laryngeal cartilage.

Diagnostic ultrasound of the hand and wrist.

To assess the efficacy, role, and limitations of diagnostic ultrasound in the hand and wrist, the results of 98 examinations performed for a variety of surgical conditions were retrospectively analyzed. Ultrasound was shown to be reliable in evaluating radiolucent foreign body, tendon rupture versus tendon adhesion, tendinitis, peritendinitis, and ganglion cyst (specificity, 1; positive predictive value, 1). A correct suggestion of soft tissue mass histology was offered in six of eight operated cases. Tumor size and extent was accurately assessed in all but one case. The observed limitation of

Microsurgery in children.

Advances in microsurgical techniques have permitted replantation and reconstructive procedures to restore amputated or injured parts with a high degree of success in children. Despite this progress, some technical concerns remain. During a 15-year period, replantations were done in 53 children with major limb (18), hand (10), or digit (25) amputations. Even in patients with crush or avulsion injuries, the results suggest that replantation procedures are justified considering the superior recuperative ability in children. Free tissue transfer was used for reconstructive procedures in 26 childre

[Free transfer in the emergency treatment of complex injuries of the arm. Apropos of 18 cases].

In 18 cases the authors used a free tissue transfer during the first 24 hours of the treatment of a complex trauma of the upper limb involving osteoarticular and neurovascular loss combined with loss of skin cover. The various flaps include: lateral arm flap (8 cases), latissimus dorsi flap (4 cases), toe to hand transfer (4 cases), radial forearm flap (2 cases). The series includes 2 failures salvaged without further sequelae by means of a groin flap. With regard to complex one-stage reconstructions, particularly those requiring replacement of bone, the outcome is improved if the selected met

Ergonomic hazards in the workplace: assessment and prevention.

1. Ergonomic hazards impact employers and workers and their families. Poor workplace design, awkward body mechanics or postures, repetitive movements, and other ergonomic hazards induce or contribute to a staggering number of cumulative trauma disorders. 2. Cumulative trauma disorders (CTD) affect hands, wrists, elbows, arms, shoulders, the lower back, and the cervical spine area. Structures involved include tendons, muscles, bones, nerves, and blood vessels. One can plan strategies for abatement by learning to recognize the hazards that contribute to CTD. 3. OSHA has published the Ergonomic P

Therapeutic use of gonadotropin-releasing hormone agonists in polycystic ovarian syndrome.

Polycystic ovarian syndrome is a heterogeneous disorder characterized by abnormal function of the hypothalamic-pituitary-gonadal axis, excessive production of androgens, aberrant intermediary metabolism, and structural changes in ovarian morphology. Long-term administration of the GnRH agonistic analogues brings about pituitary gonadotroph down-regulation after an initial period of stimulation of FSH/LH release. The resulting decrease in LH output and ovarian androgen production exerts beneficial effects on the clinical and biochemical parameters of the PCO syndrome. Ovarian volume and stroma

Aural symptoms as primary presentation of Langerhan's cell histiocytosis.

Langerhan's cell histiocytosis is an uncommon granulomatous disease, characterized by the idiopathic proliferation of Langerhan's cells or their marrow precursors. It encompasses the diseases previously associated with histiocytosis X-eosinophilic granuloma, Hand-Schuller-Christian syndrome and Letterer-Siwe syndrome. A series of 54 patients were diagnosed with this condition in Dublin over a 33-year-period (1959-1992). Twenty-seven patients had aural symptoms, of whom 15 had no other lesions at the time of presentation. Otorrhoea was the most frequent otological symptom, followed by lesions i

[Preliminary experience of the study of the hand using digital radiology with photo-excitable phosphors and direct magnification. Comparison with the traditional high resolution technique].

Photostimulable phosphor computed radiography (CR) is a new technique. The authors applied direct magnification CR to hand study (IP matrix: 1770 x 2370), and compared the results with conventional radiography ones (high MTF industrial type film, without screens, 5 x optic enlargement reading). In both cases, a mammographic unit was employed for acquiring the images. Thirty patients affected with renal osteodystrophy, psoriatic arthropathy or rheumatoid arthritis, and 20 normal subjects were examined. Digital images were processed by a particular sensitometry curve and spatial filtering with u

Acute transformation of chronic myelomonocytic leukaemia: a multivariate study of predictive factors.

In an attempt to determine the possible predictive value of the main clinical and haematological initial features of chronic myelomonocytic leukaemia (CMML) on the evolution to acute leukaemia, as well as the real impact of such an event on survival, 35 such patients were submitted to multiple regression analyses. At the time of the study 30 out of the 35 patients had died, with a median survival of 8.2 months for the whole series. 12 patients (34%) developed acute leukaemia, between 1.5 and 42.1 months from diagnosis of CMML, the actuarial median time of acute transformation being 29.4 months

Arachnodactyly and unusual dermatoglyphics: study of a case.

Clinical and dermatoglyphic findings are reported on a 3-yr old girl with multiple congenital anomalies and unusual dermatoglyphics. The anomalies, including contractural arachnodactyly, rhizomelia (a relative shortening of the proximal segment of the limbs), skin dimples, clinodactyly, disharmonic hand bone maturation, absent, hypoplastic and unusually positioned digital and metacarpophalangeal flexion creases, are not indicative of Marfan syndrome, but it is unclear what this syndrome constitutes. Among the child's most striking dermatoglyphic features, the fingertip patterns (mostly large w

[Experimental arthrosis caused by chemical cartilage damage].

In experiments on rabbits the authors studied the dynamics of cartilage degeneration and subsequent pseudorepair processes following what was doubledose chemical damage to articular cartilage. In addition to the classical histological methods they used the histochemical demonstration of hydrolase and dehydrogenase which are of prime importance to cartilage. The intraarticular instillation of sodium iodacetate caused major and irreversible damage to the whole cartilaginous layer. Subsequent pseudorepair proceeded on the one hand from the cells of the peripheral synovial membrane and on the othe

Histologic and clinical responses to porous hydroxylapatite implants in human periodontal defects. Three to twelve months postimplantation.

Twelve intrabony periodontal lesions in three volunteers received surgical debridement followed by site implantation of porous hydroxylapatite implants. These patients were followed over a total of a 1-year observation period. Blocks of treated sites were surgically removed at 3 months, 6 months and 12 months after implantation. Clinical observation indicated a reduction in pocket depth consisting of both recession and clinical gain of attachment. No ill effects were observed. Histologic examination of the treated sites showed ossification of the implant pores and the implant periphery as earl

Thorotrast-induced hepatic angiosarcoma with 39 years latency. A pathologic and immunohistochemical study.

An autopsy case of hepatic angiosarcoma which occurred in a 64-year-old man who had been examined by Thorotrast angiography 39 years previously, is reported together with the pathologic, radiologic and immunohistochemical findings. Thorotrast deposits were found in the spleen, bone marrow, right elbow joint and lymph nodes as well as the liver. The angiosarcoma was accompanied by markedly distended sinusoids with hypertrophic and hyperplastic endothelial cells, which were considered to be closely associated with angiosarcoma. The results of immunostaining indicated that only a very limited num
